Circare is seeking a Administrative Program Assistant for the Forensic ACT Team that will be responsible for the day-to-day functioning of the FACT Team.

Pay Range: $20.00- $1.63/hr.

• Coordinate team communication and manage schedules.
• Facilitate internal processes and procedures regarding program expenses and medical record management.
• Maintains CAIRS documentation.
• Maintains records of individual and program expenditures.
• Performs reception activities as needed.
• Responsible for utilizing state resources such as the Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) and the Regional Health Information Organization (RHIO) to inform the team members of any client information from other community medical providers.

• High School Diploma or GED required.
• One or more years of experience in a behavioral health or medial setting preferred.
• Must have a valid driver’s license and own transportation:
  • Circare’s insurance carrier requires that Circare employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100 000/person; 300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment. Circare is seeking a Registered Nurse for the Forensic ACT program, under the supervision of the Psychiatrist/NPP and/or Team leader will be responsible for providing medication management, assessing, and gathering of data concerning the individual's physical health, education, support services, rehabilitation, and providing medication therapy as part of crisis intervention services as needed.

Pay Range: $40.00 - $5.00/hr.

• Work under the supervision of the Psychiatrist/NPP and/or Team Leader.
• Communicate with supervisor regarding patients' medical concerns.
• Monitor patients' medical status, signs and symptoms of withdrawal and reports to supervisor and/or medical director.
• Work with other medical staff across the agency and in the community for continuity of patient medical care.
• Communicate with clinical staff regarding patients' medical concerns.
• Work directly with patients' in providing medical services, interpreting medical information and provide recommendations or make referrals for medical services.
• Develop, recommend, and review medical policies and procedures for all programs within Circare.
• Advise the agency and ensure the provision of services for medical and health related issues.
• Provide direct patient care of medical examination, implementation of medical orders, treatment of medical conditions and communicable and infectious disease reporting in accordance with the law.
• Execute and review public health education and screening protocols with regards to tuberculosis, sexually transmitted diseases, hepatitis, and HIV prevention and harm reduction in accordance with Article 27-F of the Public Health law and under the Mental Hygiene law in coordination with the Medical Director/Nurse Manager.
• Implement and carry out a medically based group for patients on the above topics.
• Serves on special review and quality assurance committees as assigned.
• Provide and document medical consultation, assessment, examination, and treatment planning and review consistent with agency policy and requirements.
• Other related assignments and duties consistent with training and experience.

• Must be a licensed Registered Nurse currently registered by the New York State Department.   
• At least one year of experience, or comparable training in integrated mental health and substance use services.
• Basic computer skills required.

JOB SUMMARY

• The Nurse supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Nurse will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility.

SUPPORT DUTIES

• Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of Alzheimer’s dementia and other diseases that cause dementia.
• Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff.
• Assist in oversight of the Memory Care Aides and Certified Nursing Assistants (CNAs), including supervision and assistance with ADL care, transportation, and non-scheduled activities by staff.
• Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail.
• Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication.
• Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training.
• Encourage family involvement through volunteering and visitation.
• Provide a clean, safe, non-hazardous unit for the residents and staff.
• Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented.
• Complete all required documentation.
• Maintain all program policies and procedures and state and federal regulations.
• Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach.
• Be flexible to meet the change in residents’ needs. We need to adapt to their needs rather than expecting them to adapt to our needs.

• Provide emotional, caring support to residents, families, and the inter-disciplinary team.
• Perform other duties as assigned.
