Responsibilities
Under the supervision of the Behavioral Health and Social Services Director, this individual will:
PROVIDE DIRECT SERVICE:
* Therapeutic Intervention and Social Services for high-risk patients.
* Conduct screenings for the Integrated Behavioral Health Program.
* Complete intake assessments, including client psychosocial history, current needs, diagnosis, and treatment recommendations.
* Provide short-term, individual, and family therapy to People's clients.
* Provide crisis intervention to patients as needed
* Provide information and referral for People's clients to other service providers (social, financial, and mental health, etc.).
* Maintain detailed current referral information.
* Document services provided to clients per guidelines and policies.
* Provide follow-up to patients to assess suitability of and follow through with referrals.
* Document referrals and rates of client follow-through.
* Identify opportunities to improve workflow, work processes, and patient care, and work cooperatively to implement these and other quality improvements.
* Adhere to HIPAA guidelines.
INTERFACE WITH OTHER HEALTH AND HUMAN SERVICE PROVIDER AGENCIES AND INDIVIDUALS:
* Communicate regularly with other service providers and referral sources to assure current information and to facilitate client access.
* Represent People's at appropriate interagency forums and meetings.
* Provide referral and information updates to other People's staff (medical, health education, and admissions) as needed.
* Develop professional relationships with referral sources.
* Develop and implement outreach strategies as needed.
OTHER DUTIES AND RESPONSIBILITIES:
* Attend all staff and team meetings
* Work cooperatively with departmental supervisor, team members, and general staff to ensure service standards and to identify and resolve problems.
* Participate in program evaluation and program planning in the area of responsibility.
* Provide leadership and expertise to the clinic staff in areas of mental health issues, therapeutic intervention strategies, referral methods, and other social work issues.
* Adhere to Clinic policies and procedures.
* Promote professionalism and ethical, client-centered interaction within the Clinic.
* Maintain current knowledge of issues related to mental health diagnosis and treatment; social work skills and practices; social service and public health issues related to clients receiving services.
* Supervise student social work interns.
* Provide instruction as needed to interns, residents, and nursing students on social work and mental health topics.
Qualifications
Education: Master's Degree in Social Work.
Experience: Prefer a minimum of two years' experience as a social worker and/or therapist.
Knowledge, Skills, and Abilities:
* Bilingual (English/Spanish) required;
* Ability to conduct a thorough psychosocial assessment, formulate a diagnosis, and make and/or implement treatment recommendations; crisis intervention; case management; familiarity with psychotherapeutic theories and strategies;
* knowledge and skill in individual and family therapy;

Responsibilities
Under the direction of the Nursing Manager, this individual's primary responsibilities include:
Patient care
* Participate in the coordination of patient care management as part of the care management team for high-risk populations.
* Provide health-related education as directed by the provider or as indicated by the nurse's assessment.
* Participate in collaboration with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care.
* Responsible for full compliance with all applicable regulations for the operation of a Class D Pharmacy.
* Telephone/walk-in triage of patients using approved national standards.
* Perform patient medication reconciliation in conjunction with the provider as part of the pre-visit assessments and care coordination, including coordinating refills with providers at time of visit.
* Follow up with patients as directed by the provider.
* Perform Birth Control Refill visits and patient education as needed.
Staff management
* Refer to and resource other extended care team members (such as the Unit Clerks, Medical Assistants, Health Ed, Referrals or other resources) to maximize patient flow and provider productivity.
Ancillary functions:
* Participate in chart audits as per protocols to monitor quality of care.
* Route incoming patient records and forms as appropriate.
* Communicate home health referrals and notify providers.
Support team and Clinic functions:
* Participate in care team huddles and other staff meetings as required.
* Participate in problem identification and resolution in functions relevant to duties.
* Maintain effective communication with Unit staff, other floor personnel, Clinic management, and all departments, acting as a resource within the clinic to other care team members.
* Demonstrate support for decision-making, policies, and procedures.
* Participate in quality improvement processes and projects, including policy and procedure development and updates.
* Participate in special projects.
* Identify opportunities to improve workflow, work processes, and patient care, and work cooperatively to implement these and other quality improvements.
* Adhere to HIPAA guidelines.
Qualifications
Education:
* RN, graduate of an accredited school of nursing
Experience:
* Two to three years of related experience or training

What you’ll be doing as a Behavior Therapist
- Apply the principles of one-on-one ABA therapy to help children with Autism reach their full potential by reading treatment plan and program descriptions, implementing treatment in line with written description, collecting data, writing insurance notes, and implementing feedback provided by the supervisor during observation sessions.
- Support the development of children in early childhood (ages 18 months to 8 years old) through engaging and fun therapeutic activities.

This role focuses exclusively on serving our patient population in a supportive, growth-oriented environment.We’re expanding into a brand-new 10,000 sq/ft facility next year, and this position has the potential to grow into a full-time role. You'll also be part of Intermountain Psychology and Counseling, with access to clinical education and mentorship from experienced mental health professionals.

Job Essentials
- Assesses the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment.
- Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in area(s) of responsibility.
- Diagnoses and treats various mental, emotional, and behavioral disorders.
- Performs social work consultation for patients and caregivers. Conducts psychosocial assessments. Provides supportive, crisis bereavement and other social work interventions as necessary.
- Develops a plan of care either individually or with input from the patient, patient family and significant others, physicians, and other members of the healthcare team.
- Modifies care plans to reflect changes in patient status.
- Refers patients to other services to improve their well-being. These referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers.
- Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues. Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.
- May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.
- Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.
- Works with the multi-disciplinary team to develop a discharge plan in order to provide further care, services, and referrals in order to ensure continuity of care.
- Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing/reimbursement standards.
- Complies with federal, state, local, and clinical program/facility standards.
- Acts within the scope of the Mental Health Practice Act.
- May identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.
Minimum Qualifications
- Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.
- Current State Licensure as LCSW.
- Computer Skills, i.e., email, word processing.
- Excellent written and verbal communication skills.
Preferred Qualifications
- Experience with patients, families, and treatment teams.
- Experience with Play therapy, Post Partum Depression and POTS (Postural Orthostatic Tachycardia Syndrome).
Physical Requirements:
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).
