Provide direct care and assistance to the person with dementia that acknowledges the individual’s remaining strengths and needs.

Schedule: Every Saturday - Sunday 2PM to 10PM

Duties Include:

  • Answer telephone and assists visitors in a positive and friendly manner.
  • Conducts work task safely within the scope of your position and in compliance with Greencroft Middlebury policies.
  • Provides effective, courteous, and respectful service to all residents, co-workers, and guests.
  • Read notes in attendant’s notebook regarding events that have occurred on all shifts.
    • Performs other related essential duties as required.
    • Performs health and safety check on the residents other related essential duties as required.
    • Participate in regular team meetings and trainings.
    • Assist personnel and residents in establishing a home-like atmosphere.
    • Assist with maintenance and proper storage of equipment & supplies.
    • Fill out menus and take to kitchen for dietary to fill trays.
    • Help to prepare and serve meals during mealtimes. Prepare and serve meals to residence in dining area or in their room, gathering trays and cleaning up.
    • Assist resident in getting up from or ready for bed as needed.
    • Launder items when appropriate.
    • Perform light housekeeping tasks such as emptying trash, wiping down surfaces, vacuum and spot clean carpet, sweeping and moping, tidy up, making beds and changing linens as needed and cleaning of bathrooms.
    • Monitors the emergency call system for emergency calls and responds appropriately.

Education & Experience

  • High school diploma or equivalent.
  • Experience working with older adults.
  • Personal or professional experience as a caregiver for someone with a dementia diagnosis (Strongly Preferred)

As a **Hospice C.N.A /H.H.A.,** you'll contribute to our success in the following ways:
+ Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician's orders, assessment data, and established standards and guidelines.
+ Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes.
+ Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.
+ Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate.
+ Implements the plan for patient safety, using the patient, family, and community resources.
+ Assists the patient in learning appropriate self-care techniques while delivering quality patient care.
+ Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.
+ Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
+ Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.
+ Documents medication regimen and updates medication profile according to established procedures.

+ 50% travel required
+ Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
