THE POSITION

A Bridge Boston Charter School Per Diem Nurse can expect to:

Postoperative- assists with: the application of the patient's dressings; transfer from the OR bed; accompany the patient to the PACU if required; and clean up and turn over of the operating room.

May assist with specialty procedures including minimally invasive vein/arterial harvesting, closure of deep fascia layer, insertion of invasive monitoring lines and cardiac sternal wound closure.

IF PERFORMING RN CIRCULATOR DUTIES:

Maintains clear communication and rapport with patient/family as appropriate for setting. Gives and documents emotional support and keeps patient/family informed of progress.

Assures that patient's personal hygiene needs are met, immediate environment is clean and safe.

Maintains a current and complete medical record. Coordinates admission, transfer and discharge of patient with appropriate personnel and documents observations, assessment treatments and outcome as necessary.

CARDIAC CATH LAB ALSO RESPONSIBLE FOR: Coordinates overall clinical program and ensures processes are in place to assure smooth operations and patient flow for the clinical activities of cardiology services specific to heart valve devices (TAVR).

Develops and implements required elements of an effective clinical program such as:

Develops methods to ensure prompt access to care.

Ensures processes are in place to provide coordinated patient flow across Physicians clinical sites.

Develops collaborative partnerships with inpatient and outpatient facilities implementing the heart valve

device.

Develops & implements clinical guidelines.

Aligns EMR to support care delivery.

Facilitates establishment of clinical trial protocols at all sites in partnership with the Valve Consortium.

Provides program and clinical input to marketing team to ensure web and related marketing tools are up to date.

Practice Development:

Develops & provides oversight of clinical program at affiliated hospitals.

Directs development of education materials.

Develops educational materials and training program for all patient care staff (inpatient and outpatient) who will participate in the clinical trial.

Helps develop valve program at affiliated hospitals that provide a referral base of patients.

Position Summary:

This position supports the Wabanaki Center for Healing and Recovery's Cedar Road program. As a Nurse this individual must provide compassionate and culturally sensitive care to clients experiencing symptoms related to substance use, including withdrawal symptoms, cravings, and other issues that may arise. This individual will also provide support and counseling to help clients through their recovery process to include the administration of medications to manage withdrawal symptoms, make sure a client feels comfortable, supported and well taken care of throughout the detox process. This position works collaboratively and efficiently with a multidisciplinary team and the client through direct care, transitional support, coordination of services, and referrals.

This is a Per Diem Position. Hourly rate plus $5.00 per hour pay differential for any hours worked between 7:00pm-7:00AM. Some travel may be required.

Duties and Responsibilities:
  • Complete nursing intake and assessment during admission. This includes meeting with client and/or family as needed.
  • Process, manage, and implement orders from Licensed Independent Practitioners.
  • Monitor client's vital signs as ordered or per protocol.
  • Dispense medications as ordered and per policy and procedures. Monitor and document all medication activity. Keep medication administration record (MAR) up to date, assuring accurate Medication Reconciliation and prescriptions for clients at discharge.
  • Maintain medication chart in order and locked.
  • Observe, note and report reactions to psychotropic drugs and other medications.
  • Ensure compliance with Do Not Use Abbreviation and Sound Alike/Look Alike medication lists.

RESPONSIBILITIES:

The Staff Primary Care Nurse Practitioner's/ Physician Assistant's primary responsibility is to provide consistently effective primary care clinical services to program clients. This includes the direct provision of medical and pharmacy services; consulting with the Urgent Care Clinic on complex cases involving both medical and psychiatric concerns; establishing and maintaining effective relations with clients, client families and support people, and with the general community; connecting clients with outpatient primary care resources, and facilitating appointments; and maintaining a comfortable, safe, non-institutional ambiance in the program. Services must be provided in a manner that is safe and helpful for clients, staff and the general community, and that evidences a non-institutional character that is congruent with Social Rehabilitation values and principles.

The Staff Primary Care Nurse Practitioner/ Physician Assistant is responsible for the direct physical clinical assessment of all newly admitted clients, the on-going monitoring and care of all clients being treated at Dore Residence, and the proper referral of clients to outpatient services upon discharge from the Residence.

The Staff Primary Care Nurse Practitioner/ Physician Assistant must demonstrate the on-going capacity work as an interdisciplinary team member in order to effectively respond to the needs of a diverse client population in psychological distress.

Under the direction of the Program Director and the medical supervision of the Primary Care Consultant, ensures that services are provided in a manner consistent with prevailing standards of medical care, with applicable Federal, State, City and County laws and regulations, and with Progress Foundation policies and procedures.

Direct services include screening, full history taking and physical evaluation and documentation, in situations where treatment if appropriate, and referral for the full spectrum of non-emergent and occasionally emergent ambulatory primary care conditions presented at the Residence. This will also include arranging a safe and timely transfer of clients to other needed treatment services.

LEADERSHIP
  • Provide services within the Residence that are congruent with Social Rehabilitation values, and promote SR values and philosophy within the Behavioral Health treatment community. (clinical)
  • Attend staff meetings and other program meetings as needed, as well as special clinical and administrative meetings both inside and outside of Progress Foundation as required to promote the Agency goal of diverting clients and providers from the unnecessary use of institutional services. Qualifications

  • MINIMUM QUALIFICATIONS:
  • All qualifications must be met consistently and continuously.
  • Degree in nursing from an accredited school of nursing, and certification as an Adult Nurse Practitioner. Previous experience providing primary care nursing services in a psychiatric setting preferred.
  • Current valid California licensure as a Registered Nurse. Must demonstrate ongoing clearance with the California State Department of Social Services, Community Care Licensing Division, allowing for direct contact with clients in licensed programs.
  • Must be able to communicate effectively with residential counselors and other service providers both orally and in writing.
  • Must have the willingness and ability to actively, enthusiastically and effectively engage psychologically distressed clients and maintain a respectful and responsive attitude while providing primary care services. This includes being respectfully responsive to persons of differing ethnic, cultural, gender, socioeconomic, and sexual-orientation backgrounds.Demonstrated on-going emotional maturity, good judgment and effective teamwork.
