Job Description Summary:

Performs basic supportive care to specified patients and families under the direction and supervision of an RN or Care Manager. Collects, documents, and reports data to the RN or Care Manager.

Job Description:

Essential Functions:

  • Collects, documents, and reports vital signs. May complete screening questions and documentation or minor specimen testing as assigned.
  • Promotes general patient safety. Holds and positions patients and assists with procedures. Prepares materials and supplies as delegated.
  • Remains compliant with HIPAA, maintains professionalism, and ensures confidentiality of patient information.
  • Ensures environmental standards are met and all unit and patient areas are stocked, cleaned, and maintained.
  • Performs clerical and administrative tasks as necessary.
  • Completes continuing education and participates in offered educational opportunities. May act as a preceptor and train other staff.


Education Requirement:

Completion of fundamentals course from an approved nursing program, or equivalent licensure or experience, required.


Licensure Requirement:

Active State Tested Nurse Aide License in the State of Ohio, or equivalent education or experience, required.


Certifications:

Current CPR certification (BLS for healthcare provider), required.


Skills:

  • Effective written and verbal communication, interpersonal and problem-solving skills.
  • Must demonstrate competence as defined in the Patient Care Assistant responsibilities/skills checklist in providing age appropriate care and services to pediatric patients of all ages and selected adult patients.
  • Displays a positive presence and interacts with patients, families, staff, outside vendors, consultants, and physicians.


Experience:

One year of patient care experience in a hospital setting within the last three years, or equivalent education or licensure, required.

Nuvance Health Medical Practice is seeking a full-time Nurse Practitioner/Physician Assistant to join our Physical Medicine practice in Newtown, Connecticut. This is in a beautiful new multiple specialty practice setting. Clinic includes, but is not limited to, procedure follow ups, trigger point injections and referrals to therapy. Position has standard Monday through Friday office hours. Potential weekend hospital rehabilitation unit coverage is paid at an additional rate.


 


Candidates should be a board certified Nurse Practitioner or Physician Assistant. At least one year experience as an APRN/PA in Pain Management, Physical Medicine and Rehabilitation, Orthopedics, or Rheumatology in an outpatient setting is preferred and/or or equivalent nursing experience. These assessments will assist the VA in making accurate disability determinations.

Key Responsibilities:

  • Review and analyze veterans' medical records through a secure web portal.
  • Conduct comprehensive, non-treating assessments of veterans to document current health conditions.
  • Complete and submit DBQs based on VA-specific templates to capture all relevant medical findings.
  • Ensure that all DBQs are thorough, accurate, and submitted within 24 hours of the assessment.
  • Assess up to four veterans daily.

Schedule:  Part-Time in person. Traveling between McAllen, Laredo, and Corpus Christi

Ideal Candidate Profile:

  • Motivated to perform independent medical assessments in a non-treating, investigative role.
  • Proficient in analyzing and synthesizing data from comprehensive medical records and physical assessments.
  • Skilled in managing time effectively, completing tasks within deadlines, and maintaining a focus on quality.
  • Comfortable and efficient in digital documentation and record review.
  • Dedicated to serving veterans and making a positive impact in their lives.

Requirements

  • Active MD or DO license with a specialization in Physical Medicine and Rehabilitation (PM&R).
  • Board certification in Neurological Surgery, PM&R, or Psychiatry and Neurology is preferred; candidates with a completed two-year residency in a relevant specialty will also be considered.
  • Strong typing skills and attention to detail are essential.
