POSITION SUMMARY/RESPONSIBILITIES
This position will serve as the link between the community, volunteer network members, health system providers, social workers, and the Community Health Division in order to increase the utilization of available health promotion programs and maximize the use of existing health services as deemed appropriate. Assists and guides patients through community health prevention and treatment services to ensure that patients are able to access and keep medical appointments, participate in recommended care plans, and receive appropriate health information about their diagnosis and treatment plan. All functions and behaviors are applicable to the clinical and community settings.
EDUCATION/EXPERIENCE
Bachelor’s degree in social sciences, health education or related field of study is required. One year of experience in the area of health education, public health, social work and/or case management may be considered as a substitute in lieu of the Bachelor’s degree. English/Spanish fluency is required.

Healthy Families is an evidence-based home visiting program that promotes child well-being through family-focused support provided in the home. This position will conduct home visits to provide support and connect families to services and resources available. This position will recruit families, help develop community partnerships and implement the Healthy Families curriculum provided. Additional activities will include patient tracking, data entry, and reporting.
EDUCATION/EXPERIENCE
Bachelor’s degree in early childhood development, social sciences or health education is preferred or related field of study. A minimum of 1 years or equivalent experience in the area of social services, health education, public health, or case management. Experience in public speaking and educational presentations demonstrating effective communication skills is required. English/Spanish fluency is strongly desired.
