ESSENTIAL FUNCTIONS
THE POSITION

Under close supervision, the Youth Employment Participant will be assigned to the Recreation & Community Services Department or any other City Department. The incumbent may perform a variety of duties including, but not limited to: provide general information and assist the public by receiving, distributing and filing a wide variety of materials; front and/or back office duties including answering phones and data entry; assist in the preparation and distribution of marketing materials, park and facility maintenance; and perform other related duties as assigned.
When assigned to the Recreation & Community Services Department duties may also include: advise patrons of regulations governing the use of the recreation facilities; assist with recreation programs including working special events; may act as group leader, referee, and/or scorekeeper in a variety of games and activities; assist with activity set-up and perform other related duties as assigned.

  • Methods and techniques for specific recreation activities such as games, arts, crafts, music, drama, and other organized leisure activities.

JOB SUMMARY:

Under the supervision of the Intake Manager, the Participant Advocate is responsible for ensuring the smooth introduction and onboarding of all new enrollees into the PACE Southeast Michigan (PACE SEMI) Program. This includes - but is not limited to - escorting new intakes through their initial visit; introducing new participants to staff and other participants; providing education on PACE services; providing ongoing advocacy on behalf of participants; conducting weekly in-person or telephone follow up with new participants; and working directly with new participants and their caregivers for a smooth transition into PACE SEMI. This position is an intricate part of enhancing the customer service experience for new participants.

SPECIFIC DUTIES AND FUNCTIONS:
  • Serve as initial ambassador for potential participants and their caregivers.
  • Provide support for newly enrolled participants as they learn structure of PACE SEMI.
  • Represent Intake and Enrollment department in respective centers.
  • Follow up with potential participant after initial home visit prior to enrollment to collect additional information and answer potential participant's questions.
  • Present potential participants to the interdisciplinary team (IDT) prior to enrollment.
  • Develop initial care plan with home visit staff and IDT using assessments from home visit staff.
  • Organize tours for potential participants and/or their caregivers, if requested.
  • Facilitate pre-enrollment evaluations with PACE primary care or behavioral health, if required.
  • Facilitate pre-enrollment evaluations with Henry Ford Health System specialists - pain management, substance abuse treatment, oncology, etc. - if required.
  • Schedule center days with participant and PACE SEMI staff prior to enrollment.
  • Make welcome calls to confirm enrollment and welcome packets including information on care teams, staff contact information, emergency plan, and medication refills.
  • Conduct and document in electronic medical record (EMR) weekly in-person or telephone follow up prior to enrollment and for 90 days after enrollment to answer questions, provide education, and solicit concerns.
  • Maintain communication with all new enrollees according to process to ensure understanding of PACE and participants' needs.
  • Act as a customer service liaison with participants and IDT.
  • Other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Associates degree required; Bachelor degree preferred.
  • Ability to effectively work both independently and as a team member.
  • Demonstrate initiative and organizational skills.
  • Minimum of one-year experience with a frail or elderly population; knowledge of health, social, and behavioral problems of the frail elderly.
  • Demonstrate interest and ability to work in interdisciplinary team environment.

Simulated Participants or SPs perform the role of the “patient” during health care simulations or clinical skills trainings for learners in the Simulation Centers located on the Health Sciences campus or via Zoom as determined by the SP Manager(s). SP’s portray specific emotions, behaviors and disease symptoms while interacting with learners in the health care field during simulated patient encounters that may include direct physical exam maneuvers by learners. SP’s aid in establishing and maintaining fidelity and realism to each simulation.

SPs consistently simulate all aspects of the scenario: including history, affect/behavior and physical findings, as well as recall actions and dialogue for completion of evaluations and/or questionnaires for the purpose of scoring.

Roles and shift will be assigned by the Program Manager.

Responsibilities

Essential Functions:

  • Memorize a case script detailing specific emotions, behaviors, and disease signs/symptoms for presentation during simulated learning situations.

  • Present case information in a standardized manner, as elicited by students during simulated interactive patient history and/or medical examinations.

  • Remain in a specific patient character.

  • Embody roles with fidelity and realism.

  • Participate in group training and of entry-level standardized patients.

  • Maintain confidentiality of information related to cases, student behaviors and feedback evaluations.

  • Provide educationally constructive verbal feedback from the patient standpoint when prompted by the facilitator.

  • Perform additional job-related duties as assigned.

The Participant Experience Liaison is accountable for connecting PACE participants to our PACE system of care, and serving as a liaison between participants and their WelbeHealth care team. This role focuses on ensuring smooth onboarding to the PACE program, and plays a key role in supporting participants throughout their entire WelbeHealth journey.
**Essential Job Duties:**
+ Act as a liaison and maintain open communication with interdisciplinary team (IDT) disciplines, other care team members, and external providers to ensure the highest level of care
+ Assist management with participant experience and service recovery related special projects, such as implementation of satisfaction surveys
+ Facilitate communication among service providers, families, participants, and caregivers
+ Provide customer service calls to ensure satisfaction with IDT and specialty care appointments
+ Support the grievance process, providing high touch customer service and partnering with IDT and participant until resolution
+ Attend all required meetings and committees as directed by supervisor
+ Continue check-ins with participants to ensure they are no longer a risk for disenrollment and their concerns have been handled
**Job Requirements:**
+ High school diploma or equivalency; professional experience may be substituted
+ Bachelor's degree in communications or relevant field preferred
+ Excellent organizational, customer service, and communication skills in settings with seniors, their families, and IDT members
+ Ability to present information to various groups and leadership
+ Highly motivated, self-directed, able to execute tasks in a quickly changing environment, and able to make sound decisions in emergency situations

**Requirements/Prior Experience:**
+ At MINIMUM:
+ A master's degree (MBA preferred), with an emphasis in science or healthcare (For internal candidates, experience can be substituted for education)
+ 3 years of experience in the healthcare industry, with a preference for 5 years or more.
+ Previous experience in people leadership, project management, process improvement, strategy, consulting, and/or banking is preferred
