The Clinical Director will directly supervise and train primary care providers (PCPs) in his/her assigned center. - Independently provides care for patients with acute and chronic illnesses encountered in older adult patients.

- Plays an active role in the management of their center and helps cover for other providers who may be out for various reasons. - Managing, mentoring and coaching PCPs in his/her assigned center to deliver outstanding clinical outcomes, including sampling other PCP's daily huddles within their center

- Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times.
- Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician.
- Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing.
- Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls.
- Performs other duties as assigned and modified at managers discretion.

MEDICAL ASSISTANT SITE OF SERVICE:Branch health/medical clinics (BHCs) and TRICARE Prime Clinics (TPCs) associated with Naval Medical Center Portsmouth, VA. POSITION QUALIFICATION/REQUIREMENTS: Degree: Possess a high school diploma or GED certificate.Education:ONE OF THE FOLLOWING:Be a graduate from a medical assistant training program accredited by Commission on Accreditation of Allied Health Education Programs (CAAHEP).The Accrediting Bureau of Health Education Schools (ABHES) of the American Medical Technologists.Formal medical services training program of the United States Armed Forces.Formal program approved by the MTF.Experience: Possess a minimum of 1 year experience as a Medical Assistant within the last 3 years.Certification: Current Basic Life Support (BLS) hands on certification through the American Heart Association or American Red Cross.U.S. Citizenship: HCWs performing under this contract shall be U.S. citizens.English Language Requirement: The Contractor shall ensure that all HCWs providing services under this contract are able to read, write, and speak English well enough to effectively communicate.Physical Capability: HCWs shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services.SPECIFIC DUTIES:•Working with all ages (Pediatrics to Geriatrics)•Proactively collaborate and integrate with members of the team, other departments, and directorates to achieve optimal care for the patients in the HOME PORT model.•Actively participate in Team huddles.•Promote effective communication with staff, patients, families and between other departments.•Proactively prepare for visits by reviewing upcoming provider schedules to avoid unnecessary visits with their provider and ensuring TEAM workflow.•Screen patients based on standard questionnaires per clinic protocol.•Prepare patient electronic health record (EHR) and other appropriate forms prior to patient's appointment (i.e., physical). Prepare paperwork during EHR failure.•Utilize automated programs and information technology tools to facilitate the patient experience. •Perform data entry for population health metrics and health promotion. It is expected the HCW will participate in updating patient data by utilizing communication and management tools (i.e., MHS Genesis, Care Point, and Secure Messaging). •Assist in the orientation of new personnel as assigned.•Collaborate with staff and support activities to maintain a safe and orderly department environment.•Perform administrative functions, to include scheduling appointments and preparing correspondence to patients, as assigned.HOURS: Vary by location - apply to inquire and discuss schedulingBase Down Days:During a planned closure of the facility due to training, closures before or after holidays family down days, Employee will only be compensated for the actual hours worked. If the clinic is closed and employee is not able to work, employee will be required to use leave or take LWOP (without penalty). SPECIAL REQUIREMENTS/SKILLSMust be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Teamwork skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential.
