DISTINGUISHING CHARACTERISTICS: The administrative nature of the work requires considerable responsibility for making complex decisions regarding City-wide and interagency homeless program efforts and/or general recreational program efforts, in addition to managing the day-to-day recreation and community services programs. The Community Services Supervisor classification is distinguished from the Program Supervisor in terms of general complexity, overall scope of multi-agency coordination and involvement at the state and local levels to deliver important program initiatives. SUPERVISION RECEIVED AND EXERCISED Responsibilities include direct supervision of Program Coordinators and a variety of other full-time, part-time and contract personnel. Responsibilities include cross-management of a variety of other interdepartmental and multijurisdictional full-time, part-time and contract personnel related to recreation, community, and homeless services. Processes and prepares budgets, including general, special revenue, grants, capital improvement, and internal fundraising funds; revenue, fee rates and schedules, equipment, and personnel functions. Prepares and submits grant applications, funding requests, reports, and reimbursements for grants administered by the department. Manages the processing of pertinent documents such as regulations, contracts, fees, permits, petitions, bid specifications, requests for proposal, requests for qualifications, ordinances, and resolutions. Manages contracts and cooperative agreements for programs, services and classes. Supervises the work of contract partners, support personnel and sub-recipient partners. Prepares and presents formal reports for public presentation. Develops comprehensive plans to satisfy future needs of department and city-wide services around operational areas. Reviews programs and budget expenditures; assists in the development of the department budget; researches and prepares specific justifications. Serves as internal City public information officer. Develops and implements marketing strategies; manages city-wide web and social media official accounts, drafts news releases and public brochures concerning recreation and community services; and homeless continuum of care services. Evaluates program attendance, participant responses and costs, and makes recommendations regarding the continuance or cancellation of programs. Assists in the selection of department recreation employees and contract services for specialized homeless services; develops and establishes work methods and standards; conducts or directs staff training and development; reviews and evaluates employee/contractor performance; initiates disciplinary actions. Works with organized community groups and committees to organize and promote programs; acts as a Recreation & Community Services Department/City-Wide Homeless Liaison and/or acts as the Liaison for the Department for general and specialized recreation and community programs and services. Serves as a representative with school district officials, and other outside officials as assigned. Provides holistic and comprehensive services to people at-risk or those experiencing homelessness including: intake assessment, benefit assessment, goal setting, and short-term and long-term case management. Analyzes problems, identifies alternative solutions, projects consequences of proposed actions, and implements recommendations in support of general recreation programs and/or homeless program goals. Acts as a resource for concerns of departments, boards, and commissions. Acts as a leader and resource and directs staff activities with the public, City departments, City boards and commissions, and other organizations on projects and programs. Builds and maintains positive working relationships with co-workers, other City employees and the public using principles of good customer service. Maintains and expands collaborative relationships with a variety of non-profit agencies, school districts, faith-based organizations, and other service providers to maximize program offerings within the community. Participates in a variety of boards and commissions. Manages and facilitates the rental of facilities including staffing, scheduling and collection of fees in accordance with set policies and procedures. Conducts special studies as assigned by the Director and submits related reports. Supports the relationship between the City of Baldwin Park and the general public by demonstrating courteous and cooperative behavior when interacting with visitors and City staff; maintains confidentiality of work-related issues and City information; performs other duties as required or assigned. KNOWLEDGE OF: Standard program evaluation methods and report writing procedures. Current literature and sources of information in the fields of operations. EXPERIENCE AND EDUCATION A bachelor’s degree in recreation services, public administration or a related field from a college or university that is accredited by one of the national accrediting institutions; and Five years of progressive experience at the level of Program Supervisor in the implementation of recreation and community services activities and programs or other programs that fall under the oversight of the Recreation and Community Services Department. License or Certificate Possession of, or ability to obtain, a valid applicable California Driver's license and maintain a satisfactory driving record. SUMMARY OF POSITION:
Under the direct supervision of the Center Manager, the PACE Community Services Nurse is responsible for assessing medical level-of-care eligibility for prospective PACE enrollees, and for assessing and implementing a plan for community-based caregiving services. He/she is one of the first introductions for prospective PACE enrollees to the PACE Interdisciplinary Team (IDT) and works in partnership with the Enrollment Specialist to ensure a smooth and successful transition into the PACE program.
ESSENTIAL JOB FUNCTIONS:
- Completes level-of-care documentation and submits for review to the Department of Health Care Services (DHCS), initially for prospective PACE enrollees and as required annually for enrolled Participants.
- Presents prospective enrollee cases to the Interdisciplinary Team (IDT), as needed.
- Participates as an active member of the IDT for Participant care planning.
- Conducts comprehensive nursing assessments, reassessments, and in-home evaluations on prospective or enrolled PACE participants developing and documenting care plan services and/or enrollment recommendations.
- Coordinates with the IDT to develop an individualized in-home Care Plan.
- Collaborates with the Home Care Supervisor to oversee the implementation and continuation of care plan services in the participant's home.
- Ensures all participant changes are documented appropriately in the medical record and communicates participant changes to IDT in a timely manner.
- Assists with identifying qualifications and competency requirements for caregiving staff, and in providing training and competency checks.
- Supports family caregivers through health education and skills-training to ensure the Participant's care needs are met in a safe and timely manner.
- Collaborates with the Center Manager, Home Care Supervisor, and other IDT members to ensure the seamless and successful delivery of community-based services, both at onset and on an ongoing basis.
- Remains up to-date on home care standards and best practices.
- Provides effective leadership by coordinating and integrating services within the department, with other departments and with the IDT's primary functions.
- Provides care to participants in-person, via telehealth, and/or via telephone, as indicated.
- Provide home health as needed.
- Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic
- Performs other job duties as required by manager/supervisor.
- May provide after-hours on-call coverage to support PACE participants when the PACE Center is closed as needed.
Required:
- Valid unrestricted California RN license.
- Graduate of an accredited school of Nursing.
- Current BLS and First Aid required
- Valid CA driver license (no violations within the last 5 years)
- Bachelor's degree or higher in Nursing (BSN/MSN)
- Certificate in Public Health Nursing (PHN)
- At least two years' nursing experience, preferably in community-based or home health nursing.
- Experience in conducting home visits
- Additional training and/or certification in geriatrics.
- At least one year of experience working with a frail and/or older adult population.
- Must be able to fluently speak, read and write English.
- Fluency in Chinese (Cantonese) and/or Vietnamese is required.

The Director of Community Services will be responsible for all reports related to the community service block grant. Summary of Requirements: Bachelor's degree in social work, Human Services, or related field required (master’s degree preferred) and three years of experience in human service program management and supervision. Experience working in the nonprofit sector is preferred. Experience working with budgets, personnel management, customer database management, grants management/writing required. A strong background in program management, personnel management, and community engagement is required. Other experience requirements include resource development; grants management oversight of major programs; budget, planning, effective working relationships at the federal, state, regional, and local governmental levels; excellent written, verbal, and technology-driven communication and presentation skills; public speaking; and effective team-based leadership. Must have skills using Microsoft Word, Excel, Outlook, or other software applications to retrieve data, create spreadsheets and reports. Must always maintain confidentiality. Ability to plan, organizes, and prioritizes assignments to independently meet deadlines and complete tasks accurately, under the pressure of constant interruption. Demonstrated ability to analyze problems/issues, gather data and information, evaluate, and recommend alternative solutions, or effect solutions with a tolerance for ambiguity and change, when necessary. Knowledge of budget planning, preparation, and administration to make realistic, equitable fiscal decisions, present and defend budget concerns persuasively. Writing and grammar skills to independently draft correspondence about various subject matters' operations, policies, and procedures. Responsibilities And Duties Monitor and administer budgets and grants. Maintain required files, records, and complete reports. Oversee and review the client application process, determine program eligibility. Investigate any complaints. Supervise and evaluate program staff. Implement CAP60 within Energy Assistance. Maintain coordination with other agencies. Attend all meetings as required. Attend various conferences, task force meetings, and related functions. Seek funding to continue operations of Program activities and support. Identify and secure funds for program operations, expansion, and continuation. Prepare monthly programmatic and any reports required by funding sources. Participate in internal and external monitoring reviews. Develop programs, tools, and implement complex projects, to support the network in their strategic policy engagement, with a focus on TEFAP and other commodities Develop and implement initiatives and programs that support strategy to promote legislative and regulatory actions that increase food, funds, and resources for network members and individuals facing food insecurity.
