The SUV Stand Up to Violence is a gun violence prevention initiative run out of NYC Health+ Hospitals/Jacobi . The SUV program is an evidence-based, violence reduction initiative that treats gun violence as a disease, by identifying its cause and it's causes and interruptingits transmission. The program hires a community outreach team of credible messengers - individuals who are hired from the communities in which they work and have backgrounds similar to those with whom they aim to connect - mentor the highest risk youth with an emphasis on conflict mediation and violence prevention.

In order to prevent violence, methods and strategies associated with disease control are implemented:

1. Detecting and interrupting conflicts -Outreach Workers (OW) identify and mediate conflict between high-risk individuals and groups in order to prevent violence and ongoing acts of retaliation.

2. Treating the highest-risk individuals the OWsmentor and support high-risk youth and young adults(ages 14 to 28) and connect them to educational, vocational, recreational and social services.

3. Changing social norms.

Opportunity Details :

  • Recruit and manage a caseload of 15 "high risk" gang/crew members and provide case management services. Responsible for mentoring these participants with a focus on providing alternatives to high-risk and violent behavior.
  • Build and maintain relationships with local gangs/crews,stay up-to-date on rivalries and plans for retaliation,facilitate conflict mediation.
  • Canvass the target areas on foot and identify and diffuse "hot spots" for shootings and violence.
  • Link participants and individuals in the target area to opportunities and programs in the community including vocational, educational, recreational, and social services.
  • Plan and participatein monthly community activities (cook-outs, basketball tournaments, etc.) in order to strengthen community awareness and garner support.
  • Engage in community shooting responses (marches, vigils, etc.) within 72-hours after a shooting, in order to mobilize the community against gun violence and change community norms.
  • Document all participantinteractions and conflict mediations in the program's computer database.
  • Attend ongoing trainings, individual supervision, and weekly staff meetings.
  • Communicate effectively with the team including other OWs, and supervisors.
  • Adhere to program safety regulations and remain in contact with a supervisor at all times.
  • Work flexible hours (mostly evenings and weekends) and respond to shootings or emergency situations.
  • Respond to violent trauma incidents and see patients in the hospital, when needed.

JOB SUMMARY: The outreach worker will work closely with families and Community School staff to support Community School Attendance Initiatives. The outreach worker will help support parents' skills, parent-child relationships, family relationships, and parent-school communication to increase student attendance.   • Implement "Success Mentors" program to improve attendance initiatives. Success mentors are the most successful strategy for combating chronic absence. Success Mentors are caring adults (or peers) who mentor chronically absent students to identify the barriers preventing them from attending school each day.
  • Assign "Success Mentors" to chronically absent students. Help Success Mentors understand that any topic is worth discussing! The idea is not for a Success Mentor to have the answers, but for the mentee to talk about and explore ideas.
  • Implement attendance incentive programs. Hold award ceremonies for improved attendance, create an attendance going culture, and manage incentive programs (ex. trips, gifts, shirts).
  • Work collaboratively with school and community to support school environment, bring in resources and advocate for families when necessary
  • Refer participants to social worker for counseling to support with crisis intervention and safety planning
  • Participate in ongoing efforts to advocate for program needs via community coalition building and via administering feedback surveys and program assessments
  • Seek partnership opportunities and bring in at least 3 new partners a year to leverage resources to support programs
  • Work closely with social worker, tutor, and case manager to support targeted student needs
  • Attend all required department/ agency meetings and/or trainings
  • Perform other duties as assigned by the Community School Director and/ or Community School Assistant Director

The department of Health Behavior & Society is seeking an Outreach Worker to work as part of a large community-level intervention aimed to reduce HIV risk and increase community cohesion among female sex workers in Baltimore, MD. The intervention is largely centered around providing services on a mobile unit and at a full-service drop-in center. Will be asked to conduct community outreach, in groups of two or more, to provide harm reduction services, as part of the SPARC Center, which has been operating in Pig town since the summer of 2017. Outreach workers will provide safer injection equipment and education, overdose response trainings, as well as conduct HIV/STI testing and harm reduction education. This will include the distribution of drug use equipment, condoms, resource lists, hygiene items, clothes, snacks and seasonal items. Prior to any community engagement, Outreach Workers will be trained to administer and distribute Naloxone and medication that can reverse the effects of an overdose of opioids (i.e., heroin, oxycontin, fentanyl), as well as trained in syringe service provision.)

*Outreach activities take place in Pigtown, Mount Clare, Carrolton Ridge, Curtis Bay and Brooklyn.

**Schedule: 4-18 hours per week. Tuesdays 11-4; Thursdays 11-4 and other outreach shifts as desired/available.

Specific Duties & Responsibilities

  • Promote the SPARC Center services and events (clinic hours, classes, support groups etc.).
  • Distribute Naloxone, train community members on its use and benefits.
  • Distribute safer drug use supplies and safer sex supplies, while providing harm reduction education.
  • Distribute promotional items, including condoms, hand sanitizer, lip balm, wipes, etc. Attend community meetings to promote services and develop rapport with neighborhood associations.
  • Build rapport with community members, including sex workers and local business owners.

Job Summary:

The Outreach Worker is a weekend part-time position under the OCC. The worker is responsible for coordinating engagement and assessment activities of the Project HOME outreach in accordance with OCC policies and procedures, as well as agency standards. Provide support and advocacy to assigned participants and unsheltered members living on the street and provide oversight and assistance in addressing problems and finding appropriate supports and resources to resolve any problems that arise on the streets and assist by coordinating with other agencies and businesses to solve problems. The Outreach Worker will conduct regular street outreach as scheduled and coordinate with businesses and other provider agencies. This position will document their engagements in the required city database systems, as well as Project HOME ETO database.

Essential Duties and Responsibilities

  • Engage, Assess and Build a rapport with homeless individuals with a focus on chronically street homeless individual and be familiar with spectrum of care and resources available to homeless individuals.
  • Conduct regular focus list meetings with case reviews with Team Leader and Admin staff.
  • Provide daily email summaries of significant placements and engagements.
  • Review daily placement opportunities, and shelter openings/closings.
  • Engage in regular staff supervision, training and team meetings.
  • Adhere to all outreach policies and procedures.
  • Coordinate your efforts within Project HOME Outreach team to ethically assist chronically homeless individuals.

  • Maintain a focus list of at least 25 individuals and update the list monthly.
  • Submit data entry, including PATH data, response outreach reports and individual contact and placements for PH outreach team. Review and submit all required reports to your supervisor and/or Senior Program Manager and ensure submission of PATH data for your contacts in the HMIS system.
  • Conduct regular street outreach activities with PH team to ensure focus list accuracy and adherence to contractual DBH requirements.
  • Assist team members with coordination at Project HOME sites and other agencies to ensure that their homeless case load individual needs are identified and assist the residential staff outlines a plan to transition participants into appropriate housing solutions.
  • Advocate on behalf of individuals to ensure their needs are being addressed and they have the appropriate levels of support and referrals to services.
  • Utilize required technology to ensure individuals are appropriately tracked in existing systems in a timely manner
  • Act as an outreach liaison by interacting with the public at large including visitors, residents, other staff and co-workers in a professional and welcoming manner.

The Outreach Worker is responsible for providing direct support to long-term homeless households who are connected to Guild for services. Guild does Outreach in Dakota, Scott and Anoka Counties. The Outreach Worker will work in partnership and collaboration with social service and housing providers to ensure that homeless households decrease the time between referral and being housed, make connections with other services as needed, and remain engaged.

Key Responsibilities:

  • Coordinate intake and perform an individualized needs assessment for all clients to develop a Housing Service Plan. Provide on-going assessment of client's progress and modify accordingly.
  • Contact individuals experiencing homelessness daily or as required weekly, at all locations where they may congregate in the community.
  • Provide transportation to the client for necessary services, including appointments and court proceedings, as required.
  • Assist the clients with the housing application process, complete the necessary paperwork for supportive and subsidized housing, and survey the rental market for affordable housing options.

LCLC reaches over 350 minors and emerging adults in the criminal justice system annually with the following services:

  • Community Holistic Defense for Juveniles and Emerging Adults: Free legal representation for indigent clients grounded in holistic defense practices which acknowledge that quality of representation and criminal legal outcomes are improved for youth when their unique developmental needs are simultaneously addressed by an integrated, interdisciplinary team of attorneys and social service providers.
  • Community Housing: LCLC owns and operates 20 units of studio style housing for emerging adult men, 18-25 years old, inside the Deer Community Justice Center. In addition, LCLC has created a network of approximately 275 private landlords and funders that place other LCLC clients and their families in subsidized affordable housing units.
  • Restorative Justice Community Court: LCLC was the lead agency that gathered numerous community and cook county court stakeholders together to design and launch the first Restorative Justice Community Court in the country in North Lawndale in September 2017. This court is based upon the tradition of restorative justice circle keeping and other restorative justice practices to enable repair and community integration for emerging adult men, 18-25, facing qualifying felony criminal legal charges. LCLC hopes to add a juvenile call to this court to serve minors 17 and younger under the same model.
  • Community Holistic Juvenile Diversion: Since 2020, LCLC has worked with the Mayor's office and the Chicago Police Department to close the Juvenile Intervention and Support Center (JISC) and create a new model of juvenile diversion for Chicago. LCLC is one of eight service providers in the city and focuses on serving youth from the west side diverted from the courts by the Chicago Police Department.


LCLC looks to continuously innovate our program portfolio to build upon new best practices for serving minors and emerging adults and better serve our communities.

Position Summary

LCLC's Outreach Worker is responsible for helping to reduce community violence and working with juveniles and emerging adults, 25 and under, who are of the highest risk of violence to help prevent shootings and retaliatory violence. More specifically, the responsibilities of outreach include but are not exclusive to being a part of the Community Holistic Defense team working in collaboration with attorneys and case managers identifying new potential clients and engaging existing clients in services, maintaining a caseload of 15 clients, mediating conflict, canvassing, monitoring and responding to community violence, serving as a liaison between LCLC and the community, and promoting the LCLC way of working to transform the lives of young people we serve and the criminal justice system as a whole. This individual will work on a rotating schedule of 10a-6p Mon-Fri; Tues-Saturday(DR1) 10a-6p along with being on call to respond to community violence up until 9p. Travel is required.

Responsibilities
  • Work with Juveniles and Emerging Adults (25 and under) who are of the highest risk of violence successfully engaging and/or re-engaging them in services and maintaining a caseload of up to 15 clients.
  • Maintains contact with clients on a weekly basis, at minimum.
  • Participates in Holistic Intake process with attorney and case management & completes eligibility screen for outreach services.
  • Complete the safety assessment and safety intervention plan that will be used to help identify appropriate services, safety risks, and needed resources, sharing findings with case management & legal.
  • Complete all required documentation in a timely manner utilizing the agency database.
  • Ensures timely information about potential and current clients and their safety is clearly identified and communicated, both verbally and in writing, to attorneys, case managers and Director.
  • Encourage and assist clients with completing individual goals identified in their work with case management.
  • Demonstrates supportive behavior and act as a role model displaying positive appropriate interactions.
  • Works with clients individually and in group setting on violence prevention skills (i.e., anger management, conflict resolution).
  • Coordinates and facilitates activities for workshops and groups for clients (peace circles, recreation, life skills).
  • Have knowledge and a clear understanding of street dynamics, trauma and the effects of trauma and operate from that perspective when interacting with clients, their families and the community.
  • Gain and maintain the trust of the community to help prevent shootings and retaliatory violence.
  • Serves as support and advocate for the clients and their families, identifying and relaying the need for resources to case management.
  • Canvassing the community and working in other unstructured situations.
  • Collaborate and network with other Outreach programs and workers to coordinate strategic responses to violence.
  • Monitor and respond to shootings or other forms of community violence in North Lawndale received through CPIC notifications, Citizens app and word of mouth as well as responding to violence in surrounding communities when necessary.
  • Willingness to work later hours during the week and during the weekends.
  • Coordinates and lead community events for clients, their families and North Lawndale community.
  • Market and promote the work of LCLC in the North Lawndale community magnifying our Holistic services to attract and identify potential clients in services.
  • Attend regular staff meetings, partner meetings, supervision meetings, community meetings as instructed by Outreach Supervisor and/or Director.
  • Appear in court on behalf of clients with attorneys as needed/directed.

  • Firsthand knowledge of North Lawndale and other communities served and knowledge of current street conflicts.
  • Some experience or training in crisis intervention, conflict resolution/mediation, and trauma preferred.
  • Experience working on a team.
