The actual base salary offer for this position will be determined by a variety of components including but not limited to work experience, education, certifications, location of the role, internal equity, and other relevant factors.LOCATION: Minneapolis, MN (Office for your Day or Work from Home) Position Description As a Care Manager - Maternal Health you will provide comprehensive, culturally responsive care management for high-risk pregnant and postpartum members, addressing medical, mental health, substance, social, and environmental needs. You will work across the continuum of care to support member engagement, promote healthy, full-term pregnancies, and reduce avoidable NICU admissions and emergency department visits. Collaborating with providers, counties, community agencies, and internal teams to develop and implement individualized care plans, identify and address barriers and ensure timely access to prenatal and postpartum services. You will play a critical role in advancing maternal health equity and improving outcomes through proactive outreach, education, and resource connection. Provide member-centered telephonic care management for members across the continuum of pregnancy and childbirth addressing medical, Mental Health (MH), and/or Substance Use Disorders (SUD) conditions that focuses on connecting UCare members to appropriate services and resources. Provide education on pregnancy, baby care, breastfeeding, and recovery post-delivery. Understand and accurately interpret and apply relevant contractual requirements, policies, procedures and regulations for care management. Partner with members and/or family members, primary care providers, clinic staff and other relevant agencies to assure appropriateness of service that meets member needs and supports desired outcomes. Complete assessment of assigned members. Monitoring health condition of the mother and the baby during pregnancy and postpartum stages. Collaborate with member/caregiver in the development of a care plan appropriate to the member's needs. Ensure care plan goals are measurable, specific and timebound. Monitor high-risk pregnant and postpartum members. Identify gaps in care, coordinate services, and support access to prenatal, postpartum, and preventative care in order to promote health birth outcomes. Enter member information in UCare's clinical documentation software. Ensure documentation is accurate, thorough, and timely. Meet and maintain all established caseload and performance metrics. Attend internal and external meetings; may serve as team representative on cross departmental meetings as assigned. Assist with CMS Star Rating/HEDIS initiatives and project improvement planning, as appropriate. Other projects and duties as assigned.Education Registered Nurse (BSN preferred). Current and unrestricted RN license in Minnesota required. Required Experience At least two years clinical experience working with birthing people and two years' experience in OB/GYN care coordination/case management across the continuum of health care (hospital, clinic, home care etc.). Emphasis on working with at risk, diverse populations. **Job Summary**
Join a talented group of more than 75 dietitians at the University of Virginia Health System in beautiful Charlottesville, Virginia. At this level one trauma center, dietitians play an integral role on multidisciplinary teams in many specialty areas, both in the 675 bed inpatient setting, as well as in over 35 outpatient clinics. RDs participate in research and quality improvement projects, host dietetic interns, and are supported in pursuing leadership and community service experiences. Continuing education opportunities are plentiful from within the accomplished nutrition team and from the health system which values ongoing training and education.
This full-time, 40 hour/week role will provide medical nutrition therapy at our Diabetes Education and Management Program (DEMP) and in maternal health at our OB Clinic. 24 hrs/week will be spent at DEMP, with 1 day per week at our Orange location and the remainder in Charlottesville. At DEMP, you will collaborate closely with nurses, endocrinologists, and other healthcare professionals to provide comprehensive care to patients with type 1, type 2 diabetes, and gestational diabetes in 1:1 sessions and in group classes. During the remaining 16hrs/week, this role will work closely with providers in our OB Clinic. Typical patients include those seeking support and guidance for gestational diabetes, pregnancies of multiples, pregnancy after bariatric surgery, and general health eating during pregnancy. The RD will work to establish strong relationships with referring providers, develop educational resources, mentor dietetic interns, and create and contribute to quality improvement projects. This position would include some remote work. CDCES preferred, however new RDs are encouraged to apply.
**Key Responsibilities:**
+ Provides medical nutrition therapy including nutrition assessment, diet modification, nutrition education, and intervention for the patient population
+ Follows all guidelines as outlined within the Company Diet Manual and the Clinical Nutrition Policies&Procedures Manual
+ Complies with all regulatory standards to include federal, state, and accrediting agencies while adhering to facility confidentiality, HIPAA regulations, and patient rights policies

The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better address racial health gaps and improve health outcomes for all New Yorkers. CHECW sits under the Agency's Chief Medical Officer.
The Office of the Chief Medical Officer, which sits in the CHECW division, seeks to hire a Senior Epidemiologist & Maternal Health. Reporting to the Deputy Chief Medical Officer, the Senior Epidemiologist & Maternal Health. will work to serve as the Health Department's lead strategist for maternal behavioral health. The Senior Epidemiologist & Maternal Health will work to identify needs related to maternal behavioral health and work collaboratively across the department and with external partners to develop, implement, and evaluate strategies and initiatives to address outstanding concerns and ensure a holistic focus to maternal health.

Serve as the Health Department lead strategist and coordinator for maternal behavioral health.

Elevate the need to address maternal behavioral health needs as critical for achieving HealthlyNYC goals.

Collaborate with Division leads and program directors to map current need and reach for existing programming.

Facilitate inter-divisional communication and collaboration, leveraging existing workgroups including the PMADS/SUD group (including the divisions of Family Child Health, Mental Hygiene, CHECW, and the Center for Population Health and Data Science) and CMO Birth Equity sub-group.

Build relationships and foster collaboration with subject matter experts within the Division of Mental Hygiene.
Work with programs to regularly review behavioral health and programming data. Use data to identify and target opportunities for alignment, such as referral pathways.

Lead implementation of behavioral health activities to carry out NYC Maternal Mortality Review Community recommendations including: Collaboration with academic institutions on workforce development goals; Provider communication and education on behavioral health and maternal health, including education on opioid agonist therapy and pain management; Distribution of naloxone through birthing facilities; Development of harm reduction materials for pregnant and parenting New Yorkers.

Liaise with external partners actively working in maternal behavioral health, including New York City Health and Hospitals, the NYC Administration for Children's Services, the NYC Department of Homeless Services, and academic partners.

Liaise with internal partners including the Office of External Affairs and the Center for Population Health and Data Science.
Preferred Skills:

Master's degree public health, public policy, social work, or a related field desirable;
