Job Summary

Under the general supervision of the Clinic Manager, the Medical Assistant performs basic and other patient care services for which the Medical Assistant has received instructions and demonstrated competency.

Responsibilities and Duties

1. Prepares exam rooms for patient visit using infection control procedures.

2. Performs patient intake utilizing Electronic Health Records.

3. Monitors patient flow and make appropriate recommendations.

4. Performs select physical exams i.e. diabetic foot exams, audiograms, etc.

5. Advocates and performs standing orders for labs, EKG, Chest X-ray and other tests as necessary.

6. Performs Autoclave duties of cleaning and sterilizing instruments by clinic practitioners and clinic protocols.

7. Provides patient education and information as instructed by clinic practitioners and clinic protocols.

8. Works effectively on Adult, OB, and Pediatric units as wells as Medical records units as assigned and assists medical clerk at front desk as assigned.

9. Travels to different Council clinics as assigned.

10. Excellent communication skills with ability to articulate clearly and confidently; greets patients with courtesy and respect.

Responsibilities
  • Assists in admission process by escorting patients from the reception area and providing appropriate preparation, instruction and assuring patient privacy.
  • Assist in FEMTO laser procedures as needed.
  • Assist in sterile processing as needed.
  • Assists in safe transport of patients. Cleans and remakes stretchers utilizing proper infection control procedures and operates stretchers to meet patient needs.
  • Under direction of RN and/or LPN, obtains vital signs, assist patients to ambulate, dress/undress.
  • Provides nourishment for patients under direction of RN and/or LPN.
  • Assists in patients discharge procedure including escorting patient safely from facility to transportation.
  • Assists in unit housekeeping activities.
  • Properly disposes of bio-hazardous waste.
  • Assists in restocking supplies.
  • Assists in other tasks as directed by RN/LPN.
  • Serves as a patient advocate.
  • Identifies own learning needs and goals and seeks direction to meet those needs.
  • Demonstrates ability to maintain proactive response to all situations.
  • Monitors and assesses the physical status of the patient through personal observation and interaction.
  • Provides appropriate and knowledgeable Instructions/training/education to patients and family members.

This is an opportunity for a mission-driven team player who is passionate about working collaboratively to ensure PACE participants meet their goals. In this role, you will assist with the provision of direct participant care by performing routine care procedures and technical/administrative support services in the clinic. This role is pivotal in providing high quality participant care, including vital signs monitoring, lab testing, phlebotomy, EKGs, immunizations, wound care support, and other procedures as directed by the healthcare provider within your scope of practice. You will partner closely with the interdisciplinary team to share any feedback on the participant's well-being or changes in status. You will document all specialties in the Electronic Medical Record (EMR). This is a center-based role.

Core Responsibilities & Expectations for the Role:

  • Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.
  • Exhibit and honor Habitat's values.
  • Performs participant intake, fulfills orders, and manages participant flow in the clinic.
  • Rooms participants in a timely manner, obtaining routine vital signs and screenings.
  • Performs laboratory and testing procedures; follow up with participants regarding test results and treatment plans.
  • Support in back-office duties such as tracking referrals, medical record management / transmission, authorizations, cleaning, and stocking exam rooms.
  • Assists Center Manager in managing inventory, medical equipment, and supplies.
  • Assist with the upkeep of routine clinic logs and clinic equipment readiness.
  • Prepare participants for examinations and treatments, administering immunizations as directed by the healthcare provider.
  • Coordinates with other clinic staff to schedule follow-up visits, external appointments, annual/bi-annual assessments, ensuring a continuity of care.
  • Documents intake in the EMR system.
  • Performs related duties as assigned.

Required Qualifications:

  • Passion and mission orientation for serving high-risk seniors and frail older adults.
  • Ability to coordinate and communicate with an interdisciplinary team.
  • Active Medical Assistant license in California and in good standing.
  • Certified in Phlebotomy.
  • High School diploma.
  • A minimum of two years' experience working with the frail or elderly.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof of all immunizations are up to date.
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.
  • Proof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law.

Preferred Qualifications:

  • Bilingual: Spanish/Mandarin/Cantonese preferred.
  • A state issued driver's license, personal transportation, and auto insurance as required by law.

Location: Sacramento, CA (Onsite)

Compensation: We take into account an individual's qualifications, skillset, and experience in determining final salary. Full time role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $22.00 - $28.00 hourly. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
