+ Provide a full scope of medical services largely geared to underserved populations
+ Receive strong clinical support services and work in an innovative practice environment utilizing Advanced Access scheduling
+ Be part of a multi-disciplinary practice that offers a blend of individual and population based care
**Physician Qualifications:**
+ MD/DO from approved medical/osteopathy school and graduation from accredited residency program in family practice
+ Must be board-certified within 24 months of commencing employment at CHC and hold license or be eligible for license in State of CT
+ Bi-lingual skills English/Spanish preferred
+ We do accept NP's for this position
**APRN Qualifications:**
+ Graduate of an accredited Master of Science in Nursing program with a specialty as a Family Nurse Practitioner ANA certification as a Family Nurse Practitioner
+ State of Connecticut APRN and RN licensure (within 12 months) required.
+ At least 12 months of outpatient experience
+ Bi-Lingual Skills English/Spanish preferred
+ Certifications Required: Medical - State of Connecticut Licensure
**Physician Assistant Qualifications:**
+ Graduate of an accredited Master program in the study of physician assistant.

With a Level IV trauma center, an on-site lab and imaging center and an on-site pharmacy, Mt. The Primary Care Physician (PCP) in our organization demonstrates:

We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP's become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
+ The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
+ The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
+ It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
+ The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
+ The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
+ For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
+ PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
+ Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
+ Performs other duties as assigned and modified at manager's discretion.

Your clinical support includes the following:

• Providing primary care services for a wide range of adult patients with both acute and chronic conditions.
• Collaborating with a multidisciplinary team that includes physicians, nurses, care coordinators, and specialists to provide holistic care.
• Managing patient care, conducting physical exams, performing diagnostic tests, and interpreting results.
• Educating patients and families on managing chronic conditions and promoting preventive care.
• Participating in patient care planning and follow-up to ensure continuity of care.

Qualifications:

• Current and valid Illinois license as an Acute Care Nurse Practitioner (ACNP), Adult Gerontology Acute Care Nurse Practitioner (AGANP), or Physician Assistant (PA-C), or ability to apply for license prior to start
• Experience in Internal Medicine or a primary care setting preferred.
• Strong clinical decision-making, diagnostic, and communication skills.
