This position is full-time, Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7:30am - 3:30pm It may be necessary, given the business need, to work occasional overtime.
We offer 8 weeks of paid training. The hours during training will be 7:30am to 3:30pm, Monday - Friday.
**Primary Responsibilities:**
+ Act as a Subject Matter Expert for other team members
+ Manage the intake of members or the admission/discharge information post notification
+ Work with hospitals, clinics, facilities and the clinical team to manage requests for services
+ Manage the referral process, processing incoming and outgoing referrals and prior authorizations, including intake, notification and census roles
+ Resolve inquiries from members and/or providers
+ Part of the challenge here is dealing positively with members and providers in sometimes challenging circumstances. As a subject matter expert, you'll also be the "go-to" resource for information. The flexibility to work evening hours will also be required

**Required Qualifications:**
+ High School Diploma / GED
+ Must be 18 years of age OR older
+ 1+ years of experience analyzing and solving customer problems
+ 1+ years of experience working in the health care industry
+ 1+ years of experience working with medical terminology
+ Valid Driver's License
+ Ability to keep all company sensitive documents secure (if applicable)
+ Dedicated work area established that is separated from other living areas that provides information privacy
+ Residing in a location that can receive a high-speed internet connection (additional information on UnitedHealth Group approved internet providers and restrictions will be provided)

**Preferred Qualifications:**

**PURPOSE** **AND** **SCOPE:**
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
**Education/Communication:**
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application in an accurate and timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
**INFECTION CONTROL:**
+ Assists in collecting information for infection control audits.
+ Supports staff and patient Adherence to infection control practices.
+ Follows infection control Policies and Procedures
+ Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.
**TRANSITIONAL CARE UNIT:**
+ Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care
+ Reinforces all education and care related matters as it relates to the new patient as allowed by state law
+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.
+ Assists with supply ordering and inventory for TCU

This is a per diem position, serving Spokane, WA. Per diem employees are required to have availability for a minimum of 10 hours per week with a maximum of 960 hours a year. Flexible schedule - days, evenings, weekends.

**Primary Responsibilities:**

**Required Qualifications:**
+ Active and unrestricted license in the state which you reside
+ Certified Nurse Practitioner through a national board
+ For NPs: Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult/Family or Gerontology Nurse Practitioners (ACNP), with preferred certification as ANP, FNP, or GNP; or for PAs - Physician Assistant National Certification through NCCPA
+ Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
+ Access to reliable transportation that will enable you to travel to SNF facilities within a designated area
+ Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment
+ Availability to work 10 hours per week
+ Ability to gain a collaborative practice agreement, if applicable in your state
**Preferred Qualifications:**
+ 1+ years of hands-on post grad experience within Long Term Care
+ Understanding of Geriatrics and Chronic Illness
+ Understanding of Advanced Illness and end of life discussions
+ Proficient computer skills including the ability to document medical information with written and electronic medical records
+ Demonstrated ability to develop and maintain positive customer relationships
+ Proven adaptability to change
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). **Job Details**
**Compensation:** Towne Park is committed to offering competitive, fair, and commensurate compensation. Actual compensation will be based on a candidate's job-related skills, experience, education or training, and location. The annual base pay range for this position is $150,000-165,000.
**Additional Compensation:** Employees may be eligible to receive annual incentive bonuses depending on their job classification and the policy guidelines.
**Benefits:** Employees are eligible to enroll in medical, dental, and vision insurance, accident insurance, critical illness insurance, hospital indemnity insurance, and telemedicine benefits. Employees are provided company-paid basic life and AD&D insurance as well as short-term and long-term disability. Employees are also able to enroll in the company's 401k retirement savings plan.
**Paid Time Off:** Employees accrue 0.0654 hours of PTO per hour worked up to a maximum of 136 hours per calendar year. Employees receive 6 paid holidays throughout the calendar year and employees accrue up to a maximum of 4 paid floating holidays per calendar year.

**SUMMARY**
The District Manager directly oversees a specific Towne Park district and is responsible for district performance for financial, guest satisfaction, client satisfaction and business development objectives. The District Manager works closely with the Vice President of Operations/Regional Director (depending on division) to ensure that corporate initiatives and Towne Park cultural perspectives are incorporated within the district's operations.
**ESSENTIAL FUNCTIONS**

**Descriptive Statement(s) - % of Time**
Maintains knowledge of operational requirements and business reporting procedures affecting operational functions and ensures Towne Park policies, procedures, and reporting are in compliance throughout the district. Directs the activities and effectiveness of Account Managers and other district support to meet company and client objectives. Assigns and trains managers in all aspects of their responsibilities to properly run a contract site in accordance with contractual obligations, legal obligations and the policies of Towne Park - 25%
Understands and drives business metrics and performance measurements to ensure effectiveness, high performance and compliance within the district. Assesses work processes regularly to ensure functionality and efficiency. Conducts weekly district staff meetings and other meetings as needed. Ensures that all Account Managers meet deadlines for forecasting and reporting the proper information to corporate accounting for closing each month's business accurately - 25%
Works closely with managers to help them understand how to improve the performance of their sites. Ensures Account Managers understand and meet the expectations of both clients and Towne Park. Collaborates with the Sales department to identify, pursue, and secure business development opportunities - 25%
Maintains regular contact with Account Managers to determine their progress in meeting the essential goals and objectives of staffing and scheduling at their sites. Meets regularly with the Manager of Talent Operations to analyze labor trends and staffing levels to accurately forecast staffing needs. Partners with Talent Team in the administration of the district's recruitment initiatives. Participates in the recruitment and development of management talent to support growth and succession planning within the district and throughout the company - 25%

**QUALIFICATIONS**
**Education:**
+ Bachelor's degree from a four-year college or university
**Required Licensure, Certification, etc.:**

**Work Experience:**
+ Minimum of seven (7) years of related experience and/or training; OR
+ Equivalent combination of education and/or experience
**Knowledge:**
+ Knowledge of general business practices including accounting, human resources and client relations
**Skills:**
+ Ability to effectively plan, set priorities, and manage several complex projects simultaneously while working under pressure to meet deadlines
+ Computer proficiency and technical aptitude with the ability to utilize MS Office (Excel, Word, PowerPoint and Outlook) and various technology applications
