This job maintains and ensures complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment. Prepares, dispenses and verifies medications ordered for an assigned group of patients, using safe, rational, and cost-effective therapeutic decision making. Assesses, evaluates and monitors patient drug therapy; provides drug information, patient education, and other clinical pharmacy services.

**Job Duties**
+ Evaluates all physician orders based on a clinical assessment of the patient which includes but is not limited to allergies, age, renal function, concomitant drug therapy, lab values, approved dosing guidelines, best practice protocols, diagnosis, and physical clinical status.
+ Assures the safe and accurate preparation and dispensing of medications including sterile products and investigational drugs measured by QA, medication occurrences, competency assessment and other documentation.
+ Manages drug therapy to optimize patient outcomes measured by intervention documentation, ADR assessment, chart reviews, target drug reviews, patient education, and the provision of drug information.
+ Maintains professional and technical knowledge, licensures and certifications through continuing education.
+ Manages the operations of the pharmacy in compliance with state and federal regulations.
+ Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Essential Job Responsibilities
  • Administer medications accurately and at appropriate time according to physician and resident service plan.
  • Document medications administered and those not given according to regulations and policies and procedures.
  • Monitor health, safety and well-being of the residents.
  • Document information pertaining to changes in resident's functional status, personal needs, appointments, vitals, etc.
  • Complete incident reports and documentation on resident's record.
  • Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention/follow-up.
  • Report concerns about residents to Resident Care Coordinator/Director of Nursing and/or Executive Director.
  • Assist residents with activities of daily living, including bathing, dressing, eating, personal hygiene and mobility.
  • Assist residents with oxygen and other medical problems outlined in the assistance plan.
  • Assist with resident laundry, daily bed making, and serving meals, as requested.
  • Identify supplies needed to perform tasks and report needs to DON
  • Encourage socialization among residents and promote activities, assisting with activities as requested.
  • Chaperone activities outside the facility when requested.
  • Maintain a safe, home-like, and pleasant environment for residents.
  • Practice safety in working around and with equipment, chemicals, tools, and utensils.

• Successfully complete the Approved Medication Assistive Personnel (AMAP) curriculum with the required passing score (company provided training).
• Administer physician ordered medications and treatments within the scope of permissible AMAP responsibilities.
• Complete required observations with the AMAP Registered Nurse to maintain certification.
• Consult with AMAP Registered Nurse concerning PRN medications, medication stock, and all other matters requiring RN consultation as defined by the AMAP program.
• Utilize electronic medication administration record and other forms of charting.
• Travel to assigned locations to administer medications per physician’s orders. Travel to and from core office location as needed.
• Providing Medication Administration to the clients we serve, to include utilization of QuickMar
• Conducting adaptive equipment audits
• Scanning of medical appointment information to the Nursing Department and ensuring all necessary medical supplies and equipment are received.
• Other duties as assigned


• Prior medication administration experience preferred
• High school degree/GED required
• 18 years of age or older
• Valid driver's license
• Ability to work in group home or independent living home-like settings.
• Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
• Ability to handle physical aspects of job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time
• Must maintain state regulatory certifications, as required by state/program requirements
• Medication Administration as required by state/program requirements
