VHCS Clinical - Private Duty
**Job Summary:**
Works in collaboration with the RN or a licensed healthcare team member to provide direct care to patients, both pediatric and adult in the home setting.
Shift: M-F 8-hour and 12 hour shifts

KEY RESPONSIBILITIES
- Collaborates with patients/families and health care providers in the delivery of health care services
- Partners with the healthcare team by following established plan of care to ensure patient progress for effective outcomes

- Identify and assess patient needs and clinical changes, accurately reporting assessment findings in a timely manner to Supervising RN and physician
TECHNICAL CAPABILITIES
- Clinical Competency (Intermediate): Demonstrates mastery of nursing interventions (such as wound care, point of care testing, vitals signs, etc.) in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering vital sign services without requiring support and instruction from others. Able to train and educate by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job.
- LPN Patient Education (Intermediate): "Role models patient education in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering health education services without requiring support and instruction from others. - LPN Care Planning (Intermediate): Participates in the development of the plan of care in consultation with the licensed healthcare team. Plans for and anticipates events outside of immediate responsibility. Anticipates problems and obstacles before they arise. Position in this function is responsible for performing moderately complex and/or specialized patient-oriented procedures under the supervision of a clinician or Registered Nurse (RN) to ensure that patients receive appropriate care. Responsible for performing operational duties as required under the supervision of the site administrator or designee.
Primary Responsibilities:
- Directs patient to proper exam/treatment area and prepares patient for examinations and other procedures. Assists in the transportation of patient and patient flow
- Obtains records and reports vital signs and any other pertinent patient information to the RN or clinician
- Administers medication, performs various patient tests, and instructs patients on procedural preparations under the supervision of the clinician or RN
- Notifies pharmacies of new prescriptions and authorizations of new refills as ordered by the clinician
- Collects, labels, and transports various specimens to appropriate destinations
- Maintains and cleans exam/treatment areas and medical equipment
- Maintains logs, records of medical procedures, and inventories of supplies
- Performs telephone screening
- Performs additional duties as assigned

Required Qualifications:
- Graduation from an accredited nursing program
- Current Washington State Practical Nursing license
- Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross)
- 1+ years of experience working as an LVN/LPN
Preferred Qualifications:
- I.V. therapy and phlebotomy certification
- 3+ years of experience working as an LVN/LPN
