CommuniCare Health Services is currently recruiting a Patient Assessment Coordinator to identify patients for potential admission into our Akron/Youngstown, OH area facilities and to maintain positive relationships with current referral sources. The Patient Assessment Coordinator will facilitate admissions by coordinating transmission of clinical and benefit information from referral sources back to the SNF admissions teams, administrations, and third party payers.
The Patient Assessment Coordinator will preferably have an RN or LPN license, a degree in Social Work, or other medical credentials.
Essential responsibilities include:
- Performing clinical assessments
- Resource to the Liaisons
The Patient Assessment Coordinator achieves these objectives by providing excellent, responsive customer service and accurate, timely, and comprehensive clinical assessments and by proactively working with all departments in the facility.
Qualified candidates for this position will have an RN or LPN license in the State of Ohio, a Social Work degree, and/or other medical credentials. The ideal candidate will preferably have three years' treatment experience in a Long Term Care setting. Previous clinical screening experience preferred, along with recent healthcare experience in an Long Term Care facility. Candidates will also preferably have knowledge of the local medical community and an outgoing, energetic personality.
Job Requirements:
- Preferred: Current, valid and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, OR Social Work Degree, OR other medical credential.
- Current, valid and active driver's license required.
- Three (3) years treatment experience in a Long Term Care setting preferred.
- Previous clinical screening experience preferred.
- Recent healthcare experience in Long Term Care preferred.
- Knowledge of local medical community preferred.
- Ability to collect, organize, evaluate and communicate professionally and assertively, both verbally and in writing, pertinent clinical information with medical and administrative staffs, patients and families.
- Strong customer service skills and commitment to service excellence.
- Outgoing, energetic personality
- Ability to project a professional image.
- Knowledge of regulatory standards and compliance requirements.
- Strong organizational, prioritizing and analytical skills.
- Ability to make independent decisions when circumstances warrant.
- Working knowledge of computer and software applications used in job functions.

**Job Summary and Qualifications**
The IRF Patient Assessment Instrument Coordinator functions under the Rehab Program Director. Provides coordination for the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. This individual provides clinical support in completion of data contained in the PAI for each patient to ensure accuracy in the scoring of functional status and reflection of the patient's care needs. In addition, this position may assist in the ongoing monitoring of identified benchmarks for the inpatient population. The IRF PAI Coordinator will have access to confidential patient information and maintains confidentiality of all information.
**What qualifications you will need:**
1. Two or more years of experience as a licensed clinician in an acute hospital or rehabilitation setting preferred
2. Ability to operate basic computer and data management software
3. Two years' experience in inpatient rehabilitation setting, preferred.
4. Other licensed clinicians, such as MSW, COTA, PTA with two or more years' direct experience in PPSC role in an inpatient rehab setting will be considered.

+ (OTR) Occupational Therapist Registered, or (OTA) Occupational Therapy Assistant, or (LPT) Licensed Physical Therapist, or (LPTA) Licensed Physical Therapist Assistant, or (SLP) Speech-Language Pathologist
