This role is key to improving maternal health by ensuring patients have access to the resources and services they need during pregnancy and postpartum. This position supports patients, families, and caregivers in overcoming social barriers within the healthcare system.

What You'll Need To Have:

Bachelor’s Degree in Social Work, or related Social Services field required

Experience in a medical or clinical setting preferred.

Background working with expecting mothers or postpartum patients is a plus!

GENERAL SUMMARY:
Assists physicians with examinations and procedures to serve the medical needs of patients with various health conditions in a professional, confidential, and caring manner. 2. *Maintains flow of patients in accordance with time of appointment in order to avoid delays for the provider and undue waiting for patients.
3. *Collects subjective and objective data and contributes to the assessment of the patient; documents appropriately in the patient chart.
4. *Makes accurate and timely observations and documents changes in the patients condition in the patient chart.
5. *Retrieves messages and responds to calls from patients, pharmacies, hospitals, nursing homes and other medical offices by use of telephone and computer in a timely manner as delegated by licensed heath care provider (MD, PA, NP).

EDUCATION AND EXPERIENCE:
