Position Summary:

The RN is responsible for coordinating the health care needs of students/people we support, with family, staff, other care providers, as well as outside professionals. The RN will ensure the evaluation, supervision, and/or provision of nursing care and routine health maintenance programs are completed. The RN is responsible for staff training in health and medical issues, and for providing a quality of care for the students/people we support.

Reporting Relationship:

The RN will directly report to a Nursing Supervisor within their division; Residential, Education, or Adult Services. In the absence of the Nursing Supervisor, they may report directly to the Associate Director of Nursing and/or the Director of Clinical Services.

Job Duties and Responsibilities:

  • Complies with all Office of People With Developmental Disabilities (OPWDD), Part 624, Part 633, and/or State Education Department (SED) regulations and reports any violations, deficiencies or incidents to supervisor
  • Assess the health care needs of each student/person we support based on information available such as test results, laboratory data, observation and staff input
  • Develop an individualized plan of nursing services for each student/person who requires nursing care, including those who require medication administration for diagnosed medical conditions and update at least annually or whenever there is a significant change in the student/person's condition
  • Establish and maintain a professional working relationship with family, physicians, dentists, and other medical/health care agencies and professionals
  • Educates staff about the chronic conditions and related health care needs of the students/persons, as well as, health and safety skills related to seizures, blood borne pathogens, etc.
  • Provides initial and ongoing training to staff in all nursing procedures being delegated, including medication administration periodically reviewing that performance is consistent with standards of nursing care.
  • Complete certification/annual recertification (annual evaluation of performance) for all medication administration certified staff

Job Duties and Responsibilities (Education Division Specific):

  • Provides professional nursing care to students with tracheostomies, gastrostomy/jejunostomy tubes, children with colostomies, those who may require oxygen, and children who may require intermittent catheterization in the educational program
  • Identifies and assesses each student's health needs, implements services and evaluates plans of care which impact student learning, achievement and safety
  • Administers prescribed medications and treatments and assesses, interprets and documents student responses
  • Assists in providing New York State mandated and annual screening programs (Scoliosis screening, heights, weights, vision screening)
  • Provides in-service training to staff and/or family members when applicable
  • Administers emergency first aid, or assists students in obtaining treatment at a medical facility

  • Assess and monitor all health-related tracking forms, such as charts, Plans of Nursing Instructions, etc.
  • Ensure that each individual's medication regimen is assessed at least every six months or whenever there is a significant change and provide follow-up on all recommendations.
  • Assess each individual's level of capacity to administer medications at least annually.
  • Assist in the planning of self-administration of medication programs for individuals as needed.
  • Monitor implementation of special diets in cooperation with the dietitian and the Residential Habilitation Manager.

  • Dispense medications as required and provide assistance with toileting, behavioral issues, meals and medical emergencies.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Includes the following. Other duties may be assigned.

  • Gathers basic identifying information to admit clients to First Step Center and explains program rules and expectations
  • Applies compresses, ice bags, and hot water bottles
  • Observes patients and reports adverse reactions to medication or treatment to medical personnel in charge
  • Administers specified medication, orally or by subcutaneous or intermuscular injection noting time and amount on patients' charts
  • Assembles and uses such equipment as catheters, tracheotomy tubes, and oxygen suppliers
  • Collects samples such as urine, blood and sputum from patients for testing and performs routine laboratory tests on samples
  • Sterilizes equipment and supplies
  • Prepares or examines food trays for prescribed diet and feeds patients
  • Records food and fluid intake and output

Education and/or Experience: Associate degree with one year of experience in the treatment of chemical dependency including the treatment needs of patients withdrawing from alcohol or substance abuse, or equivalent combination of education and experience.

Duties

The Buffalo VA Medical Center is seeking a qualified full-time Registered Nurse for the Acute Care Department (Progressive Care - 5C).

The Progressive Care Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, creative approaches to management of complex patient care, and collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. He/she will collaborate in the development, implementation, evaluation, and revision of policies, procedures, guidelines, and demonstrate performance and leadership that is broad enough to improve the care for acutely ill patients who are moderately stable with an elevated risk for instability.

The incumbent will be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administer medications and procedures per established policies and guidelines. Influence care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.

Basic Requirements:

Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Qualifications

  • Must have and maintain an active, unencumbered license (RN) in the state in which the clinician will practice
    • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)- Aveanna can assist in obtaining this requirement after hire, if necessary.
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Must have reliable transportation
