**Job Summary and Qualifications**
Role Summary : Demonstrates a high level of professional competence in the area of Medical Nutrition Therapy with emphasis on the education of people with such chronic diseases as oncology, diabetes, cardiovascular disease, hypertension, dyslipidemia, and obesity. Serves as an expert in the area of nutrition but also becomes competent in other areas of care and education of the population served. Collaborates with other health care providers and utilizes current standards and research to enhance the quality of education in the management of complex patient and family issues. Provides cost-effective quality education to patients, families, physicians, health care team members and the community (i.e. school staff, community groups).
**What qualifications you will need:**
+ **Required Education:** Bachelor's Degree
+ **Preferred Education:** Master's Degree
+ **Required License:** Registered Dietitian. Current licensure as a Dietitian/Nutritionist with the North Carolina Board of Dietetics/Nutrition. BCLS. For Nutrition Therapy Center: Weight Management Certification with American Dietetic Association within six months of role assumption. For Diabetes Center: Certified Diabetes Educator within two years of role assumption For Cancer Center: Certified Specialist in Oncology Nutrition (CSO) within two years of role assumption.
+ **Required Experience:** Three years recent clinical nutrition experience in relevant practice area.

Job Responsibilities
Provide Participant Centered Nutrition Services (PCNS) by phone or in person.
May conduct all duties of the Nutrition Assistant (NA) and Program Support Staff.
Assist participant with WIC Health online nutrition education.
Enroll participants by completing a nutritional assessment, program eligibility and determine nutrition risk.
Prescribe, tailor, authorize, issue, and reissue all food packages. Document nutrition services provided, including referrals and follow-up.
Review and approve formula requests requiring medical documentation in consultation with the Health Care Provider.
Conducts nutrition risk assessment to determine all nutrition risk criteria and program eligibility.
Identify the need for an Individual Care Plan and refer to the Qualified Nutritionist, as appropriate.
Carry out the High-Risk Care Plan established by the Qualified Nutritionist and provide participant centered nutrition counseling for the high risk participant. Work in conjunction with the Qualified Nutritionist to ensure continuity of care and to decide when the high risk care plan can be closed.
Coordinates nutrition education, including breastfeeding promotion and support that is responsive to the identified needs/interests of each participant.
Conducts breastfeeding assessments and provides counseling in regard to any breastfeeding concern or issues. Refer to Designated Breastfeeding Expert as needed.
Refers participants to other health-related and social services, as necessary.
Tracks high-risk participants' progress in improving their health and document for outcomes.
Ensure the efficient flow of the assessment and education processes.
Complete required Catholic Charities, WIC and DOH trainings.
Maintain continuing education credits as required by position.
Comply with the Agency's Compliance Program, Code of Ethics, laws, regulations, and ethical standards applicable to your position in the WIC program and Catholic Charities.
Provides leadership and assist in orienting, training and mentoring new Local Agency staff.
Provides oversight and guidance to the NA staff and support staff.

You'll Do**
+ Provide nutritional care for a variety of inpatient units, including higher acuity patients

**What You'll Do**
+ Be responsible for actively providing Medical Nutrition Therapy screening, assessing, and documenting the inpatient populations' nutritional needs
+ Implement care plans and educate patients (and family members) with a variety of disease states
+ Also will assist the Clinical Nutrition Manager with projects as requested and perform other duties as assigned
