The Patient Safety Nurse supports NOHS safety programs on a daily basis to meet identified strategic initiatives and performance improvement goals. Responsibilities:
* Monitors the online reporting system: Development, education, daily review of data, scheduling review of patient medical records for data collection, analysis of data and presentation to committees.
* Responsible for reporting restraint deaths from NOMC and NORH to CMS as required. Notification comes from Patient Services Department to the Patient Safety Department in a timely manner to be reported to CMS as per the regulation.
* Collects, assimilates, and aggregates data for trending and reporting purposes. Presents data to committees as requested.
* Maintains statistics and files in compliance with Joint Commission, State, and Federal regulatory agencies.
* Develops and provides educational workshops on the use of the hospital-wide occurrence reporting system. Assesses information by reviewing and trending occurrences.
* Develops education for all levels of staff to report occurrences. Performs assessment of patient care areas for patient safety, develops action plans for identified trends and assimilates data for improvement in patient safety activities. Monitors that improvement plan to assure compliance or modifies plan to fit the patient safety initiative.
* Making physical rounds in all patient areas to identify safety risks and works with leadership for enforcement of standards/goals/regulations. Notifies Leadership and Patient Safety Director of issues of compliance.
* Responsible for the patient fall program at North Oaks and presentation to Patient Safety Committee.
* Is responsible for collection and aggregration of patient fall data, assimilation of data, identification of trends and development of action plans to reduce falls for North Oaks Health System.
* Prepares data for Patient Safety Committee and other committees as requested.
* Facilitates as needed, CQI teams in response to identified trends that compromise patient safety. May serve as facilitator to start the team.
* Performing Root Cause Analysis, develops action plans from the RCA, conducts research to support change, develops bibliography and assures compliance with regulatory standards for RCA completion and assists with regulatory reporting as requested. Conducts research for patient safety initiatives/goals and develops or contributes to programs for patient safety.
* Receives and investigates reports of product problems to determine appropriate response in conjunction with the Biomedical Department. Assures that the SMDA process is followed. Report events to MedSun in a timely manner.
* Analyzes the online reporting system to identify the need for further review into the occurrence. Responsible for initiating a medical record review as identified by the event as a subcommittee of the Patient Safety Committee. Prepares a formal medical record review. Responsible for scheduling a medical record chart review meeting with administrative staff Assimilates information from that meeting to forward to the appropriate entity for follow-up.
* Ensures maximum confidentiality of and access to patient safety data.
* Provides aggregate analysis of patient safety data and maintains statistical reporting and trending of patient safety events and near misses.
* Assists with the referral system for staff to report potential claims involving the hospital and interfaces with the in-house legal department to assure that information is communicated and documented regarding the claim.
* Investigates and analyzes actual and potential events that pose a risk to patient safety. Interfaces with the Legal Department regarding analysis of events. Keeps the Director of Risk Management and Patient Safety apprised of all events that affect or could affect patient safety.
* Is a resource for medical/risk inquiries from physician, nurse administrators and others regarding emergent patient care issues and loss control. Maintains awareness of legislative activities that may affect patient safety programs.
* Interfaces with security on policy/procedure issues for protection of patients.
* Investigates patient responses related to patient safety and quality of care issues as requested.
* Other responsibilities as assigned in the area of patient safety and patient safety education. Participates in educational programs for the scope of clinical risk management and safety.
* Responsible for review of medical records where coding recognizes a Healthcare Acquired Condition (HAC) or Patient Safety Indicator (PSI) and forwarding appropriate information to the Administrator, Clinical leadership and Patient Financial Services per process.
* Works directly with and takes direction from Patient Safety Manager, CMO, and Physician Services regarding OPPE, FPPE, and physician review process.
* Accesses and uses Medkinectics database to support physician review processes.
