**Opportunity to train 1 on 1 with an RN while in school.**
PRN - 1 shift every 2 weeks
**General Summary:**
Performs various general duties and assists in the provision of patient care of all age groups as directed by the Registered Nurse and Organizational Leadership. Provides direct care for patients, answers call lights, performs clerical/courier/transporter functions. Collects data through a variety of means including observation and through the use of instrumentation. Recognizes and communicates pertinent information to multidisciplinary teams to ensure prompt attention to patient and departmental needs.
**Essential Job Functions**
Assist patients with measures of comfort and activities of daily living, including but not limited to bathing, feeding, toileting, dressing, and grooming.
Assist patients with mobility by performing active and passive range of motion, lifting, turning, repositioning, ambulation, and transportation.
Performs routine patient care as delegated by the Registered Nurse including but not limited to obtaining specimens, recording vital signs, applying ice/heat/compress/ dressings, intake/output, point of care testing, 12 lead EKG, and reapplying monitors. May perform CPR. Explains procedures to patients allaying apprehension and eliciting cooperation. May assist in restraining combative patients.
Performs tasks and procedures independently after successful performance with a RN or ONLY under the direct supervision and responsibility of a RN including but not limited to changing peripheral IV site dressings, phlebotomy, removal of peripheral IV's, inserting and/or removing urinary catheters, performing simple trach care, administering non-medicated enemas, performing oral suctioning, changing ostomy appliances and performing ostomy care, and assisting with transferring patients to a higher level of care.
Assists nursing team in the process of admitting, transferring, and discharging patients, including notifying appropriate individuals and departments, assembles patient charts and related paperwork, assisting and performing environmental orientation of patients, preparing, stocking, cleaning of rooms, documenting historical information, and securing belongings and valuables.
Clerical skills/duties including but not limited to courier services, answering phones/call lights and orders, obtains, stocks, places, tests, discontinues, and removes patient equipment and supplies.
Communicates and Documents pertinent patient and general information and services provided.
Identifies, troubleshoots, and works with others as applicable to resolve issues and concerns.
Participates in hand-off/report of pertinent information regarding patient and departmental needs upon arrival and when leaving.
Other duties as assigned.

**Minimum Years and Type of Experience:**
Must be currently enrolled in a professional school of nursing and have successfully completed one clinical nursing course and one clinical experience, or be within 90 days post graduation.

Must be currently enrolled in a professional school of nursing and have successfully completed one clinical nursing course and one clinical experience and be CPR certified. Maintains and Demonstrates competency in required clinical skills/competencies. Demonstrates computer literacy and basic technological abilities or completion of basic computer class. Primary Function/General Purpose of Position
The Lead Radiation Therapist performs radiation therapy procedures including assisting in examinations and operation of therapeutic equipment. The Lead Radiation Therapist delivers prescribed doses of external beam and radioactive elements. This role functions as the point person for operational questions without the Chief Radiation Therapist and as a technical resource to Radiation Therapists. The Lead Radiation Therapist supports and promotes department efforts in meeting established projects and goals and/or specific individual goals and promotes the mission and values of the Organization. The Radiation Therapist works under the guidance of the Radiation Oncologist, Physicist, and Manager to deliver high-quality care and services.

+ Ensures radiation safety to minimize exposure to patients, staff, and the public.

**SUMMARY OF JOB RESPONSIBILITIES:**
Under the direct supervision of the administrative director (or his physician designee) and administrative direction of the Department Director, provides outpatient clinical services (individual, group, family, couples) to patients at a Mercy Health Physicians Outpatient Clinic Site in Lorain, Ohio. Completes all medical records documentation required for diagnostic reassessment, treatment planning, referrals, transfers, discharge/termination planning, and quality/performance improvement. Fulfills all requirements for advanced pre-certification and continuing treatment authorizations with third-party payers.
**QUALIFICATIONS:**
**EDUCATION** :
+ Masters Degree from an accredited training program in area of speciality.
+ LISW or LPCC by Ohio Counselor and Social Work Board and 2-4 years post licensure experience performing same or related job duties (i.e. outpatient counseling).
OR
· CNS with Certificate of Authority in psychiatric nursing/mental health practice.

**PRINCIPAL JOB ACCOUNTABILITIES:**
**Spends at least 80% of worked time to deliver direct, billable treatment services to Mercy Health Physicians, in compliance with Department and BSMH policies, procedures, and Standards of Practice.**
**Completes all medical records documentation (content and timeframe requirements) mandated by Departmental BSMH regulatory (e.g. licensure, certifications, accreditations, legal), and third-party payer (managed care, HMO, Medicare, commercial insurance, etc.) agents.**
**Provides at least two psychotherapy groups, reviewed, and approved in advance by BSMH. Performance measured by written description group (narrative, schedule, admission criteria, therapists, etc.) Scheduling system and charge reports.**
**Pre-certifies admission of outpatients for treatment in advance of visits and obtains authorizations for continuing care, from third party payers according to contractual obligations. Performance measured by documented pre-certification/continuing certification number or letter, specifying length and frequency of authorization.**
**Provides high-quality services to Clinic Outpatients, in compliance with Departmental, BSMH, licensure, and accreditation agents and with accepted standards of practice. Performance measured by patient satisfaction, medical records, documentation, and audit/survey results.**
**Provides scheduled work hours during clinic hours to accommodate patient needs and service demands. Performance measured by master schedule review.**
**Completes all BSMH and Department treatment and provider-related work (e.g. provider applications, reports to third-parties, legal documents) within timeframes requested for response. Performance measured by stamped date in requestors office when information is received.**

PATIENT CARE ASSISTANT OBSERVATION
1DOU is a 30 bed observation unit with mostly telemetry and med/surg type patients. Most patients are on the floor for 24-48 hours for observation, or up to 3 days if admitted inpatient. A fast-paced unit, 1DOU has many admissions and discharges every day. They welcome new grads to their unit, and it is a great place to solidify nursing skills.

A caregiver that is responsible for providing or assisting with patients' care, transporting, maintaining a safe and clean environment, monitoring equipment function and responsible for documenting on appropriate forms. The Patient Care Assistant must be able to demonstrate the knowledge and skills necessary to provide care to age groups according to specific chronological age, developmental age and/or psycho-social maturity.
MINIMUM QUALIFICATIONS:
Education:
High school diploma or GED required.
Required length of experience:
Six (6) months of prior healthcare experience preferred.
STNA preferred but not required.
Required licensure, certification, and/or registry:
BLS required, may obtain within 90 days of hire.
