What You'll Do

• Conduct group therapy, psychoeducation, and process groups designed to meet each client's
challenges and needs
• Assist clients in developing healthy coping skills, positive behaviors, and recovery tools
• Collaborate consistently with clinical staff to keep track of client challenges and progress
• Escalate any significant client concerns to Clinical Director
• Maintain thorough documentation of all group sessions conducted
• Ensure all clinical assessments and individual client charting is completed


What We're Looking For


• Bachelor's degree in psychology, counseling, or related field preferred
• Min. one year experience in treatment industry preferred
• Valid licensure/certification necessary to practice in this state:

  • Board Certified Music Therapist
  • Licensed Clinical Professional Art Therapist
  • Yoga Instructor Certification
  • Personal Fitness Trainer Certificate

• If in recovery, a minimum one-year period of sustained sobriety is required.
• Excellent interpersonal, observational, and communication skills

General Purpose of Job:

As part of a larger population health strategy, the Care Facilitator will play an integral role in the larger ambulatory care management redesign for Sparrow Medical Group (SMG) practices, acting as the liaison to their locally assigned clinic(s). Although this position will report to the Population Health Service Organization (PHSO), their responsibilities and day-to-day interactions will be under the guidance of the Office Manager and Practice Leads, supporting the care and coverage needed in the clinic.

Common Duties and Responsibilities:

As part of a larger network, the Care Facilitator will work their colleagues to set a standardized approach to patient outreach through both episodic and longitudinal care management. They will work the Epic Registries to proactively identify high-risk patients with a focus on chronic disease management. Additional strategic priorities include: advanced care planning, self-management training, and behavioral health support.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

* Provides Self-Management education and support to care team members patients and families
* Educates and supports appropriate care team members in patient self-management support concepts and techniques
* Actively works with patients to use self-management goal-setting techniques and develop action plans to encourage self-care
* Assisting the patients and/or families in recognizing and prioritizing the problems that need to be addressed
* Providing support to help patients and/or families cope with the disruptions and distress associated with managing their illness and empowering them to regain control of their life, carry on normal roles and activities, and manage the emotional impact of their illness
* Problem solves with patients and their families to maximize their ability to follow preventive, remedial and/or rehabilitative recommendations
* Provide individual or group counseling / coaching as appropriate
* Educates and supports office staff in providing Care Coordination
* Develops and maintains an effective professional working relationship with all patients, families, physicians /providers and health care team members
* Develops and maintains an effective professional working relationship with specialists and facilities (including hospitals, home health care and other ancillary providers) with which SMG physicians have ongoing relationships
* Coordinates care with payer case manager for patients with complex or catastrophic conditions
* Develops, recommends and implements a plan, in conjunction with the patient, family and health care team, which addresses the problems and needs identified in the assessment
* Provides or arranges chronic disease education (as appropriate) for patients and families as needed
* Provides consultation services for other department staff
* Assists in developing a process to track care coordination activities
* Assists in developing written procedures and/or guidelines on care coordination processes and trains appropriate care team members on the processes
* Facilitates follow-up with patients discharged from the hospital or Emergency Department as needed to assure patients understand self-care and treatment expectations and are seen in primary care offices on a timely basis
* Educates and supports office staff in performing a comprehensive assessment
* Screens selected cases to identify needed services and interventions to deliver quality comprehensive care that addresses the patient's full range of health care needs
* Identifies issues, which may impact medical outcomes based on a comprehensive assessment that may include the patient's medical condition, support systems, finances, living situation, housing, behavior, cognition, function and abilities, and the patient's choices and preferences
* Works with office staff as part of an Interdisciplinary Team, develops and implements a plan which addresses problems and needs identified in the assessment and ensures continuity of care by:
* Assisting patient and family with access to appropriate resources and services based on a thorough knowledge of community resources and eligibility requirements
* Advocating on behalf of patients and their families with internal/external resources and payers for needed services, including negotiating with insurance companies for non-covered benefits
* Communicates with patients, families, members of the health care team and others in a professional, diplomatic and empathetic manner
* Provides clear, concise, timely written documentation on the patient's medical record and on departmental records
* Collaborates with other members of the Sparrow Health System (SHS) departments to collect analyze and utilize data for quality / process improvement
* Participates in activities, which support and advance the department and SHS's mission, vision and goals to improve outcomes and operations
* Attends mandatory department and SHS meetings or training sessions
* Provides or arranges education for patients, families, physicians and other members of the health care team, to enhance patient care or improve department outcomes and operations

Job Requirements

Sunnyview Rehab Hospital a premier physical rehabilitation hospital located in Schenectady, NY, is in need of a Clinical Admissions Facilitator **working in Syracuse, NY** to perform preadmission screenings, which is a detailed and comprehensive clinical assessment of the patient's condition, need for rehabilitation therapy and medical treatment to be provided in the Inpatient Rehabilitation Facility (IRF) setting. Incumbent will be working out of their home based in Western, New York and traveling to area hospitals when needed. This position is Monday through Friday business hours with occasional weekend and holiday coverage as needed. **This position will work out of St. Joseph's Hospital, Crouse Hospital, and Upstate University Hospital,** screening for Sunnyview Rehab Hospital.
**Responsibilities:**
+ The Clinical Pre-Admission Facilitator is responsible for patient assessment for acute rehabilitation based on established criteria of the acute rehabilitation center and with compliance to CMS regulations.
+ Education of clients, family members, referral sources, and external payers regarding the effective and efficient utilization of program services and available resources.
+ Screening of clients through on site and/or telephonic assessment and record review in collaboration with physicians, nurses, therapists, social workers, case managers and other interdisciplinary team members involved in the client's care. If the client is unable to participate in an interview, family members or significant others may also be interviewed to obtain accurate information as to pre-morbid functional status, personal interests, cultural considerations, and discharge plans affecting the rehabilitation admission.
+ Determination of medical stability, readiness for program participation, and those services recommended are appropriate, reasonable and necessary. This is done in collaboration with the physical medicine and rehabilitation physician or other licensed treating physician with specialized training and experience in rehabilitation.
+ Facilitation of benefit eligibility and precertification by payers prior to the rehabilitation admission.
+ Coordination of a smooth transition from the referring facility or site to the rehabilitation setting
+ Performs on-site and/or record review, including telephone preadmission screening and clinical assessment
**What you will need:**
+ Must have Active Licensure as a Physical Therapist, Occupational Therapist, or Registered Nurse
+ A minimum of **2 years of clinical experience** ; experience in the rehabilitation of chronically or catastrophically ill or injured individuals or in an acute care hospital required
+ Acute Rehab knowledge is a MUST
+ Demonstrated expertise in analysis, decision making, time management, oral and written communication, and computer use
+ Excellent assessment skills and knowledge of insurance and managed care principles and practices
+ Ability to function independently
+ The position requires a moderate amount of physical work - individual must be able to maneuver throughout halls, stairways, and patient rooms. Interpretation of environmental input requires visual and auditory skills
+ Ability to utilize multiple Electronic Medical Record Platforms
_Pay Range: $35.80-$48.10 Pay is based on_ **_license type,_** _experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location_

The Industrial Nurse specializes in assessing/treating workplace health and safety issues and providing medical aid to persons who are injured or become ill in the workplace. They will contact and direct EMS if required.

Role and Responsibilities

1. Responds to job-related medical emergencies and provides onsite treatment to sick or injured workers.

2. Recommends additional or specialized medical treatment when necessary. Contacts and supports EMS as required.

3. Promotes and maintains the health and wellness of workers.

4. Identifies hazards in the workplace; recommends ways to reduce and prevent hazardous exposures on the job.

5. Conducts training for first aid, safety, CPR, and health programs.

7. Presents recommendations for tasks that can be potentially hazardous to workers.

8. Maintains all records, logs, and files as required.

Skills and Qualifications

1. Education: Associate degree in Nursing required; Bachelors degree preferred, Registered nurse (RN) designation required, State nursing license

2. Experience: Minimum 3 years of experience in a manufacturing company or an urgent care/emergency department setting

3. Knowledge: Ability to administer medical aid in emergency situations. ex: CPR or AED use. Extensive knowledge of nursing principles and procedures. Extensive knowledge of workplace hazards and safety risks. Maintains current knowledge of industrial safety and medical best practices.

4. Skills: Ability to work in high pressure situations and remain calm in medical emergencies, Excellent verbal and written communication skills.

Proficient in Microsoft Office Suite or similar software.

5. Domestic role. No interaction with Region or HQ
