POSITION OVERVIEW:
Under supervision of the OASAS Program Nursing Director, the Nurse Supervisor is responsible for coordinating and delivering patient care utilizing the nursing process. The Nurse Supervisor participates as a member of the interdisciplinary team providing support and information about the nursing services. The Nurse Supervisor coordinates care between other professional disciplines as needed. Through documentation and verbal communication, essential information is shared with health care team members. When care is not provided directly by the Nurse Supervisor, the Nurse Supervisor assigns/delegates care to other qualified members of the health team. This position will adhere to all policies, procedures and regulations set forth by New York State Office of Alcoholism and Substance Abuse to ensure patient safety and compliance. The requirements listed below represent the knowledge, skill, and/or ability required. (4pm - 12am M-F).
KEY ESSENTIALS:
- Uses computer for basic word processing and / or basic spreadsheet use.
- Uses a variety of software packages to maintain databases and create complex, custom reports.
- Supervises all the LPN’s and monitors productivity to ensures policies and procedures are being followed.
- Assists the Medical Director with the history and physicals of the outpatient clients.
- Conducts monthly audits of client records and present the results to the QA committee.
- Develops an action plan to address trends.
- Dispensing of methadone as required.
- Conducts preliminary nursing assessments.
- Reviews daily, weekly and monthly reports of the methadone clinic.
- Reviews policies and procedures with Director and participates in any changes that need to be tailored to the flow of operation.
- Participates in weekly client case conferences.
- Conducts “Health related topics” group meeting.
- Assists the Regional and Assistant director with the day-to-day operations when necessary.
- Other duties as assigned.
REQUIREMENT:
- High School Diploma required.
- Bachelor of Science Nursing preferred.
- Graduate of an accredited Nursing School.
- Must possess a valid and unrestricted NYS license as a Registered Professional Nurse.
- Minimum three (3) years practical experience in substance use disorder program required.
- Minimum of three (3) years of supervisory experience preferred.
- Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
- Current NYS mandated infection control certification (approved by NYSDOH, valid for four years, 4.0 hours).
- Must obtain Mandated reporter (2 hours) within thirty (30) days.

SUMMARY
Family Service League is seeking a full-time Registered Nurse (RN) Supervisor for the Critical Time Intervention (CTI) program in Mastic, NY. The RN Supervisor will work as part of an interdisciplinary team to provide care management to, and address the medical needs of adolescents and adults who have behavioral health needs and who have not been successfully engaged in services during or after critical times in transition. Additionally, the RN Supervisor will provide general supervisory oversight to the CTI team. CTI promotes community integration, self-advocacy, and access to ongoing support by helping individuals develop and utilize strong ties to their professional and non-professional support systems during and after transition periods. This is a community-based position with frequent field work, beginning with client engagement in hospital settings and providing support for approximately 9 months following discharge.

RESPONSIBILITIES
- The RN Supervisor will work closely with the Program Director to provide clinical and administrative oversight to the multidisciplinary CTI staff, including master’s level clinicians, Case Managers and Peer Specialists.
- Co-facilitate regular team meetings and conference cases with staff to determine ongoing recommendations.
- Utilize data and other reports to monitor program outcomes, follow up with staff, and to inform decisions.
- Provide formal and informal supervision to all program staff on agency policy, clinical interventions/recommendations, documentation, use of the EMR, and CTI practices and procedures.
- Engage clients and natural support with in-person, field-based interaction, as well as telephonic and in-office settings
- Assess basic physical and behavioral health needs within RN scope of practice and facilitate linkages/coordinate care with community providers.
- The RN Supervisor will provide patient education and counseling around health issues.
- Provide crisis de-escalation and assessment as needed.
- Hold a small caseload of adolescents and adults with the full range of DSM diagnoses, initially engaging in a hospital setting and providing support for approximately 9 months following discharge.
- Coordinate with collateral contacts, including family and other providers.
- Establish and maintain positive working relationships with others, both internally and externally.
- Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
- Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.
- Set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information, and activities.
- The RN Supervisor will complete training and maintain proficiency with de-escalation skills.
- Assist with management projects, including the review and tracking of statistics related to program objectives and performance indicators.
- Maintain relationships with community providers, hospital systems, and other key stakeholders.
- Ensure programmatic compliance with related regulations and agency policies.
- Ensure the health and safety of staff and clients during emergencies.
- Abide by FSL's Code of Ethics.

QUALIFICATIONS
Bachelor’s Degree in a related field required.
Licensed as a NYS Registered Nurse (RN) required.
Three years of experience working with individuals with mental health, substance use, or co-occurring disorders required.
Prior supervisory experience preferred.
Proficient computer skills, including Microsoft Office and Electronic Health Records, are required. Excellent interpersonal skills and strong verbal and written communication skills required.
Ability to solve problems, make clear decisions and work independently, as well as part of a team required.
Must be responsive to deadlines.
Bilingual Spanish strongly preferred.
Valid and clean NYS Driver's License and able to and operate a vehicle in a safe manner under all driving conditions required.

POSITION SUMMARY:
The Nursing Supervisor is a Registered Nurse (RN) who is responsible for the overall supervision of nursing care at High Point Center for Care in both Assisted Living and Memory Care. He/she will ensure the safety and well-being of all residents; provide direct resident care activities to include assessments, planning, implementing and evaluation of nursing care. He/she will provide managerial and/or clinical support to residents, Licensed Practical Nurses (LPN) and Quality of Life Specialists (HHA) to promote good health and achieve high quality of life.
ESSENTIAL JOB FUNCTIONS:
- Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the resident’s needs. Demonstrate good decision-making skills. Provide written evaluation and oral counseling of assigned staff.
- Assist in maintaining charting and documentation on each resident’s status.
- Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, DOH Regulations, and administrative policies.
- 4.Assists with completing vital signs and treatments as ordered by resident’s physician and/or nurse practitioner.
- Ensure physicians orders are carried out within a timely manner.
- Ensure Narcotic counts are accurate.
- Communicate with health care providers to ensure residents have all medication orders and supplies for their care.
- Communicate with resident family members as needed on all issues/concerns for residents.
- Participate in the assessment and admission of new residents. Participate in discharge planning as needed.
- Assist as needed with recruiting, hiring, providing orientation/training, and retaining nursing staff.
- Responsible for identifying resident medical and behavioral changes and developing and monitoring of residents’ care plan.
- Communicate with interdisciplinary team and Health Services Director on pertinent information regarding resident conditions.
- Assess and monitor complex resident health status and determine the appropriate nursing intervention.
- Make rounds with physicians and assist in examinations and/or treatment of residents.
- Perform other duties as assigned.
EDUCATION REQUIRED :
• Must be a graduate from an accredited school of nursing with a current New York State Registered Nurse (RN) license.
• CPR Certification is required.
• Experience with working with Alzheimer's and Dementia residents in an Assisted Living or Memory Care Setting is preferred
• Two (2) years of supervisory/management experience.
KNOWLEDGE/SKILLS:
• The ability to speak, read and comprehend the English language.
• Ability to learn and work with the specific needs of those residents suffering from Alzheimer’s disease and/or other related dementias.
• Willingness to be available for any/all emergencies regarding the facility.
• Must possess the ability to make independent decisions when situations warrant such action.
• Familiarity with Microsoft Office Suite products and the ability to work with an electronic medical record system.
• Agreement to complete orientation program for Heartfelt Connections Program and other community based training programs.
• Must display patience, tact, enthusiasm, and a cheerful disposition.
• Must have knowledge of basic nursing care techniques. Must be willing to learn new techniques and principles and willing to incorporate them into existing resident care practices.
• Must possess excellent communication and customer service skills
• Ability to work as a team and display a positive attitude
