We’re currently looking for an Occasional Care Companion in Rancho Santa Margarita, CA 92688

A kind and social woman in Rancho Santa Margarita is seeking a Care Companion to begin immediately. Support is needed 1–2 times every other month when her husband is traveling for work. Shifts may fall on any weekday, typically from 5pm-10pm (or as late as midnight). The family will provide at least one week's notice prior to each shift. The client enjoys baking, cooking, and watching movies. She uses a wheelchair and needs assistance with select daily activities, including reaching items for meal prep. She also values having someone nearby for safety in case of a fall. A Hoyer lift is available, and the family is happy to provide training if needed. An ideal candidate will be friendly, conversational, and dependable.

  • Start date: Immediate
  • Length of position: Ongoing
  • Schedule: Intermittent care - No guaranteed hours

Position Summary:

The physician provides diagnostic, preventative, and therapeutic services in Emergency Departments, outpatient and inpatient settings. In addition, as an expert licensed provider, the incumbent is responsible for both direct and indirect specialized child, adolescent, and adult services, administrative duties, and continuing education.

Scope of Work:

As a senior staff physician, the incumbent performs and provides the diagnosis, care, and treatment of primarily emergency care and some inpatients. The position is located at the Tohono O'odham Nation Sells Hospital, Emergency Department, Sells, Arizona. Incumbent works under the general supervision of the Clinical Director, who provides administrative oversight. Incumbent functions with considerable independence during patient care.

Essential Duties and Responsibilities: (Depending on the area of assignment, an incumbent may not be required to perform some of the duties listed below):

  • Provides preventative, acute, and emergency care for pediatric to geriatric patients primarily in the Emergency Department, with cross-coverage as needed for the Outpatient and Inpatient facilities.
  • Interviews and examines patients, reviews past medical history, and requests and performs diagnostic tests and examinations.
  • Makes preliminary diagnosis, recognizing rare and difficult to identity symptoms or signs.
  • Directs, prescribes, or provides a complete treatment regimen or arranges for specialized care or patient referral as necessary.
  • Make notes of observations incorporated into clinical records.
  • Manages and oversees all medical categories for cases that do not require referral.
  • Provide total patient care, including prevention, health maintenance, early diagnosis, treatment, and follow-up services.
  • Routine duties shall include, even if rarely required, providing medical care and public health services to individuals eligible for services provided by TONHC in homes, schools, clinics, job sites, and other community locations within the TONHC service area.
  • Make observations appropriate to the patient's underlying disease and treatment plan, including current symptoms.
  • Review patient medications, diet, therapy course, adherence to the treatment management plan, and other observations to develop, modify and assess the treatment plan.
  • Instruct and counsel patients and their families.
  • Performs and assists other physicians in the performance of minor surgery.
  • Delivers infants when necessitated by urgent/emergent circumstances and provides or arranges for postpartum care.
  • Coordinates and integrates information obtained into ongoing health care information systems utilized by TONHC.
  • Ensures preparation of appropriate medical records for all patients seen to ensure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care.
  • Disseminate clinical reports and correspondence concerning patients to physicians, hospitals, and other individuals and facilities.
  • Works closely with allied health professionals to provide effective interdisciplinary health care services and treatment.
  • Participates in a comprehensive Quality Improvement Program that meets the accreditation standards of The Joint Commission.
  • Ensures clinical care is consistent with standards of practice, guidelines, and department-specific policies and procedures.
  • Communicate effectively with patients and families in situations of medical, psychosocial, and cultural complexity.

In this role, you will be responsible for providing comprehensive pre- and post-care for our GI patients, as well as intra-procedure care as assigned. You will assess and address the physical, psychological, social, cultural, and developmental needs of each patient, delivering both direct and indirect nursing care within the framework of the physicians' plan of care.

This is an occasional position, filling in as needed at our Stevens Point facility, with the potential to work at our Wausau facility.

Essential Job Functions and Responsibilities

* Provides pre and post care of the GI patient, and intra procedure care as assigned; completes accurate documentation in the electronic medical record, administers IVs and medications, assesses patients, orders follow-up tests and appointments, and provides education to patients and families. Utilizes nursing process in the assessment and care of the patient prior to, during and after a procedure taking into consideration standards of nursing care.
* Provides competent care and sedation of the patient intra-procedure, as assigned.
* Completes preparation of charts as assigned, ensuring essential documentation is available for procedure and pertinent issues are addressed prior to procedure. Assures information required to successfully measure and report care quality measures are properly documented for all procedures.
* Delegates patient care duties to support personnel according to their level of knowledge and expertise, all while practicing the teamwork approach to patient care.
* Completes project work as requested, including but not limited to quality improvement, infection control, safety, recording meeting minutes, chart audits, medication and supply ordering, policy reviews, post calls/patient follow-up, and education of peers.
* Participates in the development, maintenance, and review of endoscopic policies, procedures, and protocols.
* Participates in various center committees (Quality Improvement, Infection Control, and Safety) as needed. Understands committee functions and works to achieve committee goals.
* Maintains proficiency in endoscopic nursing skills and equipment utilized throughout the delivery of patient care.
* Assumes responsibility for personal growth in regard to knowledge of current procedures, techniques and trends. Attends in-services and educational opportunities as presented.
* Completes all annual competencies related to the performance of the Registered Nurse.
* Serves as a resource and role model to peer and co-workers in the delivery of patient care, personal conduct, and appearance.
* Performs other duties as assigned.

Minimum Qualifications

* Education
* Associate Degree in Nursing required.


* Licensure/Certification
* Current Registered Nurse licensure in the State of Wisconsin required.
* BLS and ACLS certification required.


* Experience
* Minimum of two years nursing experience highly preferred.
* Experience in GI or an outpatient procedure setting preferred.

* Possesses interpersonal, communication, and listening skills necessary to deal effectively and courteously with patients, physicians, and all staff members.
* Maintain structure of the Endoscopy Center protocols while considering compassionate care for our patients.
* Proficient computer skills working in an Electronic Medical Record (EMR).
* Demonstrates professionalism and respect in all forms of communication and correspondence.
* Advanced clinical nursing assessment and documentation skills.

* Ability to work in a fast paced, multi-tasking environment and cope with rapidly changing patient care needs.
