The Care Promoter 1 is an integral member of a Physician-lead Care Team that is focused on providing excellent and comprehensive primary care for a specific population of patients.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
+ Measures and records patient vital signs; records patient interview and medical history.
+ Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel.
+ Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient.
+ Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel.
+ Perform non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic waived lab tests.
+ Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision.
+ Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times.
+ Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician.
+ Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing.
+ Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls.
+ Performs other duties as assigned and modified at manager's discretion.

+ Spoken and written fluency in English
EDUCATION AND EXPERIENCE CRITERIA:
+ High school diploma or equivalent education (GED) required
+ Graduation from a nationally accredited Medical Assistant program **OR** comparable healthcare vocational training (i.e. foreign-trained physician, paramedic, etc.) required
+ A minimum of 1 year of work experience as a Medical Assistant, Patient Care Technician or similar in a hospital, medical clinic or comparable environment **OR** documented clinical rotation training can be substituted for work experience
+ BLS for Healthcare Providers required
+ Experience working with geriatric patients is a plus
+ EMR system experience preferred

Responsible for contacting members following appointments/services rendered to ensure member understanding and navigation of the health care system, and coordination support for follow-up care. - Communicates lab results and other exam findings with members and primary care providers.
- Communicates with nurse practitioners on member health issues to ensure timely follow-up and response.
- Supports care coordination and intervention by sending electronic messages/faxes to primary care providers.
- Closes gaps in health care by preparing member lab result letters and coordinates follow-up on lab results.
- Fulfills nurse practitioner supply orders and maintains adequate nursing supply inventory.
- Performs administrative tasks such as telephone and email response, documentation follow-up and updates to member records.
- Facilitates follow-up calls to members to ensure positive member experience/outcomes and ensure needs are met.
- Assists with special projects under the direction of clinical leadership.
Required Qualifications
- At least 1 year of health care experience preferably in a family practice, home health, primary care, palliative care or hospice setting, or equivalent combination of relevant education and experience.
- Graduate of an accredited medical assistant (MA) program.
- Demonstrated understanding of patient care, medical terminology, coding procedures, reference tools and appropriate clinical pharmacology for medical assistant practice scope.
- Excellent customer service skills.
- Ability to work independently or as part of a team.
- Ability and desire to meet or exceed Care Connections clinical integration and departmental metrics and conjunctively provide excellent customer service.
- Strong attention to detail, ability to multi-task, prioritize, meet deadlines and demonstrate follow-through.
- Utilizes critical-thinking skills to identify issues, problem-solve and demonstrate initiative.

**Licensing/Certification**

**Medical Assistants are eligible for $5,000 sign on bonus**
The Medical Assistant is a multi-skilled clinical professional that works within the scope of practice and in alignment with the standards of excellence and quality. The Medical Assistant provides indirect and/or direct patient care under the direction of the provider.
Under the direction of the provider, contributes to the completion of the patient's chief complaint (including but not limited to respirations, blood pressure, pulse oximetry, height, weight and temperature) and documentation of patient/family needs. Identifies significant changes in patient condition through data collection and reports them to the provider. Understands and adheres to the legal responsibilities and requirements within the Medical Assistant role.
Under the direction of the provider, participates in planning and implementing care for patients within the scope of Medical Assisting practice. Troubleshoots issues and escalate problems to provider, direct supervisor, or appropriate internal resource. Demonstrates standards of excellence in care in all interactions, for both internal and external customers.
Within the Medical Assisting scope and state guidelines, administers ordered medications and/or vaccines via oral, injection, topically, as directed. May performing casting duties according to state regulations and appropriate certification requirements.
Documents in EMR accurately and appropriately. Manages My Chart request and incoming patient calls. Manages in basket messages under the Provider's verbatim instructions.
Other duties include but not limited to ordering supplies appropriately, tracking referrals, labs and diagnostic tests, disposing contaminated supplies, etc.
**Licensing/Certification**
Medical Assisting certification (required within 12 months of placement in MA role): Certified Medical Assistant (American Association of Medical Assisting), Registered Medical Assistant (American Medical Technologists), Certified Clinical Medical Assistant (National Healthcareer Association, or National Certified Medical Assistant (National Center for Competency Testing). If incumbent is unable to obtain MA certification, an Assessment-Based Recognition in Order Entry (ABR-OE) is acceptable within same time period.
BLS Basic Life Support (preferred upon hire, required prior to independent patient care).
**Education:** High school degree, preferred vocational/technical degree in medical assisting. Enrollment in an approved Medical Assistant program (required within 30 days of employment)
**Minimum Experience:** One year of healthcare experience, or 24 months of clerical physician practice experience preferred.
