- Primary Diagnosis: Holoprosencephaly
- Requires maximum support with all ADLs
- Requires:
- Prolonged G-tube feedings
- Respiratory treatments

- Attends church with her mother on Sunday mornings (optional accompaniment)
You'll be supported by client's admissions nurse , and integrated into a well-coordinated care team.
What You'll Do:
- Provide direct, high-quality care in a home environment
- Monitor and manage complex care needs including respiratory and nutritional support
- Maintain accurate documentation and communicate effectively with the clinical team
- Support the client in attending regular activities as desired by the family
Qualifications:
- Active Colorado RN or LPN license (required)
- Current CPR certification (required)

PASCO is currently seeking a compassionate and dedicated RN to provide personalized one-on-one care for a bilingual (Spanish) female client under 10 years of age. This client lives in North Glenn area and requires respiratory and GB cares. This dayshift position can be 10 or 12 hours on Saturday or Sunday. Typically shifts needed are either 10 or 12 hours in duration (i.e. 8a - 6p). Spanish speaking RN highly preferred.
POSITION
Private Duty Nurse (PDN)
POSITION PURPOSE
Provide direct, comprehensive nursing services for a single female client on an hourly basis. Additionally, this role will provide client education, complete and submit all required documentation, and may supervise or direct other assigned care providers.
KEY DUTIES & RESPONSIBILITIES
Provide skilled nursing services per physician orders, aligning with standards of practice and regulations:
- Interpret and execute physicians' orders promptly and accurately.
- Conduct comprehensive client assessments, initiating preventive and rehabilitative procedures as needed.
- Support clients in achieving care goals and adjust goals based on changing conditions.
- Build and sustain relationships with clients and their in-home support systems.
- Coordinate with the RN Case Manager, family, physician, and PDN staff to ensure high-quality care.
- Maintain and update in-home charts and medication records per PASCO policies.
- Report lab and diagnostic results to relevant care providers.
- Participate in case conferences, sharing pertinent information with the RN Case Manager.
- Understand and follow the client's individualized Plan of Care, recommending updates as needed.
- Complete and submit all required documentation per regulations and agency guidelines.
- Supervise and guide assigned LPNs and aides, ensuring compliance with regulations and policies.
- Provide training and feedback to LPNs, CNAs, or other care providers.
- Ensure CNA supervisory forms and care quality standards are met within timelines.
- Educate clients and caregivers, referring them to community resources when appropriate.
- Maintain effective communication with team members and nursing administration.
- Notify the Nursing Supervisor of significant changes (e.g., wounds, infections, hospitalizations).
- Attend nurse meetings and seek additional knowledge as required.
- Address clinical, compliance, or special circumstances proactively.
- Follow HIPAA regulations, ensuring confidentiality of client records.
- Comply with State, Federal, and PASCO guidelines.
- Perform other duties as assigned by nursing administration.
EXPERIENCE
- 1-3+ years experience practicing as a PDN providing in-home care (highly preferred)
- Documented experience and skill set matching client/family needs
- Home Health nursing experience (highly preferred)
- Active and current drivers license/insurance
EDUCATION/LICENSES
- Current unencumbered license to practice as an LPN or RN in the state of Colorado (confirmed through DORA)
- Current BLS Certification

The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, following the developed care plan and physician's orders while maintaining confidentiality and professional boundaries.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient conditions, reporting changes as necessary.
- Document all activities, assessments, and nursing actions in a timely manner.
- Participate in the implementation and updating of the nursing care plan.
- Recognize changes in patient needs and take appropriate nursing actions.
- Educate patients and families on disease processes and self-care techniques.
- Maintain knowledge of nursing competencies through ongoing education and training.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Proficiency in clinical assessments and compliance with nursing policies.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience is a plus.
- Attention to detail and effective time management.
- Strong problem-solving and conflict resolution skills.
- Excellent organization and communication abilities.
- Ability to speak, write, read, and understand English.
- Must be able to travel and lift up to 50 pounds.
- Prolonged walking, standing, bending, kneeling, and reaching are required.
- Must be able to respond appropriately to emergency situations.
