**SUMMARY OF POSITION:**
Aid in the total rehabilitation of patients of all ages in providing physical therapy services. Documents patient care and results of treatment appropriately in the medical record to comply with policies and procedures of the department and regulatory agencies. Demonstrates ability to treat and care for patients of all ages.
**MINIMUM QUALIFICATION(S):**
+ License to practice as Physical Therapist Assistant in the state of employment.
+ Basic Life Support Certification (BLS).

- Bachelor's degree, Master's level, or DPT, which meets the requirements of an approved physical therapy program, and is licensed through examination by the State Board of Physical Therapy.
- Graduate of a Physical Therapy education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or APTA.
- One year of Physical Therapy experience is required.
- 1-2 years of Home Health experience is preferred.
- Competency in computer skills.
- Proficient in OASIS , SOC
- CPR certified
- Experience with EMR systems
- Pass a background check
- Provide two (2) professional references
- Able to travel to cases as assigned
- Ability to work independently and manage time effectively
- Meets the requirements of federal and/or state criminal and abuse background checks
- Must have access to dependable transportation, auto insurance coverage
