What You'll Be Doing
The Nurse Practitioner (NP), under the general direction of the Medical Director and Practice Administrator, will provide healthcare in a family practice clinical settings within their scope.
The successful candidate will / be able to:
- Obtain a comprehensive health history through patient/family interviews and chart reviews; assess presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing thorough physical examination
- Develop and implement treatment plan including, but not limited to prescribe/dispense medications in compliance with medical practice guidelines and state laws
- Able to diagnosis disease states and administer appropriate treatment
- Understand and practice within specified limits and consults with Medical Director as appropriate
- Order/perform appropriate laboratory, diagnostic and other screening tests, including seek other information as needed, and consult with physicians and other clinicians for the evaluation of illness as needed
- Document assessment and treatment plan clearly and concisely in patient chart, in a timely fashion
- Prepare documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written format or electronically
- Effectively conduct patient consults and communicate plan of treatment and care with patient/family to ensure patient awareness of self-management and health promotion/disease prevention.
- Recommend appropriate community resources to meet patient/family needs
- Work with specified care teams for preparations for each patient office visit and the follow-up care post visit
- Maintain cooperative and respectful attitude with clinic personnel, providers, and patients to enhance continuity of care and a positive working environment
- Communicate appropriate information to other professionals and community agencies
- Ensure an appropriate discharge plan by facilitating the implementation of the discharge plan, discharge teaching and update discharge plan in collaboration with the interdisciplinary team in accordance with patient's clinical course and continuing needs to expedite post discharge care

- The NP shall be capable of performing a full range of general medical services to include the ability and experience to adequately diagnose and treat urgent/acute disease and injuries including but not limited to:
- Skin, Head, Eyes, Ears, Nose, Nose/Throat, Neck, Cardiovascular System, Gastrointestinal System, Lymphatic System, Musculoskeletal System, Genitourinary System, Reproductive System, Psychiatric Disease, Central Nervous System, Endocrine System, and Peripheral Nervous System.
- Provide a full range of emergency services or crisis intervention in the absence of the physician, including lifesaving emergency procedures to stabilize a patient within the scope of practice and approved protocols.
- Examine all eligible company healthcare system beneficiaries for emergency medical care to render a medical assessment of their condition and take appropriate action.
- Provide immediate evaluation and management of emergency patients as they present themselves for care, seeing no less than an average of two patients per hour.
- Provide emergency medical care to both inpatients, if part of code team response, and outpatients, summon appropriate on-call specialists as necessary, and continue medical management until the arrival of the appropriate specialist.
- Respond to "Code Blue" (cardiac or respiratory arrest) medical emergency alerts within the hospital and provide resuscitation treatment until more specialized treatment arrives. - Provide emergency care to patients in the holding area when necessary.
- Prepare appropriate narrative summaries and patient profile changes. Document history, physical examinations, and preoperative diagnosis. Maintain documentation of treatment IAW established procedures.
- Use facility electronic medical records as directed.
- If staffing permits, accompany emergency room technicians on ambulance runs when indicated by the nature of the emergency. This includes initial response for disaster-type situations. Remain on the scene until relieved by a staff physician.
- Perform emergency room procedures compatible with the medical facility's operating capacity and equipment. Respond to mass casualty situations per hospital procedures.
- As workload permits during scheduled duty, attend quarterly staff meetings as required.
- Become familiar with the hospital formulary. Prescribe pharmaceuticals according to the availability of drugs listed therein.
- Remain in the emergency room or immediately available to the emergency room throughout the tour of duty, except in response to emergencies and/or disaster ambulance runs. Remain on a tour of duty until appropriate replacement is on site.
- Maintain medical records IAW AR 40-66, Medical Record Administration. And correct any deficiencies within 48 hours of their next scheduled shift.
- Use an electronic medical record system for keeping records, ordering of ancillary procedures, ordering of medications, writing doctor's orders, and performing other required patient record functions.
- Utilize standardized, templated patient encounter sheets as supplements to the SF 558, to facilitate documentation of patient care.
- Verify the content and correctness of all prepared and transcribed reports by affixing his/her own signature and stamp to all copies of the document and validating its contents.
- Document appropriate history, physical examination, required laboratory and x-ray examinations, and preliminary diagnosis (as appropriate) prior to healthcare treatment.
The Nurse Practitioner shall meet the following minimum qualifications:
- Have a minimum of one year of experience within the last two years routinely practicing as a nurse practitioner in rendering emergency medicine.
- Must be a US Citizen.
- Must have and maintain a current, active, valid, unrestricted state license to practice medicine in one of the 50 states in the United States, or the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the US Virgin Islands as required to obtain or renew a DEA registration number under the then effective DEA rules. The license must correspond to the DEA registration number.
- Current National Commission on Certification of Physician Assistants (NCCPA) certification, American Nurses Credentialing Center (ANCC) certification, or American Academy of Nurse Practitioners (AANP) certification.
- Have and maintain current Basic Cardiac Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Life Support (PALS) certifications by way of an American Heart Association-approved training program.

- Deliver educational materials to patient populations in order to teach them better health habits and provide an increase to overall population health
- Perform daily checks of supplies and order additional ones as needed
- Perform checks of equipment to maintain proper working order
- Perform tests and exams as needed, including tests requiring machines like EKGs or X-rays
- Collaborate with different health team members and correctional team members to ensure all protocols, security policies, and procedures are followed while still delivering healthcare to those in need
- Keep clear, accurate records as required by protocols
- Communicate with health team members and other staff who assist in the delivery of medical care
- Transcribe med orders
- Perform any additional medical related duties
- D.O or M.D, Nurse Practitioner
- Indiana License (Michigan license bonus, but not required)
- BLS/CPR certified
Schedule: Will Depend on where you are needed. Flexible/Hybrid

Job Description
We are seeking Board-Certified Nurse Practitioners to provide telehealth wellness visits that identify care gaps, support preventive care, and address patient health concerns. NPs in this program will not serve as the patient's new primary care provider, but instead act as a
bridge-to-care, ensuring patients are guided back into the healthcare system with the appropriate next steps. Each visit is scheduled for 1 hour, beginning with a 20 to 30 minute intake conducted by a Medical Assistant (MA), followed by a 30 to 40 minute NP-led visit where clinical assessments, counseling, and care planning are completed.

● Active Nurse Practitioner license in at least one of the following states is required:
WA, NJ, MI, IA, PA, IL, MS, or FL.
● Must have an established collaborating/supervising physician agreement in any
state where such agreements are required for autonomous NP practice.
● Master's degree in Nursing or related field.
● Minimum of 2 years of NP experience, with experience in primary care, geriatrics, or
Medicare populations strongly preferred.
● Familiarity with HEDIS measures, preventive screenings, and quality gap closure.
● Medicare enrollment required, or willingness to apply immediately upon
contracting.
● Strong ability to provide patient-centered education and referral coordination.
● Experience with Athena EMR strongly preferred.
● Excellent communication skills, both verbal and written.
● Comfortable with technology-driven care delivery and EMR documentation. ● Multi-state NP licensure across the program's target states.
● Knowledge of risk adjustment coding and documentation in a telehealth setting.
● Experience working in telehealth wellness visit models.
