**A Brief Overview**
The Social Work Manager is a part of and engages with the service line leadership team. The Manager has the responsibility to collaborate with the service line leadership team when decisions are made around program and staff development. Oversees service line efficiencies to ensure appropriate utilization of Health System resources including patient financial assistance.
+ Supervises and directs the work of professional and support staff to achieve optimum patient flow and cycle times appropriate for services; analyzes staffing mix and staff deployment to meet clinic needs; interviews and selects new employees; assures training and orientation for all new employees; monitors staff productivity; schedules work; coordinates reporting of payroll; establishes standards and evaluates performance; implements employee disciplinary process and responds to grievances as needed; assures employee competencies and long-term development through regular performance appraisals.
+ Facilitates professional development and clinical growth of social work staff through implementation and maintenance of program specific competencies. Promotes and facilitates educational opportunities based on assessment of staff needs.

**Education Qualifications**
+ Master's degree in social work from accredited program required.
**Experience Qualifications**

Position Summary: This position supports the mission of Swedish Medical Center optimizing the quality of life of all those served by expanding the traditional focus on medical needs to the needs of the whole person. The Penn Memory Center (PMC) is accepting applications for our second clinical social work fellow. This one-year fellowship provides an opportunity for a post-graduate LSW to gain experience providing brief treatment psychotherapy (up to 6 sessions) for individuals cared for at the Penn Memory Center and/or their family members and caregivers. This hybrid position requires at least two days in the office as well as a telehealth therapy for those living in the state the fellow is licensed in.
Job Description
The fellow will be responsible for building up their practice through networking and referral from other PMC staff members. In addition to their psychotherapy practice, the fellow will have a role in PMC social programming and may be asked to cover for other social workers on a case-by-case basis. The fellow will be provided with two hours of individual or group weekly supervision per week with a supervising clinician to count towards an accumulation of clinical hours needed to sit for their LCSW exam. The fellow will also be encouraged to seek advanced training in specialized psychotherapeutic techniques.

**Preferred Qualifications**
**Role Purpose**
Coordinates the administration of comprehensive social work. Understands and participates in the operational processes of the patient care area and as part of the multidisciplinary team to deliver the safest and most efficient/effective patient care to patients. Evaluates patient care and programming through part-time direct supervision and chart reviews.
**Responsibilities**
+ Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
+ May participate in the development of departmental staffing, revenue and/or expense budgets and having direct responsibility for adhering to those goals. This includes responding to changes in the business which may affect the ability to achieve the budget goals.
+ Assists with operations of unit services and integration with other departments.
+ Hires, manages, and evaluates staff and services to build an effective work unit.

In this vital leadership role, you will oversee a team of Social Workers, Behavioral Health Specialists, and a Social Work Supervisor—ensuring high-quality care that aligns with each participant’s individualized care plan.
What you'll be doing
- Lead and supervise a multidisciplinary team including Social Workers, Behavioral Health Specialists, and a Social Work Supervisor.
- Oversee the quality and consistency of psychosocial assessments, care coordination, and participation in Interdisciplinary Team (IDT) meetings.
- Ensure timely and effective delivery of social work services in response to participant needs.
- Support professional development and performance of the social work team.
- Collaborate closely with clinical leadership to improve care models and participant outcomes.
- Minimum 6 years of progressive experience in social work, including at least 2 years in a supervisory or leadership role along with 1 year of experience with a frail or elderly population.
- Master’s degree in Social Work (MSW) required.
- Social work licensure preferred.
- Strong interpersonal and communication skills; ability to lead with empathy and accountability.
- Valid state driver’s license with a clean DMV record.
- Copy of recent vehicle insurance required.

ESSESNTIAL DUTIES AND RESPONSIBILITIES:
- Support the implementation of participants’ care plans and act as a liaison between participants, caregivers, community agencies, and the interdisciplinary team (IDT).
- Coordinate participant admissions and discharges to healthcare facilities, temporary respites, or permanent placements, including referrals to housing options and accompanying participants as needed.
- Provide education and ongoing support to participants and families regarding PACE services, resources, and benefits, including Social Security, Medicaid, and Medicare applications and renewals.
- Assist participants in maintaining housing and independence through interventions, home visits, and collaboration with caregivers and housing providers.
- Offer guidance to team members on topics like dementia, behavioral challenges, and care strategies.
- Manage documentation of clinical services, including placements, hospital admissions/discharges, home visits, and significant events, adhering to PACE requirements.
- Assist participants and caregivers with grievances, appeals, and maintaining Medicaid eligibility.
- Facilitate transitional care in the event of program termination, ensuring continuity of services and referrals to other providers.
- Provide case management services and coordinate appointments between social service staff and participants.
- Perform administrative tasks such as filing, scanning, faxing, and mailing documents.
- Follow organizational policies, safety guidelines, and confidentiality standards to maintain a safe and compliant work environment.
- Participate in Quality Improvement initiatives, continuing education, training, and maintaining professional certifications .
EXPERIENCE EDUCATION AND CERTIFICATIONS:
- Bachelor's degree in social work or related field
- Experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
- 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
PRE-EMPLOYMENT REQUIREMENTS:
