About The Role
We are rapidly expanding our operations as we build new partnerships across the healthcare spectrum. We are looking for an experienced, customer-centric individual to grow our Care Navigation arm. As a Care Navigator specializing in community resources, you will play a vital role in ensuring that patients receive comprehensive care by connecting them with relevant community services and by assisting patients in closing care gaps. The Care Navigator plays a crucial role in providing a positive, welcoming experience for patients. The ideal candidate will have a strong customer-service orientation, be passionate about patient care, and never hesitate to go the extra mile to ensure a positive member experience.
This is a temporary full-time position through December 31st, 2025.

What you will do:- Provide direct individual care navigation services to an assigned caseload of patients to support them in navigating through the complex healthcare system
- Conduct thorough assessments of patients to identify social, economic, and environmental factors that may impact their health and well-being
- Serve as an advocate for patients in accessing community resources, ensuring they receive the necessary support to overcome barriers to care
- Research, identify, and maintain an up-to-date database of community resources, including support groups, financial assistance programs, housing services, and other relevant resources
- Educate patients on available community resources and services, explaining how they can access and benefit from these resources to improve their overall quality of life
- Use strong communication skills to build trust with prospective patients and caregivers to encourage and motivate them to schedule necessary appointments and be active in their health care
- Facilitate communication between healthcare providers and community organizations to ensure seamless referrals and transitions of care for patients.
- Demonstrate cultural competence and sensitivity when working with patients from diverse backgrounds, respecting individual beliefs, values, and preferences
- Maintain accurate and detailed records of patient interactions, resource recommendations, and outcomes in accordance with healthcare protocols and privacy regulations
- As necessary, you will escalate questions and concerns to other teams for additional support
- Other duties and/or projects as assigned including outbound calling to schedule patients

The hourly range for this full-time position is 25/hr to 30/hr + benefits. Our salary ranges are determined by role, level, and location. The range displayed on each job posting reflects the minimum and maximum target for new hire salaries for the position across all US locations. Within the range, individual pay is determined by work location and additional factors, including job-related skills, experience, and relevant education or training. - Assists with any patient emergency as needed.
- Recommends and/or conducts specific areas of in-service or continuing education with patients and caregiver as appropriate.
- Assists with compliance of quality measures for CSNP program
- Assist with HEDIS measures and collection of data
- Assist with collection and presentation of QA data for the CSNP program
- Participates on ICT meetings for CSNP members
- Functions in CNA role if needed for member visits
- Performs other duties as assigned.
JOB REQUIREMENTS:
- Graduated from an accredited Certified Nursing Assistant (CNA) with a current CNA certification in the state of practice preferred
- 2 years’ clinical experience as a Certified Nursing Assistant (CNA) in geriatric, adult or family practice setting, preferred
- 2 years’ experience working in a community-based program involving geriatric, adult or family practice setting
- Pertinent clinical work experience relevant to population served.
- Current BLS certification
- Must be able to demonstrate computer skills, including use of e-mail and Microsoft office word and excel
- Must be able to demonstrate effective written communication skills including but not limited to writing reports and correspondence
- Must be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to members, providers, families and staff and listening to members, providers and families

The Referral Navigator is responsible for processing referrals from Primary Care Providers (both internal and external/network) for the purposes of obtaining specialist care by our internal / network specialty Providers.
Responsible for building, submitting, processing, and scheduling referrals from primary care providers for the purposes of obtaining specialist care by our internal and external/network specialty providers. This includes but is not limited to confirming all necessary documents and information has been provided, sending back for additional information, cancelling those that do not meet criteria or do not have necessary information required within a specified time frame, forwarding to MD review if questionable, accepting referrals and determining if the referral meets outsource or overflow criteria. While most of the work is computer-based, there is a lot of paperwork handling, faxing, scanning, and multiple telephone calls. This position has direct face to face contact with multiple patients.
Schedule: Monday to Friday from 8:00 am to 5:00 pm
Location: 2716 Tennaya Drive, Las Vegas, NV 89128
Primary Responsibilities:
• Building new referrals from network providers
• Reviewing to ensure all necessary documents and information has been provided
• Sending back documents for additional information
• Canceling those that do not meet criteria or do not have necessary information required within a specified time frame, forwarding to MD review if questionable
• Accepting referrals and determining if meets outsource or overflow criteria
• While most of the work is computer-based, there is a lot of paperwork handling, faxing, and scanning in addition to multiple telephone calls
• This is primarily a sedentary desk job which will consist of >90% of time spent working on a computer and handling the telephone
• Perform all other duties assigned

• 1+ year of experience and/or training as a Medical Assistant
• Bilingual in English/Spanish
