Position Summary:
Previews and interprets the documentation of physician services in patient records to report physician activity for professional services billing. Key components of the reporting of physician services include coding of services using CPT-4 service codes and ICD-10 diagnoses codes. Must apply Departmental standards for the documentation and reporting of physician services in determining the correct level of service supported by the documentation of physician services in the patient record.
- Systematically review and analyze patient records to determine the principal diagnosis and level of service, all significant additional diagnoses and procedures and to produce a coded abstract for physician billing (60%)
- Review up to 90 patient charts per day and abstract diagnoses, physician services and procedures with 95% accuracy, 100% of the time.
- Abstract charts of patients no longer under care of practice physicians within two working days of discharge from care, 90% of the time.
- Code diagnoses, level of service and procedures consistent with patient care documentation (20%)
Complete ICD-10 and CPT-4 coding with 95% accuracy, 100% of the time. - Serve as a resource to Department physicians regarding guidelines for patient care documentation and respond to physician inquiries on proper documentation of patient care (10%)
- Complete a Documentation Problem Sheet within four hours of identification of a problem/issue, 90% of the time.
- Respond to physician inquiries within two hours with recommendations and/or options for problem resolution, 90% of the time.
- Maintain a thorough knowledge of current diagnosis and procedural coding practices and their application to the Product of services provided by Department physicians (10%)
- Maintain certification as a procedural coder.
Education:
- Degree in Health information management or equivalent experience
- 1-3 years Coding Experience.
- Strong computer, administrative, and organizational skills (written and oral) required
- Required Licenses/certificates/
- C.P.C. Certification (Certified Procedural Coder)
Skills:
- Ability to read and analyze medical records.
- Clear oral and written communication skills.
- Professional telephone etiquette.

JOB FUNCTIONS
Essential Functions • Systematically review and analyze patient medical records to determine all appropriate diagnosis and procedures performed, and to produce coded abstract for physician billing. • Submission of coded services for billing and or data entry of the coded services for claim submission in accordance with departmental productivity and accuracy standards. • Review of inpatient data and reconciliation of billable services including, review of system processing and appropriate escalation and timely communication of errors and omissions and clinical documentation insufficiencies. • Review and resolution of coding related edits and errors that impact claim submission. • Maintaining thorough knowledge of coding and documentation requirements outlined by CPT, ICD-10 CMS and CHOP Compliance for all physician services performed.

**The role you'll contribute:**
The Medical Assistant, under the direction of the Office Manager and/or Clinical Supervisor, provides appropriate care to patients within the scope of the Medical Practice Act. Assists in patient education. Assists physician during treatment and examination of patient and administers procedures and treatments as ordered by the physician.

Assists with rooming patients for appointments. Obtains accurate temperature, pulse, respiration, blood pressure, height and weight. Documents correctly on progress note.
Prepares patient for physical examination. Screens patients for appropriate information. Instructs patients in collection of samples and tests.
Explains procedures and treatments to patients and when necessary to significant others prior to implementation.
Reviews patient charts to ensure that all test results and correspondence are present prior to the physician seeing the patient.
Assists with the preparation of equipment and aids physician during treatment, examination, and testing of patients
Qualifications

High School Diploma or Equivalent
Graduate of a recognized Medical Assistant program or 3 years experience
BCLS - Basic Life Support

**The role you'll contribute: Medical Assistant ~ Certified**
The Medical Assistant, under the direction of the Office Manager and/or Clinical Supervisor, provides appropriate care to patients within the scope of the Medical Practice Act. Assists in patient education. Assists physician during treatment and examination of patient and administers procedures and treatments as ordered by the physician.
+ Assists with rooming patients for appointments. Obtains accurate temperature, pulse, respiration, blood pressure, height and weight. Documents correctly on progress note.
+ Prepares patient for physical examination. Screens patients for appropriate information. Instructs patients in collection of samples and tests.
+ Explains procedures and treatments to patients and when necessary to significant others prior to implementation.
+ Reviews patient charts to ensure that all test results and correspondence are present prior to the physician seeing the patient.
+ Assists with the preparation of equipment and aids physician during treatment, examination, and testing of patients.
+ Administers prescribed medications and treatments in accordance with AdventHealth Division policy and procedures and nursing standards. Documents medications completely in the patients chart after they are given.
+ Reviews appointment bookings and ensures preferences are given to patients in emergency situations. Maintains timely flow of patients.
+ Responds to and returns in-coming telephone calls in a timely manner. Instructs patient and family regarding medication and treatment instructions.
+ Responsible for preparation of exam rooms including restocking of treatment areas and disinfecting of exam tables and counters.
Qualifications
**The expertise and experiences you'll need to succeed:**
+ Minimum of a High School Diploma or Equivalent
+ Graduate of a recognized Medical Assistant program or 3 years' experience
+ BLS (condition of employment)
+ CMA /RMA
+ Registered or Certified from a recognized Medical Assistant Program

The **Certified Medical Assistant** is responsible for assisting in providing clinical care to select patients under the supervision of the professional nurse or licensed medical practitioner and provides clerical office assistance.

+ High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
