The Registered Nurse provides safe therapeutic care in a holistic and systematic way, incorporating differences into the provision of care. Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education. Relies on education and training to complete responsibilities within scope of licensure as registered nurse. #CB

Responsibilities

* Clinical Practice and Care Management
* Performs psycholosocial/physiologic assessments that integrates changing data
* Formulates a plan of care based on trends of similar patients, including patient education and discharge planning. Tailors and prioritizes caring practice to individual needs, including cultural/ ethical/ spiritual needs.
* Participates in the planning of routine transitional health care needs. (i.e. treatment options patient placement options, end of life options)
* Responds to changes in patient's needs and provides holistic care using independent clinical judgment based on knowledge drawn from education and experience.
* Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care. Collaborates with patients/ families to identify realistic desired outcomes.
* Ensures efficiency in the delivery of care: determines immediate priorities and offers solution for problems.
* Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict.
* Identifies variances from expected outcomes.
* Evaluates patient outcomes and makes revisions in the plan of care.
* Documents all patient care activities as per documentation standards. Assists the healthcare team with the documentation process.

* Consistently communicates/collaborates with the health care team members, patients and families to maximize patient outcomes.
* Appropriately delegates patient care activities to other health care team members and participates in making and revising staff assignments to achieve maximum productivity

Qualifications

* Eight years of recent acute care experience
* Graduate of accredited school of nursing
* Per license requirements
* Will be required to be a Charge Nurse, a Preceptor, a Unit Champion for mandatory initiatives, and have mandatory annual health requirements. Proficient in the English language, verbal and written communication skills, computer skills
* Current Louisiana State license as RN, current ANCC-recognized certification, and BLS certification

The Patient Care Technician (PCT) is responsible for a variety of tasks and services to assist in the provision of care and comfort for patients under the direct supervision of a Registered Nurse (RN). The PCT will report and record observations and contribute to the efficient functionality of the department.

Responsibilities

Patient Care

• Partners with the RN and other healthcare team members to achieve patient care outcomes and goals of care, as identified in the plan of care.
• Obtains records and/or accurately enters data into the EMR, as identified in the PCT Scope of Practice. Competency validation and documentation of skill performance is required.
• Performs specific tasks as directed by the RN and other healthcare team members, following competency validation.
• May assist with responsibilities of the Unit Secretary, per department expectations and appropriate competency validation.
• Assist patients with activities of daily living.
• May assist in transporting medications that are not controlled substances from pharmacy delivery devices to medication storage areas on patient care unit, as well as transporting medications from patient care unit to pharmacy.
• Demonstrates an awareness of patient and personal safety.
• Incorporates St. Luke’s Comfort/C.A.R.E. Professional Practice Model in their daily care. 

Effective Communication

• Demonstrates good verbal and written communication skills with patients, families, healthcare team members and other customers. Verbal communication is clear and precise. Written communication is legible, if handwritten, and done in a timely manner using approved abbreviations.
• Verbally presents factual, concise, and timely information and/or patient care data to the RN, the healthcare team, patients and/or family members in a professional manner.
• Real-time EMR documentation is done in accordance with hospital and unit policies.
• Hand-offs will be done with attention to detail and with safety as a high priority. Hand-offs are utilized to communicate patient treatment and assessment information, patient education needs, goals of care and other quality care measures at each change of caregiver or setting of care.
• Collaborates with healthcare team to provide patient care.
  • Consults with RN and/or other healthcare team members, as appropriate.
  • Obtains instruction and supervision, as necessary, when implementing new or unfamiliar tasks or duties.
  • Complies with established department routines, policies and procedures.

Qualifications

• Maintain current BLS certification
• Writes, reads, speaks, and comprehends fluent English
• Be able to complete a basic computer course on the Electronic Medical Records

The PCT provides support services to the RN to achieve the goal of assisting individuals in the restoration and maintenance of maximal physical, psycho social and spiritual health. Sunnyview Rehabilitation Hospital is a 115-bed acute hospital specializing in physical rehabilitation. Each patient has a dedicated team of physicians, nurses, therapists and specialists . all focused on one goal . taking back what was stolen by a stroke, traumatic injury or disabling illness. What you will need:

• High School Diploma or GED

• Ability to do heavy lifting, pushing, pulling and standing for long hours

• Completion of 1 week SPHP PCT training program and unit specific competencies

• Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

• PCT Pay Range:$17.25 - $25.00
