About you
You will be a Registered Nurse with a valid Pin number and will understand your commitment to your profession through the standards of practice set by the NMC.
You will have a passion to mentor and share your knowledge with your colleagues, along with providing support as a practice supervisor and assessor to students. You will understand the importance of effective communication, both verbally and written, and role model professional behaviours.
You will advocate and deliver a person-centred approach to care for the people living in our care home and their families.
Safeguarding is everyone's responsibility and therefore it is important that you are able to recognise the signs which may indicate possible abuse, harm or neglect in its different forms and know what to do if there are any concerns. All colleagues are required to attend safeguarding training appropriate to their role and to undertake additional training in associated areas. You also have the ability to seek appropriate advice and report concerns, including escalation if action is not taken.

Following the release of ARRS GP information and funding we are lookingfor newly qualified (within 2 years and not having had a substantive post),enthusiastic, forward-thinking GPs to join Healthier West Wirral Primary CareNetwork. We will consider applications for between 6 and 7 sessions with astart date as soon as possible. There is also the the potential to be involved inresearch oppertunities.
Main duties of the jobUndertake home visits for patients inneed of medical attention, including those who are housebound or unable toattend surgery who have been referred by GPs.
Provide comprehensive and timely careto patients, ensuring appropriate diagnosis, treatment, and follow-up.
Providegeneral practice care across the 5 GP Practices within Healthier WestWirral PCN, including consultations, diagnosis, treatment, and managementof both acute and chronic conditions.
Workcollaboratively with practice teams, including nursing staff, healthcareassistants, pharmacists, and other ARRS roles.
Work within a multidisciplinary team, gaining exposure to innovative primary care models such as AVS.
Contribute to improving access to care for patients, reducing waiting times, and supporting clinical capacity within the practices.

Job responsibilitiesAcute VisitingService (AVS) 4 Sessions
Undertake home visits for patients inneed of medical attention, including those who are housebound or unable toattend surgerywho have been referred by GPs.

Work alongside physician associatesand paramedics already established in the Acute Home Visiting Team
Work closely with practice staff,community health teams, and the wider PCN to ensure continuity of care forpatients across services.
Ensure accurate and timelydocumentation of visits, clinical assessments, and treatment plans.
Ensure there is appropriate feedbackto referring GP/ clinician as required
Triage and treat patients wishing tosee a health care professional, making any necessary referrals to other membersof the primary health care team
Manage patients with problems needingreferral to secondary care.
Ensure Service is safe and effectiveand remains within boundaries of competence, and to acknowledge limitations.
Advise patients on general healthcare and minor ailments, with referral to other members of the primary andsecondary health care team as necessary
Work from the hub and withincommunities as an autonomous practitioner caring for patients and families.
Ensure that personal and professionalclinical standards are maintained.
To communicate at all levels withinthe team ensuring an effective service is delivered.
To maintain accurate, contemporaneoushealthcare records appropriate to the consultation.
Ensure evidenced-based care isdelivered at the highest standards ensuring delivery of high quality patientcare.
Works with local policies andprocedures.
To achieve and demonstrate agreedstandards of personal and professional development in order to meet the needsof the service.
To participate in the audit process,evaluation and implementing plans and practice change in order to meet patientneed.

Reports to clinical director
GP PracticeSessions 3 Sessions
Providegeneral practice care, including consultations, diagnosis, treatment, andmanagement of both acute and chronic conditions.
Face to face and telephoneconsultations
Checking and signing repeatprescriptions
Workcollaboratively with practice teams, including nursing staff, healthcareassistants, pharmacists, and other ARRS roles.
Administration - dealingwith queries, patient test results, referral letters NHS/private, task, emailsand other paperwork and correspondence in a timely manner
Supportthe management of long-term conditions and deliver preventative care.
Contributeto improving access to care for patients, reducing waiting times, andsupporting clinical capacity within the practices.
Cover all the clinical management ofthe patients in surgery as appropriate including initiating investigations,reviewing results, making referrals to secondary care or to other providers asappropriate
Participatein clinical meetings, multidisciplinary team meetings, and otherpractice/PCN-level discussions to improve patient care.
Reports to clinical director
This Job Description is not exhaustive and maychange as the post develops, but such change will not take place withoutconsultation between the post holder and his/her manager. Job descriptionsshould be reviewed at least annually at the appraisal meeting.
There is a need to travel between sites, thereforeability to drive and uses own car is essential.

The post holder is required to comply with allrelevant policies and procedures pertinent to their post. Failure to followcorrect policies and procedures may result in disciplinary action.
In Accordance with the Risk Management Strategy,employees will participate, whenever required, with the risk managementprocess. They will support line managers by attending mandatory and statutorytraining, completing incident/accident forms for every adverse event or nearmiss that occurs, report all defects and complaints, and communicate anydangerous situation to individuals potentially at risk.
