Overview:
The Prevention Navigator is the main link among clients, agency staff and medical care providers. This will be done by conducting assessments, enrolling clients into case management programs and navigating them through primary care, providing health education and various other services. Navigators build relationships, solve problems and locate resources. Prevention Navigators promote the recovery, independence, health and wellbeing of service users. This position will work in a drop-in space offering low-threshold services while providing care, intake and risk assessments and connect to testing, case management as well as higher threshold programs The Prevention Navigator must be knowledgeable about community resources, including educational and social support services available to clients. Duties may also include recruiting clients, administering questionnaires, data gathering, maintaining patient databases, assisting with event organizing, and performing various administrative tasks.
Responsibilities:
Primary:
- Conduct initial intake and ongoing assessments to evaluate participants' health status, wellness goals, and experiences with accessing healthcare.
- Provide health education, promotion, and risk reduction counseling tailored to individual needs.
- Facilitate harm reduction-focused group sessions that provide education, peer support, and skill-building opportunities, fostering a safe and inclusive space for participants to share experiences and strategies.
- Maintain regular communication with clients and collaborate with agency staff to support care coordination and goal achievement.
- Engage clients consistently to foster trust, monitor progress, and conduct follow-ups as needed.
- Assist clients in navigating the healthcare system, including scheduling appointments, accompanying them when appropriate, and supporting effective communication with medical providers.
- Conduct syringe transactions in accordance with program protocols, ensuring safe distribution, disposal education, and respectful engagement with participants.
- Document all services provided in a timely and accurate manner, ideally within two business days of service delivery.
- Attend meetings as deemed appropriate and related to the scope of service.
- Adhere to workplace policies and procedures, including confidentiality, documentation, channels of communication, workplace culture, dress code, and conflict resolution.
- Provide data entry support for any programmatic data needing to be submitted into databases.
- Conduct or be able to learn rapid testing for HIV and HCV Participate in quality assurance and quality improvement planning project and activities as directed

POSITION SUMMARY:
CHASI’s Prevention Department focuses on seeking out and coordinating outreach, education and referrals for priority populations to address a broad range of needs to improve overall health and wellbeing and decrease acquisition of HIV/HCV and STIs through comprehensive effective strategies. At the core of the service delivery model, Navigators play a critical role in these efforts by aiming to address the barriers clients face in accessing the services they want and need. Navigators will provide case management and referrals services for clients in the target populations in order to provide comprehensive HIV prevention services.
This is a full time (40 hours per week) position requires a flexible schedule such that some of the job tasks occasionally, with notice, will be conducted during evening hours and on weekends.
DUTIES & RESPONSIBILITIES:

Ability to navigate all referrals from CHASI Testers, program Coordinator, and other staff of priority population clients.

Conduct brief intakes, screenings, and linkage & navigation documentation needed to identify the full complement of needs for enrolled persons from priority populations.

Educate and counsel persons on a variety of prevention education, strategies, and materials according to client presentation and needs.

Develop, coordinate, and integrate service plans in collaboration with clients.

Assist clients with appointment scheduling and provider follow ups; plan logistics, provide reminders, assist with transportation, and accompany clients to appointments as needed

Perform and maintain effective case management for a caseload of clients, as assigned, from intake to discharge.

QUALIFICATIONS:

Summary
Under the supervision of the Senior Prevention and Strategic Partnerships Director, the Prevention Educator develops and implements educational and outreach presentations, programs, and activities focusing on healthy relationships, intimate partner violence, teen dating violence, bullying, etc. The Prevention Educator travels and works primarily in Orange County, collaborating with school districts, walk-in centers, local universities, faith-based organizations, and community groups. The Prevention Educator maintains and establishes new community partnerships to drive education and outreach for violence prevention education and awareness. The Prevention Educator recruits participants to join groups and to assist in the agency's domestic violence outreach and prevention education efforts throughout target communities.
Duties and Responsibilities
Program Responsibilities
- Prevention Education
- Secure, develop, and facilitate presentations and groups on intimate partner violence and teen dating violence for youth and adult audiences.
- Conduct educational workshops for first responders, including law enforcement, medical professionals, and salon professionals.
- Support the execution of campaigns, including but not limited to, Domestic Violence Awareness Month, Teen Dating Violence Awareness and Prevention Month, Child Abuse Prevention Month, Sexual Assault Awareness Month, Elder Abuse Awareness Day.
- Facilitate the 40-Hour DV Advocate Training and the Personal Empowerment Program (PEP).
- Update and maintain databases by recording activities, participants, outcomes, and perform quality assurance checks and compile reports.
- Digital Prevention & Communications
- Plan and ensure media postings align with agency branding guidelines to increase followers and engagement.
- Create and gather content to regularly update social media across platforms, aiming to grow our online footprint.
- Create visual and/or graphic materials, draft captions and messaging, edit and review content, and obtain final approval for all e-communication pieces.
- Responsible for gathering stories, drafting content, finalizing layout, obtaining final approvals, and ensuring regular content execution.
- Bachelor's degree or commensurate work experience preferred.
- Bilingual English and Spanish preferred.
- One year of experience in a domestic violence program preferred.
- Excellent written, verbal communication, public speaking, and interpersonal skills.
- Ability to travel to various locations within Orange County.
- Creativity and motivation to positively impact the local community.
- Proactive, take initiative, and adaptable in a dynamic work environment.
