JOB DESCRIPTION
The primary purpose of this position is to directly supervise charge nurse and team leaders, resident aides. Supervises nursing assistants.
Monday - Friday, Nights; Every Other Weekend
Responsibilities
- Supports and enhances the philosophy of the nursing department.
- Creates a harmonious work environment, maintains effective communication within the nursing department and between nursing and the other departments for the purpose of problem solving, updates general information.
- Promotes an atmosphere that is sensitive and responsive to needs of staff and residents, and families.
- Serves as a clinical resource person to NurseManagers/charge nurses and staff in problem solving, development of nursingcare plans and resident assessments.
- Assumes clinical duties as charge/staff nurse as indicated.
- Complies with regulatory agencies and policies and procedures. Promotes and facilitates implementation of policies on units.
- Responsible for communicating pertinent resident, staff, and facility information to the oncoming supervisor.
- Promotes continuity of care and compliance with comprehensive care plan for residents.
- Responsible for coordinating appropriate emergency care, intervention and transportation of the residents, coordinating with the primary physician, medical director and nursing administration.
- Update administration, DON, ADON and Medical Director as needed for facility /resident concerns or problems.
- Assumes responsibility for professional growth and development and promotes staff development.
- Assumes responsibility for operations of nursing department in the absence of DON, ADON with coordination from administration as appropriate.
- Completes long and short term staffing needs for nursing to ensure adequate and safe staffing for all units, communicating changes as per policy.
- Reviews staffing and maintains follows minimum standards canceling agency staff as necessary.
- Participates in development, promotion, interpretation and implementation of standards of care.
- Monitors and follows up on abnormal lab test results and coordinates intervention with NP/MD and Nurse Manager.
- Serves on committees as requested.
- Coordinates with nursing staff, resident admission/discharge and assists staff as needed.
- Provide input on nursing personnel performance.
- Assists administration and regulatory agencies with resident and staff investigations.
- Monitors personnel attendance, counsels and conferences staff, maintaining contact with DON, and Nurse Manager.
- Respects confidentiality of residents, departments and nursing home information.
- Safety rules and procedures are followed. Safety hazards and unsafe acts are reported and corrective action taken.
- Follows up on all incidents and accidents. Completes incident and accident reports per policy and notifies appropriate parties.
- Utilizes personal protective equipment, engineering and work practice controls to prevent/minimize occupational exposure to blood and other infectious materials.
- Follows infection control policies and procedures.
- Assists Nurse Managers and MDS coordinator with MDS and PRI documentation.
- Assists with nursing staff orientation as needed and makes recommendations to staff development coordinator/DON and Human Resources.
- Performs additional job-related duties as necessary within scope of responsibility.

The Patient Care Coordinator is an on-site clinical team member who assists in coordinating care for the assigned team. + Monitors lab results, following up with physicians and team members.
+ Covers from other Patient Care Coordinators when needed.
+ Participate in interdisciplinary team conferences as needed.
+ Performs medication reconciliation on all therapy only cases as needed
+ Monitors admissions, discharges and re-hospitalizations and communicates with the appropriate clinical liaison(s) or facilities to facilitate continuity of care between settings.
+ Aids clinical staff in follow up communication with MD offices and referral sources as applicable.
+ Coordinates all incoming orders associated with census to ensure appropriateness and accuracy and timeliness of care, enters into Medical Record, and coordinates with scheduling as appropriate.
+ Assists in document control processes, including but not limited to, review of Face-to-Face documentation and physician orders for home health. Reports problems immediately to the Clinical Manager.
+ Triages and directors all incoming communications during workday hours for the current census assigned and follows up to facilitate appropriate service delivery.
+ Current Registered Nurse License Required
+ One-year recent home health experience preferred.
+ Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify appropriate resource for hand off.
+ Must have requisite knowledge of current Home Health Medicare, Medicaid, State and Federal regulations and Joint Commission accreditation standards. Must be able to demonstrate problem-solving abilities and have excellent telephone, interpersonal and written communication skills.
+ Strong commitment to teamwork and customer service, both internally and externally, is required.
+ Must have strong/proven data entry and general computer skills.
+ Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty. Maintains a valid driver's license
