The Patient Care Associate, under the direction of a licensed nurse or other medical staff, in the health care setting assists the professional staff by performing, veni-puncture, finger-sticks, HIV Pre and Post Test Counseling, Rapid HIV Testing, responding and assisting with emergencies, facilitating telehealth encounters, various clerical and health-related duties associated with patient care (ADLs as indicated) and contributing to the safety, care and comfort of patients. The duties are assigned by and are performed under the supervision of a Registered Nurse. The Patient Care Associate may be required to perform the duties listed below and other duties involving same type, area of responsibility. Patient care associates also observe their patient's mental, emotional and physical conditions and report any changes to the nursing staff.
Responsibilities:
1. Facilitate Telehealth encounters as needed
2. Administer total care (bed bath, partial or complete)
3. Assist with patient's personal hygiene - including skin care, mouth care,
shaving, etc.
4. Transfers patients - in and out of bed, stretcher and wheel chair. Including
physical and mechanical lifting
5. Positions patients and carry out range of motion exercise

transfer patients. PCAs may face hazards from exposure to chemical and infectious
diseases. In addition, they have to treat patients that are confused, irrational, agitated

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City's efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.
Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management.
The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Job Description
Under general supervision, provides a wide range of professional nursing services to students, staff and others in support of a college health service program; assists in the planning and implementation of college health service activities through cooperation with appropriate medical authorities; monitors the effectiveness of program activities and services; provides training to staff and others pertaining to first aid, child care, and other health practice areas; serves as a resource regarding student health management strategies, problems and concerns; and performs related work as assigned.
DISTINGUISHING CHARACTERISTICS
This is the first full working level class in the Health Services Nurse Series within the Yosemite Community College District (YCCD). An incumbent of this class must be a Registered Nurse in the State of California, but duties do not require possession of a Public Health Nurse Certificate. Incumbents at this level provide a wide range of standard nursing services typically associated with a health clinic setting.
This class may be distinguished from the higher-level class of Health Services Senior Nurse because incumbents in that class, in addition to having more patient care experience, must be licensed as a Registered Nurse in California and must possess a valid California Public Health Nurse Certificate. Based on required education and experience, a Health Services Senior Nurse is expected to coordinate and perform the most complex nursing duties that require knowledge of public health nursing issues as they apply to an educational institution.
TYPICAL WORKING CONDITIONS
Work is generally performed in an indoor health care clinic setting. Travel between campuses may be required.
SUPERVISION RECEIVED AND EXERCISED
Incumbents in this class work under the general supervision of a manager or senior manager, working alone on routine or regular work assignments and checking with a supervisor on non-routine assignments or when in doubt as to the correct procedures to follow.

Licenses and Certificates

POSITION SUMMARY
The Health Services Administrator is responsible for effectively and efficiently managing the institution’s overall health care delivery system and monitoring all health service contract activities; consulting with the Director, Jail Operations and Manager, Jail Operations regarding routine institutional issues of an administrative nature, and discussing issues of a complex or unusual clinical nature involving patient management with the Site Medical Director and providers.
JOB REQUIREMENTS

LICENSING: Current Louisiana RN license
CERTIFICATION: Current CPR Certification
EDUCATION: Associates degree in a healthcare-related field required. Bachelor's degree preferred.

JOB SUMMARY AND QUALIFICATIONS:
To assist service recipients with health related activities that the service recipient would self- manage if able. Under the direction and supervision of the Sr. Director. The LPN HSC will provide clerical and office support in maintaining health care services and insuring that all documentations and medical supply needs of the persons supported are provided. The LPN HSC will work closely with the Contracted Oversight RN.
WHAT YOU SHOULD HAVE FOR THIS ROLE:
The position requires current license as a Licensed Practical Nurse in the State of Kentucky and at least two years of nursing experience preferably working with persons with intellectual and developmentally disabilities or in a home health environment. Must have knowledge of the rules, policies and procedures, and guidelines established by the State of Kentucky for community support programs for individuals with intellectual and developmental disabilities, and be able to complete required DIDD training.
RESPONSIBILITIES:
1. Ensure that all health related information is entered into practical health record system
2. Maintain all appropriate paper files relative to healthcare
3. Coordinate the scheduling of all medical appointments and follow up. Ensure that all appointments are kept or rescheduled.
4. Follow up on all referrals and recommendations from the health care professionals.
5. Ensure that all person supported records meet DIDD requirements.
6. Review and sign off on all medication variances weekly.
7. Monitor and maintain medication refill request and order supplies as needed.
8. Coordinate AT equipment. Bed, wheelchairs, hoyer lift repairs and ensure AT clinic packet is completed.
9. Filing of all medical documents and encounters in chart daily
10. Ensure that all consents are updated and uploaded to the person supported file
11. Review, Complete and upload all encounters in EHR
12. Thinning charts of all persons supported and storing them according to office procedure
13. Monitor Lab tracking, request copies for file, insure that all ordered labs are completed.
14. Track all annual physicals, dentals and vision exams and ensure they are completed within the DIDD guidelines.
15. Work in the homes at least 16 hours a month or as needed.
16. Be available and on call when the Case Manager is on Vacation or unavailable.
17. Coordinate (as needed) between and among health care professionals, such as Primary Care Physician, CSN Case Mangers, and medical specialist, i.e. (cardiologist, orthopedists, psychiatrist, etc.) to assure the effective flow of information.
18. Performance quality improvement (PQI) duties and reports as assigned by supervision & PQI committee.
19. Any other job duties as assigned by supervision.
