**PURPOSE** **AND** **SCOPE:**
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. **PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
**Education/Communication:**
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application in an accurate and timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.

**INFECTION CONTROL:**

**TRANSITIONAL CARE UNIT:**

+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.

**Job Summary**
The Patient Care Technician (PCT) provides high-quality, patient-centered care by performing delegated tasks in alignment with the PCT's training and the department's needs.     POSITION SUMMARY

    **Banner University Medical Center Tucson is seeking a Patient Care Assistant to join our Medical Surgical Unit.** The Medical Surgical Unit patient population consists of young adults to geriatric. Candidates may see patients with Diabetes, G.I concerns, Memory problems, Respiratory illnesses and more. Our Medical Surgical of currently 15 PCA's in this 28 bed unit, strives for excellence in this fast-paced, dynamic setting. We are looking for a Patient Care Assistants to join our dedicated, multidisciplinary team. Some key responsibilities include assisting with Daily ADL's , obtain V ital. signs, blood sugars, document intake and out put ETC and preparing patients for their procedures, such as, vital monitoring, safety checks, EKG monitoring and assisting with hygienic tasks. This position will also require transporting patients during admissions and discharges along with handling patient information. Equipment used includes Hoyer lift, Walkers , Bladder Scanners, EKG machines and Virtual Translators **.** You will be able to build and utilize your communication and time management skills by carrying out items in collaboration with the clinical and the support team.Y **ou as a** **good team member, self driven who demonstrates good work ethic, will fit well in this team!**
    + **Full-Time Day & Full-Time Night Shift position available (3 -12 hour shifts)**
    + **Shift Hours from (Days) 6am-6:30pm** **&** **(Nights) 6pm-6:30am**
    + The schedule is created a month or two in advance.There are no set day schedules.
    + Weekend Rotations are required in this role.Alternate weekends( 2 weekends a month) day shift Saturday and Sunday, Night shift Friday and Saturday.

    POSITION SUMMARY
    This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records and provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.
    CORE FUNCTIONS
    1. Provides assistance in activities of daily living and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed.
    2. Collects patient specimens and data, including vital signs, input/output and other delegated measurements in accordance with unit/facility policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.
    3. Recognizes and reports abnormalities and/or changes in the patient's health status and responses to treatment to a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.
    4. Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and according to state scope of practice; procedures may include blood draws, foley catheters removal, EKGs, and EKG monitoring. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional.
    5. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.
    6. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record.
    7. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
    8. Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in OR room turnover.
    9. This position works under supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with staff at all levels throughout the facility, as well as patients, families and physicians
    MINIMUM QUALIFICATIONS
    High school diploma/GED or equivalent working knowledge.
    Current BLS certification is required. Current Certified Nursing Assistant (CNA) in state of practice is required for new hires and transfers. In Tucson: Current Certified Nursing Assistant (CNA) in state of practice is required for new hires. Existing Tucson incumbents may not transfer out of their geographical market without meeting the CNA requirement. Requires skills and abilities typically attained with 1 year of medical clerical experience; preferably in a hospital or physician office setting, or prior experience in customer service. Working knowledge of medical terminology and computer literacy and keyboarding skills is required.

    Employees working at Banner Behavioral Health Hospital or BTMC Behavioral must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

    PREFERRED QUALIFICATIONS
    In Arizona the Licensed Nursing Assistant (LNA) licensure is preferred. Recent healthcare experience preferred. Successful completion of an approved unit secretary program is preferred.
