- Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
- Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
- Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
- Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
Qualifications:
- Physical Therapy license is required.

What You'll Do as a Physical Therapist (PT)
Deliver physical therapy services in accordance with physician's plan of care and state PT practice guidelines
Assess and evaluate patients' therapeutic, rehabilitative, and functional status
Develop and direct individualized therapy programs focused on rehabilitation and recovery
Evaluate home environments and recommend safety/therapy modifications
Train patients and caregivers in therapeutic appliances and prosthetic device use
Provide instruction and support to families/caregivers and collaborate during case conferences
Supervise and guide Physical Therapy Assistants (PTAs) per policy and regulations
Monitor and report patients' responses to treatment, adjusting plans as needed
Prepare clinical and progress summaries, participate in discharge planning, and coordinate with interdisciplinary teams
What We're Looking For
Active Physical Therapy license in Indiana and/or Ohio
Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
CPR certification and recent TB test required
Valid driver's license, reliable transportation, and auto insurance
2+ years of physical therapy or rehab experience preferred (community/home health experience ideal)
Strong communication, organizational, and leadership skills
Ability to build rapport with patients and handle physical workload
