JOB ROLE

ESSENTIAL JOB FUNCTIONS
- Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery.
- Maintain complete and well organized closed (discharged) medical records ensuring proper chronology. Also maintain active medical records if he/she is uploading information in PCC.
- Conduct periodic, random audits of active (in-house) medical records for both quantitative and qualitative data.
- Notify appropriate individual(s) when a medical record is incomplete as needed.
- Maintain a sign-out log for medical records(s) removed from the department and follow up as necessary.
- Maintain the legal confidentiality and physical security of medical records as required by HIPAA and/or other regulations.
- Determine proper release of medical record information in response to proper informational requests in compliance with HIPAA and/or other regulatory guidelines.
- Code diagnoses and services provided with ICD-10 statistical classification as deemed necessary.
- File medical record information in a timely manner and thin/purge medical records in accordance with facility policy.
- Report any allegation of abuse, neglect, exploitation or misappropriation of resident property per facility policy.
- Participate in the Quality Assurance Program as requested or directed.
- Maintain stock of forms and record supplies.
KNOWLEDGE, ABILITIES AND SKILLS
Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a long-term care setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to define problems, collect date, establish facts and draw valid conclusions.

**Job Summary and Qualifications**
As a Medical Records Specialist, you would be responsible for assisting the HIM Director by routinely performing duties in support of the management of the Horizon Patient Folder (HPF)/McKesson Patient Folder (MPF) workflow queues, working applicable worklists within 3M 360 Encompass, the resolution of unbilled accounts, and the processing of physician suspensions. In addition, you will serve as the primary point of contact when the HIM Director and/or HIM Coordinator is unavailable.
In this role you will:
+ Retrieves discharged medical records from various departments in the hospital and reconciles them to ensure that all records are accounted for.
+ Facilitates the retrieval and printing of medical records from storage, as well as the storage, archival and record retention of documents and/or other Alternate Media that cannot be scanned into HPF/MPF (e.g., fetal monitor strips).
+ May assist with the physician suspension process by evaluating if a physician should be put on suspension, creating the list of recommended suspensions for approval, sending out notice letters, making reminder calls, etc.
+ Prepares medical records and loose documents for scanning.
+ Scans medical record documents.
+ Indexes medical record documentation.
+ Performs a paper document to PC screen quality control validation to ensure that all documents associated with each record have been scanned.
+ Completes any certification program and continuing education that may be required by state law to accurately perform the duties of the birth certificate clerk completion and works under the guidelines and process as defined by the state.
+ Interacts with the parents to collect and document the birth information, delivering the appropriate forms to them and providing guidance in the completion of the forms.

The Medical Records Clerk ensures all forms are properly identified, completed, and signed. Enters all necessary information into the system. Communicates with physicians and staff to clarify diagnoses or get additional information. May also assign a code to each diagnosis and procedure. The Medical Records Clerk assembles and maintains patients' health information in medical records and charts. The Medical Records Clerk performs basic administrative/clerical/operational/customer support/computational tasks. Typically works on routine and patterned assignments.
The Medical Records Clerk ensures all forms are properly identified, completed, and signed. Enters all necessary information into the system. Communicates with physicians and staff to clarify diagnoses or get additional information. May also assign a code to each diagnosis and procedure. Decisions are limited to defined parameters around work expectations, quality standards, priorities and timing, and works under close supervision and/or within established policies/practices and guidelines with minimal opportunity for deviation.

**Specific duties include, but are not limited to:**
+ Complete medical records requests via email, fax, and mail per a medical records release within a timely fashion.
+ Document payment for records requests received from law offices.
+ Provide back up support the Scheduling team and Front Office team as needed. Job duties include greeting patients, answering phones, scheduling patient appointments, entering patient information into scheduling database, confirming patient appointments and collection of necessary on-site paperwork.
+ Collect and distribute mail within the clinic.
**Position Requirements:**
+ High School Diploma or equivalent experience required; Certificate from College or Technical School preferred.
+ 1-2 years in distributing Medical Records to general public and other practicing providers required.
**Physical Requirements:**
The employee may be exposed to radioactive isotopes, ionizing radiation, and a strong magnetic field. May be exposed to radiation, blood/body fluids and infectious disease.
+ More than 50% of the time:
+ Sit, stand, walk.
+ Repetitive movement of hands, arms and legs.
+ See, speak and hear to be able to communicate with patients.
+ Less than 50% of the time:
