Job Description:
Reporting to the Director of Nursing, the Nurse Trainer is responsible for providing nursing services to up to 20 Residents with developmental disabilities or mental illness living in group homes which provide opportunities for them to live in a home-based environment-achieving the highest level of independence possible.
Coordinates medical services and supplies with medical providers. Reviews and maintains medical documentation and audits health records and services documented by staff for compliance. Trains staff on health topics including medication administration, provides vaccines/injections, conducts tb screenings for employees/residents and documents health services. Provides on-call services during the assigned rotation schedule.
* Shift: Full-Time, primarily days with rotating on-call every 10-12 weeks.

Job Duties:
* Resident Relations: Promote a culture of person-centered services that advances the Company's mission to provide quality care and services in a community-based setting to individuals with disabilities. Ensure a high rate of resident satisfaction. Ensure the health and safety needs of all residents are met by providing nursing services to residents. Participate in the screenings and interviews of prospective Residents as requested. Attend all Resident PP/ITP/IHP staffing meetings or send medical summary prior to the meeting.
* Training & Administrative Support: Train direct care staff in Medication Administration in accordance with Rule 116. Maintains documentation of med training and re-training. Maintains med error logs and identifies trends with corrective action in quarterly NT review. Conducts quarterly Nurse Trainer audits. Submits billing for med administration classes in a timely manner. Trains staff on Basic Health and Safety and other health related topics as required by the Director or of Nursing. Provide immunizations and TB screening to residents/employees and maintain accurate records of the same.
* Provide Nursing Services and Oversee the Health and Safety Needs of Residents: Plan, organize, and supervise the delivery of care to residential residents. Complete annual nursing assessment, quarterly nursing assessment, TD, self-medication assessment and monthly observations, admission, NSP's, TB signs and symptoms check list, ensure annual influenza immunization and other immunizations are up to date, tetanus, pneumococcal, and hepatitis series. Visit assigned Residents no less than bi-weekly to evaluate their current condition and assist in healthcare planning. Participate in the screenings and interviews of prospective Residents as requested. Attend annual resident PP/ITP/IHP staffing meetings or send medical summary prior to the meeting. Review all labs and forward to the prescribing physician for review. Communicate effectively with physicians & service providers regarding Resident care. Order medical supplies as needed.
* Maintain and Review Documentation, Audit for Compliance: Maintain records of assigned Residents for medication charts, vaccinations, and annual assessments accurately and timely. Review all Physician Order Sheets and Medication Administration Records prior to being placed in the Resident's chart in a timely manner (at least monthly). Review and sign incident reports in assigned homes with follow up as needed. Review all medication error reports, complete medication error form and forward to Public Health, DHS, and OIG (Office of Inspector General) according to regulations. Conduct quarterly medication administration audit at each facility and report on errors in quality assurance review at monthly safety meeting. Ensure that each facility meets state, federal, CARF (Commission on Accreditation of Rehabilitation Facilities) and any governing agency's regulatory guidelines. Complete billing for Medication Administration training accurately.
* On-Call Services: Provide on-call services to assigned group homes from 3pm to 7am Monday through Friday and scheduled weekends/evenings. Respond to calls within fifteen (15) minutes. Answer calls in a timely manner and provide on-site services as required within 24 hours. Follows through with reported problems and communicates them to supervisor/primary care nurse. Check nurses' mailbox at group homes weekly.
Position Qualifications:
* Education: Illinois Professional Registered Nurse (RN) License with 1 year of RN experience and be certified as a Nurse Trainer by DHS (or obtain DHS certification within 2 months).
* Experience: 1 year of RN experience. Experience working with people with developmental disabilities or mental illness helpful but not required.
* Certifications/Licenses: RN License and Nurse Trainer license from DHS. CPR, Crisis Prevention Institute (CPI) and DSP (Direct Support Professional) training, including medication administration, provided by the company must be successfully completed within the first 4 months to be certified. Passing CPR and CPI training is also required annually.
* Pre-Employment Tests: Must pass state required TABE literacy assessment prior to employment.
* Computer Skills: Proficient with Microsoft Word, Excel, and Outlook.
* Background Checks:
* Must pass criminal background check.
* Must pass various State and Federal registry checks.
* Must pass DCFS Abuse and Neglect Tracking System check
* Must pass driving history check and Company policy criteria, maintain valid driver's license and be 21 or older to drive for Company related local travel.
* Driving: Use of personal insured vehicle for Company related travel is required.
Physical Demands:
* Sitting: Frequently sitting in the normal course of office-sedentary type work.
* Standing: Occasionally standing, walking, bending, squatting, reaching, and twisting in the normal course of office-sedentary type work and for training workers.
* Hearing, Speaking: Frequently listening to and speaking with Residents, employees, doctors, and other healthcare providers to communicate about health concerns, treatment services and training needs and other health related requests.
* Seeing: Continually visually alert to observe and assess Residents physically, to read and write or type documents, observe resident and employee interactions, and communicate with all.
* Handling: Frequently using hands in writing and typing, general office work and in physically providing health care service to the Residents.
* Movement: Kneeling required to perform CPR practice sessions, complete skills testing and use as required. Occasionally bending, reaching, and twisting; occasionally climbing stairs and/or ladders squatting.
* Lifting: May need to lift or carry up to 50 lbs. to assist a Resident to their feet from a lying or sitting position, to help with walking or for the Resident to change positions.

Responsibilities: In this role you will assume responsibility for all nursing care procedures and related health care activities for assigned clients across New Hampshire in both day and residential programs. You will provide direction and oversight to staff for the medical needs of individuals served, consistent with the State Regulations, guardians, physicians and other consulting staff and provide ongoing medical monitoring and quality assurance of residence according to Easterseals' medical policy and procedures.
In this role you will also provide medication administration training to staff and independent contractors and supervise staff in relation to medical and health treatment for individuals served. You will also complete nursing assessments for each individual assigned as well as provide oversight of ordering prescriptions, medical supplies, scheduling all medical and dental appointments and ensure follow-through with staff and follow up treatment as ordered.

We are seeking an experienced Registered Nurse Trainer to support our Vista Care Intellectual & Developmental Disability Services operations in Southern Illinois.
LOCATION / WORK TYPE:
* Work Schedule: Full Time; Hybrid schedule - 2-3 days in office with a flexible work schedule.
* Location: This position supports Vista Care's programs across Southern Illinois.
RESPONSIBILITIES:
* Leadership and Guidance: Provide overall direction to the Operations department by mentoring, coaching, and auditing direct care staff.
* Medication Administration: Conduct medication administration classes and supervise the medication certification practicum for new staff members.
* Health Monitoring: Maintain a high level of awareness regarding the health and medical needs of the individuals supported. Provide necessary medical information to relevant parties in accordance with HIPAA guidelines.
* Compliance and Regulation: Understand and develop practices that align with state regulations, quality standards, and other health and safety regulations.
* Training and Certification: Complete required classes and maintain certification in medication administration, CPR/First Aid/AED, and Positive Behavior Support training.
* Confidentiality: Maintain strict confidentiality regarding company information, including any information pertaining to the individuals supported.
* Provide nursing case management for the people receiving our services to ensure the BEST CARE possible.
* Complete onsite nursing assessments as needed.
