About Us:
New Forest Primary Care Network is committed to delivering innovative, patient-centred care. We are expanding our Home Visiting Team to enhance responsiveness and provide timely medical interventions in patients homes. Working closely with skilled paramedics, this initiative supports better health outcomes and reduces pressure on urgent and emergency care services.
The Role:
We are seeking a motivated and compassionate Salaried GP to join our dynamic Home Visiting Team on a fixed-term basis until March 2028. This role is designed to provide enhanced, on-the-day capacity for patients identified as urgently needing home-based medical intervention working over and above existing GMS provision.
Key Responsibilities:
Conduct timely home visits, prioritising earlier in the day to support urgent patient needs.
Collaborate closely with paramedics to provide seamless, responsive care in the community.
Work proactively with our frailest patients to develop advanced care plans, including comprehensive medication reviews.
Help prevent avoidable hospital admissions by delivering effective interventions at home.
Contribute to reducing overnight hospital stays and conveyance rates to emergency departments.
Improve system flow by alleviating pressures on local urgent and emergency care pathways.
Deliver high-quality, patient-focused clinical care aligned with best practice and PCN objectives.
All the above responibilites contribute to the overarching goals: enabling people to live well with frailty, reducing avoidable acute admissions and bed-days, improving patient experience, and supporting system sustainability. These outcomes align with the ICB Frailty Signature Move metrics and national priorities on prevention and population health, focusing on proactive intervention to avert crises.

Who Were Looking For:
GMC-registered GP with current license to practice.
Experience in community or home-based care is desirable but not essential.
Strong communication skills and the ability to work collaboratively with paramedics and other healthcare professionals.
Must have a valid driving licence and access to reliable transport (Mileage expenses will be reimbursed in line with PCN policy)
Commitment to improving patient outcomes and reducing hospital admissions.
Adaptability and enthusiasm for a proactive, patient-centred approach.

Set in a semi-rural location in Hook, Hampshire, Priory Hospital Mildmay Oaks supports the NHS by providing low secure services and level 2 rehabilitation services for both male and female patients. This includes specialist learning disability and autism spectrum support.
What you'll be doing
You will be part of a compassionate team committed to our patients' wellbeing. We take a personalised approach, using proven therapies to help people move forward with confidence and build a healthier, more fulfilling future. As a Consultant Psychiatrist, you will play a key part in their recovery, assisting qualified staff with the assessment and implementation of individual patient care plans, undertaking routine tasks and activities as directed or delegated to facilitate wellbeing, dignity, and treatment of patients.
- Taking clinical responsibility for patients, including multidisciplinary ward rounds, risk reviews, strategic reviews, and the development of a therapeutic milieu.
- Fulfilling the role of Responsible Clinician to patients detained under the Mental Health Act (1983).
- Supervising the process of referral and admission, including contributing to preadmission assessments of young people with other team members.
- Offering a psychiatric perspective to the multidisciplinary team in relation to the assessment, formulation, and treatment of adults.

We’re hiring Advanced Emergency Medical Technicians (AEMTs) that will respond to emergency and non-emergency requests for medical assistance and deliver high-quality care, treatment and customer service to patients.
Responsibilities:
- Advanced EMTs provide an assessment of patients, determine necessary care and deliver emergency services by utilizing appropriate medical techniques and equipment.
- Document patient information, condition and treatment while maintaining confidentiality and patient rights.
- Performs such activities as application of splints, administration of oxygen or intravenous injections, treatment of minor wounds or abrasions, or administration of artificial resuscitation.
- Take pride in providing a safe, clean, and well-stocked environment for patients.
- Use appropriate Advanced EMT skills to provide care including communications, medical equipment, cleaning procedures, office equipment and tools.
- Advanced EMTs operate an ambulance in conjunction with applicable company safety policies, and traffic laws related to the operation of emergency medical response vehicles.
- Work collaboratively and in a professional manner with all allied health and public safety personnel as well as your fellow EMTs.
Other Responsibilities:
- Participate in community programs to maintain AMR image and establish strong community relations.
Minimum Required Qualifications:
- High school diploma or equivalent (GED)
- State Driver’s License
- State certified Advanced EMT
- CPR/BLS certifications
- Driving record in compliance with company policy
- Pass Physical Agility Test
- Some work experience, preferably in healthcare
