Responsibilities:The Physician Assistant provides medical services/ patient care to patients admitted to ARS facilities and throughout treatment within the continuum of care. The Physician Assistant supports and follows the policies, rules, and regulations of the facility as well as the standards of State and Federal regulations.
Works effectively with the facility leadership team to ensure success of the facility by completing the following:
JOB DUTIES:
- Provides excellent medical care with the ARS philosophy outlined by the Chief Medical Officer and Medical Director. This includes knowledge of medications, assessment for medication complications and detoxification protocols, and prescribing medications in general.
- Demonstrates proficiency using the electronic health record with all required documentation. ensuring compliance with all company policies and procedures, state regulation, Joint Commission standards and Medical Needs Criteria. These records will be completed and authenticated in a timely manner per policy.
- This includes but is not limited to the following electronic record documents:
a. Admission History and Physical Evaluation
b. Medical Progress Notes
c. Discharge Summary
d. Peer to Peer Form
e. Medication and ancillary orders
f. Patient Allergies
g. Diagnosis/ update diagnosis, laboratory Data interpretation and documentation
h. HIV results positive or negative must be reviewed and documented in the medical record. STD positive results must be communicated to nursing so they can report to the State Health dept as applicable and reviewed with patient to to consider treatment options.
- Understands ASAM criteria and Medical Needs Criteria provided and will document decision making accordingly
- Screen for Substance Use Disorders (SUD) including: psychoactive substance toxicity, intoxication, and withdrawal symptoms.
- Screen for pain and understands treatment strategies for pain including pharmacologic and nonpharmacologic approaches.
- Strategies should reflect an individualized approach based on the individual's current presentation, medical judgment and the risks and benefits associated with strategies , including potential risk of dependency, addiction, and abuse.
- Screen for nutrition deficiencies and make interventions or refer for a nutrition consultation as appropriate.
- Review any physical examination reports and medical screening results conducted off-site of a client for the preadmissions process or for other medical concerns, in order to ensure that the client's medical needs are considered and addressed in the development of the treatment plan and throughout treatment;
- Understands and communicates the On-Call responsibilities; will provide contact and back-up numbers, being responsive and available not only when assigned on-call duties, but as backup when medical provider/Medical Director on-call is unavailable. When on call, will be within one hour of the facility in case an emergency that requires going to facility.
- Implements plans for Performance Improvement as needed.
- With the Medical Director, develop cost-effective methods to provide medically necessary therapeutic services.
- Assists in formulating and supporting the highest standards of patient care.
- With the Medical Director, supervise all Level of Care changes and provides peer reviews when required.
- Provides supervision/ support to Physician Assistants as required.
- Understands and abides by all safety/risk, infection control, and other guidelines, as required.
- If Non-Exempt-turns in time sheets/ expenses to the Medical Director/Designee at the designated time frames.
- Performs other tasks and duties as assigned.

• Collaborate, coordinate and consult with other providers such as counselors, psychologists, social workers and medical staff of other local providers, including but not limited to state hospitals, local hospitals, other community agencies, primary healthcare providers, CCNC, public Health Departments, Federally Qualified Health Clinics in order to discuss treatment plans, progress in services and where appropriate, the medical aspects of the person being supported and the implications for their physical healthcare needs.
• Participate in staffing of complex or high-risk individuals with other program staff.
• Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.

Overview
Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city.
Responsibilities
Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. The mental health service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units ("mental observation units") for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, outpatient-level clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Physician's Assistant will be matched with the population that most closely meets their clinical expertise and desired work location and schedule. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs.
- Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
- Evaluate patients for the need for a higher level of care, including hospitalization. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care
- Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner
- Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.
- Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication
- Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed
- Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.
- Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist
- Provide clinical oversight for psychiatric physician’s assistant trainees as needed, including appropriate direct supervision and documentation of evaluation and feedback
- Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees
Department Preferences
- Demonstrate advanced knowledge of psychopathology in assessing patients in need of mental health treatment, including mental status examination and diagnosis.
- Demonstrate advanced knowledge of psychopharmacology as it pertains to the patient population being treated and writing prescriptions.

Part-Time Psychiatry Medical Director Positions Available - Florida, Massachussetts, Pennsylvania
1) 40 MILES FROM PITTSBURGH
Medical directorship role near Pittsburgh, PA. Small detox, residential, with plans to open up PHP. We have a dual certified NP seeing patients daily there, looking for someone to go on site 1-2 times per month, be available for treatment team, signing off on records, supervision, etc. Monthly stipend is $5k/month. May have additional work nearby or other locations in the future, not just yet.
2) SUBURBAN BOSTON & CAPE COD OPTIONS
Massachussetts Options are for three sites - detox/residential in Suburban Boston, PHP/IOP in Suburban Boston, another IOP in Cape Cod. Main site to be visited 1-2 times per month would be in Suburban Boston. Same requirements as above. We have several midlevels doing the work at these sites. Also $k per month. Both of the roles should average about 20 hours per month.
3) PALM BEACH, FL
Medical Directorship Role - Weekends in Palm Beach are just inpatient rounds in a psychiatric hospital. We just need additional weekend help, minimum of 2 weekend days per month, ideally 3-4. 20+ census, may cover the main site and consults at another. At the main site we have residents and midlevels to assist. Pay is no more than 1500 per day.
