Job Description

Ambulatory care clinical pharmacy practice is the provision of integrated, accessible health care services by the Pharmacotherapy Care Specialist.

This individual is accountable for addressing medication needs, developing sustained partnerships with patients, and practicing in the context of family and community. This is accomplished through direct patient care and medication management for ambulatory patients, coordination of care, patient advocacy, wellness and health promotion, triage and referral, and patient education and self-management.

The ambulatory care Pharmacotherapy Care Specialist may work in either an institutional or community-based clinic involved in direct care of a diverse patient population. The incumbent works in a variety of environments in either a hospital and/or community-based settings. Each site has its own unique practice style and may focus on primary care or focus on one specific disease state.

Responsibilities

1.Optimizes medication therapy by selecting, initiating, monitoring, continuing, and modifying medications based on patient-specific parameters and goals. The Specialist can perform therapeutic drug monitoring with dose adjustments as per evidence based guidelines.

2.Follows national guidelines and evidence-based medicine for medication therapy management of disease states including but not limited to;

a.COPD

b.Asthma

c.Smoking Cessation

3.Provides education to patients, family members, and/or caretakers about disease states and medications, including;

e.Interacting foods, disease states, and drugs where appropriate

h.Recognition of side effects of therapy and the importance of notifying his/her physician if any should occur

i.Importance of adherence to pharmacotherapy and appointments

5.Serves as an educational site preceptor for post-doctorate pharmacy residents and doctor of pharmacy students
