The Department of Regional Cardiothoracic Surgery has an opening for a Full-time Nurse Practitioner **(Acute Care)** or Physician Assistant at Fairview Hospital and Metro. The position encompasses OR, inpatient and outpatient services.
**Schedule** : 0700 - 1530, ; flexible schedule with on-call requirement.
* **This position is open to New Graduates** **that have completed a cardiothoracic rotation.**
*APRNs must be **Acute Care certified** - preferred OR first assist experience, but not required.
Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.
**Responsibilities:**
+ Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team.
+ Performs and interprets common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.

**Education:**
+ Graduate of an A.R.C. approved Physician Assistant Program.

• Assists in collecting  information for infection control audits.

TRANSITIONAL CARE UNIT:

• Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care 

• Reinforces all education and care related matters as it relates to the new patient     as allowed by state law

• Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.   

• Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.

• Sets and preforms a treatment on all machines used in the transitional care unit.

• Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

• Assists with coordination of transition to patient’s modality choice.   

• Assists with supply ordering and inventory for TCU

The J53 nursing unit is one of five Cardiovascular Stepdown Units at Cleveland Clinic's Main Campus and offers 24 beds to patients receiving coronary artery bypass surgery, valve repair or replacement and minimally invasive or off-pump coronary bypass surgery. This unit also cares for cardiac-related admissions and Total Artificial Hearts (TAH).
**A caregiver in this position works days, from 7:00pm to 7:30a.**

+ Provide or assist with patient care under the supervision of an RN.
