Become the Case Manager You've Always Wanted to Be

  • Participate in the case management on-call schedule as needed.

Qualifications

License or Certification:

  • Must be qualified to conduct assessments within your discipline (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).
  • If required, must hold a valid license for your discipline.
  • Must be eligible for CCM® or ACM certification upon entry OR within two years of entering the role.
  • CCM® or ACM certification required OR must be obtained within two years of placement in Case Manager II position.

Minimum Qualifications:

  • For Nursing, an Associate Degree in Nursing is required; RN licensure with BSN preferred. A diploma is acceptable in states where it is the minimum licensure requirement.
  • For other licensed or certified healthcare professionals, a minimum of a bachelor’s degree is needed, with a graduate degree preferred.
  • Two years of rehabilitation experience is preferred.
