**Job Summary and Qualifications**
The Manager Case Management reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department. The Manager for Case Management is accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team. This position also functions as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the case management team. The Manager for Case Management supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with hospital policies, and applicable regulations and guidelines. The Manager for Case Management monitors quality of the interactions, documentation and assessments of the case management staff. The Manager for Case Management coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
**What qualifications you will need:**
+ Bachelor's Degree in Nursing OR Licensed Clinical Social Worker with Master's Degree in Social Work required
+ 3+ years experience in clinical nursing in an acute care setting OR hospital social work required
+ 2+ years of supervisory/leadership experience preferred
+ InterQual experience preferred
+ Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation OR currently licensed as a LCSW or a LMSW in the state(s)of practice according to law and regulation required

- Claim Assessment and Quantification: Analyze financial records and supporting documentation to determine the validity and value of disaster-related claims. Molina Pharmacy Services/Management staff work to ensure that Molina members, providers, and pharmacies have access to all medically necessary prescription drugs and those drugs are used in a cost-effective, safe manner. The MTM Pharmacy Service Representative is responsible for performing outreach attempts and handle inbound calls for our Molina Healthcare members. Duties include, but are not limited to, transferring members to internal departments for medication reviews, scheduling appointments, and acting as a liaison between other departments.
**Job Duties**
+ Handles and records inbound/outbound pharmacy calls in required systems from members, providers, pharmacists, pharmacies, and technicians to meet departmental and CMS standards.
+ Enforce HIPAA compliance standards and regulations while maintaining guidelines and other necessary information for our providers, members, technicians and pharmacists.
+ Provides clerical services and support to the pharmacist and technicians staff with day-to-day operations as delegated.
+ Follow policies and procedures for appropriate call resolution or transfer to internal departments as required
+ Utilize appropriate resources to ensure members contact information is current and up to date.
+ Effectively communicate general Medicare and Medicaid plan benefits to existing members.
