Requirements:

Case Managers must have the following:

• Bachelor's Degree (at minimum) in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
  
  • If degree is in social work, social work licensure is required.
• Either one year of experience with DD (Development Disability) populations or offer will be contingent on taking/passing the Relias I/DD course (paid expense by Fraser).
• Experience (or at least a strong interest) in working with adults with serious and persistent mental illness
• Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness and reflection.

• Valid Minnesota Driver’s License with acceptable record.

• • CADI/BI WAIVER Case Managers: provide client-centered case management services to individuals receiving CADI/BI waiver services. Case Manager typically have about 35 clients on their caseloads and meet with clients two to three times per year. Caseloads are a majority of adults, with a small mix of children as well.
  • DD WAIVER & NON-WAIVER Case Manager: provide client-centered case management services to individuals receiving DD services. Case Managers typically have about 43 clients on their caseloads and meet with clients two to three times per year. Caseloads is a mix of children and adults.

Position Summary:

The MFIP Career Counselor serves as an advocate for participants in the Minnesota Family Investment Program (MFIP), providing employment support to participants through ongoing individualized career and case management services. Each MFIP participant faces unique challenges and requires individualized solutions to support their career pathway. MFIP Career Counselors serve participants flexibly, including in-person, virtual services, occasional travel to meet participants in the community, and maintaining contact through frequent outreach. MFIP Career Counselors complete and maintain essential documentation within established timelines and in accordance with applicable procedures, policies and requirements.

Day in the life : In a typical day, a MFIP Career Counselor can expect to.

• Individualized Career and Case Management Services : Provide employment and career advancement services to participants through ongoing case planning,
  individualized and personalized goal setting.
• Case Management Coordination : Ensure comprehensive and optimal service delivery for all individuals by serving as a main contact and liaison
  for the individual and cooperating partners, persons or agencies. Coordinates with county partners regarding cash, food, health care and childcare assistance for participants
• Documentation : MFIP Career Counselors complete essential documentation in areas such as fiscal information, an electronic case file, and case noting regarding participant needs, services provided, and participant progress. Maintains all individual program files in accordance with applicable procedures, policies and laws.

Job Pay & Perks:

• Typical starting pay range: $25.24 - $7.04
• This is a scheduled-hybrid role whose essential functions require work to be conducted at a physical location, which may include a GESMN or partner facility, at least two (2) days per week. The remaining days can be worked from the employee's home office or a designated community setting as directed by the manager.

• Evaluate and modify program design for the purpose of meeting licensure requirements and needs of clients.

• Responsible for complying with federal and state statutes, upholding ethical standards established and implied by the profession, DHS regulations, etc.
• Participate in weekly treatment team meetings to review and update client progress notes.
• Initiate and maintain ongoing communication with client referrals, probation, and social service agencies, etc.
• Complete discharge summaries for each client at the conclusion of treatment services.

• Bachelor's degree required.
• Current alcohol and drug counseling license in the state of Minnesota.
• Complete continuing education courses in current treatment concepts and methods as required by the state of Minnesota to maintain alcohol and drug counselor license.
• Must be at least 21 years of age or older.
• At least 1 year of freedom from problematic substances.

Job Responsibilities and Duties:

Case Managers at Pinnacle Services have the opportunity to work with persons served on a mix of CADI and DD waivers. These persons served may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging.

The Case Manager is responsible for:

• Assessing the areas of the person served life where assistance is needed.
• Offering and coordinating resources for the person served being supported.
• Monitoring service delivery and working with providers to ensure the person served needs are met.
• Service plan development.
• Attend regular meetings with person served and their interdisciplinary teams.
• Case Noting client related work to create billable units.

Salary:

Non-LSW wage: $49,000/ year salaried for 40hrs/week.

LSW wage: $51,600/year salaried for 40hrs/week.

Requirements

Case Manager Requirements:

• Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field or
• Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
• One year of experience in education or treatment of persons served with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
• Applicants must have a
  valid drivers license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance.
• Applicants must successfully clear local and/or stated background checks.

Preferred Qualifications:

• Possess a LSW or higher form of BOSW Licensure.
