Job Summary:
We are seeking a compassionate and dedicated Personal Care Assistant to support clients with their daily living activities and promote independence, dignity, and well being. This role is ideal for individuals who are passionate about making a meaningful difference in the lives of others.
Key Responsibilities:- Assist clients with personal hygiene tasks, including bathing, dressing, grooming, and toileting.
- Support mobility needs, including transferring, positioning, and ambulation.
- Prepare and serve nutritious meals based on dietary requirements.
- Provide medication reminders and basic health monitoring (as allowed by law).
- Assist with light housekeeping duties such as laundry, cleaning, and organizing living spaces.
- Offer companionship and emotional support to reduce feelings of isolation.
- Escort clients to medical appointments, errands, or social outings, as needed.
- Observe and report changes in the client’s condition to supervisors or family members.
- Maintain accurate documentation of care provided.
- Excellent communication and interpersonal skills
- Strong sense of empathy, patience, and compassion
- Ability to follow detailed care plans and instructions
- Good time management and organizational skills
- Physical ability to perform care tasks, including lifting and standing for extended periods
- Discretion and respect for client confidentiality and privacy
- High school diploma or GED required
- Certification as a Personal Care Assistant, Home Health Aide (HHA), or Certified Nursing Assistant (CNA) preferred (may be required depending on state regulations)
- Previous experience in a caregiving, healthcare, or home care role is strongly preferred
- Valid CPR/First Aid certification is a plus
- Must pass a background check and any required health screenings

Provide in center laundry services including washing and folding clothes
Assist in identifying signs and symptoms indicating physical, emotional or behavioral changes in participants and report abnormal findings to the clinic
Escort participants to appointments as necessary

Assignments rotate between in facility and home services
**Minimum Qualifications:**
· High School Diploma or equivalent required.
· Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
· Experience in a long-term care facility or with a community-based geriatric program preferred.
· Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change.
· Ability to work with minimal supervision and exercise independent judgment.
· Superior written and verbal communication and presentation skills appropriate for audience comprehension.

**Position Purpose:**
The personal care assistant/worker is responsible for providing various care services to program participants including in center care and home needs to assist with activities of daily living
This role serves the underserved geriatric population by meeting their needs in the community to help with maintaining their independence in the home setting.
The position is Monday-Friday with rotating weekend and holiday coverage.
**Position Details:**
This position will primarily consist of home care assignments. However, occasional facility work will also be required.
Position will require working one weekend a month and rotating holiday coverage.
**What you will do:**

Obtain vital signs and blood glucose readings as ordered
Provide personal care assistance with activities of daily living including toileting, feeding assistance, showers and grooming in the home and facility
Provide in center laundry services including washing and folding clothes
Assist in identifying signs and symptoms indicating physical, emotional or behavioral changes in participants and report abnormal findings to the clinic
Escort participants to appointments as necessary
In home personal care services
In home light housekeeping
In home light meal prep
Assignments rotate between in facility and home services.
**Minimum Qualifications:**
· High School Diploma or equivalent required.
· Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
· Experience in a long-term care facility or with a community-based geriatric program preferred.
· Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change.
· Ability to work with minimal supervision and exercise independent judgment.
· Superior written and verbal communication and presentation skills appropriate for audience comprehension.
· Valid drivers license and use of personal vehicle for home care visits
· Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service.
· Position may require 10% travel within the Centers geographic region and use of personal vehicle.

- Help clients dress, bathe, eat etc.
- Assist clients with physical therapy exercises
- Cook meals according to the customer's restrictions
- Help customers with their shopping
- Do necessary housekeeping to maintain a clean environment
- Provide emotional support
- Inform physicians or supervisors about uncommon events
- Act quickly and responsibly in cases of emergency
Qualifications:
- At least 18 years of age.
- PCA/ HHA certification
- Have a sympathetic attitude toward the care of the sick and elderly.
- Ability to carry out verbal and written directions.
- Caregiving/ Direct Patient Care: 1 year (Preferred)
