Putnam Hospital Center in Carmel, New York, just one hour north of New York City, is seeking a per diem General Surgery Physician to provide additional call coverage for a busy and growing group. Located in a suburban community, the hospital is a 164-bed acute care facility offering the highest quality medical, surgical, psychiatric, pediatric and obstetrical/gynecological care, as well as 24-hour emergency services. The hospital has recently completed a $34 million addition and has all single-patient rooms.
This is an employed position with Nuvance Health Medical Practice.

Medical Office RN Cardiovascular Center department in Norwalk, CT 40 hours per week, 8 hour shifts Monday-Friday 8:30am- 5:00pm.
Summary:
To assist the physician or physician assistant in providing primary medical care to the patients of the Office Practice.
Responsibilities:
1. Prepares materials, supplies, equipment and the patient to assist the physician or the physician assistant with the patient's medical exam.
2. Obtains the patients history and basic medical data such as vital signs, temperature, height and weight, vision and chief complaint.
3. Provides direct patient care, such as ear irrigation, vaccinations and medications, wound care and suture removal.
4. Collects, prepares and analyzes patient specimens as directed by the physician or physician assistant.
5. Instructs the patient and family on how to perform physician recommended preventive care, take prescribed medication, prepare for tests, monitor treatment progress and note symptom changes.
6. Contacts the patient by telephone to provide requested follow-up, advise them of test results or relay a physician's instructions.
7. Performs organizational responsibilities to achieve the Practices' and WCMG missions through a process of total quality improvement.
8. Fulfills all compliance responsibilities related to the position.
9. Performs other duties as assigned.

Minimum Experience: three years
Desired: Associates Degree preferred. Previous medical office experience preferred.

*Title:* Per Diem "Temporary" Registered Dietitian - Danbury Hospital Cancer Center - Danbury, CT. Hours: PRN, variable 8-hour shifts between 8:00am - 6:30pm Monday - Friday for 90 days.
*Summary: *Plans, coordinates, and implements nutritional therapy services for adult oncology population in ambulatory setting. As a Clinical Dietitian that practices as an expert in medical nutrition therapy, this individual will specialize in the nutrition care ofadult patients with cancer.
Responsibilities:
1. Involves working collaboratively with the interdisciplinary team in the ambulatory caresetting to guide and support patients' nutrition care throughout and beyond their cancer treatment.
2. Develops andmaintains a comprehensive nutritional plan of care for cancer patients and their families and aids in the adjustment to living with a chronic and life-threatening illness.
3. Use own knowledge and skills to identify and re-evaluate nutrition risk throughout treatment course; independently perform thenutrition care process, including nutrition screening, assessment, diagnosis, intervention, monitoring, and evaluation, specific to
* Age and medical status.
* Provide nutrition education and symptom management consultation for oncology diagnoses and side effects of treatment.
* In collaboration with team, develop and communicate an interdisciplinary nutrition plan of care.
* Assess, coordinate and provide diet therapy and nutrition education activities for the Cancer Center.
* Plan and provide nutritional care to patients. Evaluate and assesses patient responses to diet therapy though direct interview, calculating nutrient intake, and monitoring tolerance of foods or nutrition support (tube feeding, TPN/parenteral nutrition).
* Possess a clinical understanding of metabolism in relation to clinical nutrition; knowledge of food chemistry, macro/micro nutrient composition of foods, and digestion.
* Demonstrate a high level of analytical skills to interpret laboratory data, identify, plan, and assess diet plans and their physiologic effect.
* Possess the interpersonal skills to interact with a diverse patient population, as well as effectively interact with physicians, fellows, nursing team, etc.
* Function as a preceptor in support of the dietetic internship program, as needed.
* Participate in community events and outreach initiatives, program development and evaluation, and any other assigned responsibilities relating to nutrition and cancer.
*Other Information:*
*Education Required: *Bachelor degree in Nutrition Registered Dietitian with 2 years' minimum experience.
Currently Licensed Dietitian/Nutritionist in the State of Connecticut.
Prefer: Masters Degree in Nutrition.
Prefer: Current Oncology Nutrition Certification (CSO).
Salary Range: $28.78 - $53.45 Hourly (Pay per years of relative experience)
