We are seeking a compassionate and dedicated Resident Care Manager to join our team at Colonial Vista Post Acute. In this role, you will play a pivotal part in enhancing the quality of life for our residents by providing exceptional care and support. As a Resident Care Manager, you will be responsible for overseeing the daily operations of our residents, ensuring that they receive the highest standards of personalized care tailored to their individual needs. Responsibilities
- Administer patient assessments, oversee the assessment process assuring that assessments are done in an accurate and timely manner.
- Develop, implement, and monitor individualized care plans for residents.
- Supervise and train staff members to ensure high-quality care delivery.
- Coordinate with healthcare professionals to manage residents' medical needs.
- Conduct regular assessments of residents' physical, emotional, and social well-being.
- Facilitate communication between residents, families, and staff to address any concerns or needs.
- Ensure compliance with all regulatory requirements and healthcare standards.
- Assist in planning the nursing services portion of the resident's discharge planning as necessary
- Admit, transfer and discharge residents as required.
- Complete resident accident/injury reports as necessary and submit to the DNS.
- Complete sections of the MDS as required.
- Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary. • Review the resident's chart for specific treatments, medication orders, diets, etc., as necessary.
- Prepare and administer medications when needed.
- Dispose of drugs and narcotics as required
- Make daily rounds of your unit to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
- Develop and maintain good working rapport with inter-departmental personnel.
- Participate in morning MACC meetings, weekly PDPM meetings, monthly triple check meeting, QAPI and resident care plan meetings.
- Be on call schedule and cover shifts when needed.
Requirements
- Nursing degree from an accredited college or university
- Active RN/LPN license
- Proven experience in a healthcare management or supervisory role.
- Strong understanding of resident care best practices and regulatory compliance.
- Excellent communication and interpersonal skills to engage with residents and families.
- Ability to lead and motivate a team in a fast-paced environment.
- Compassionate and empathetic approach to resident care and staff support.
- Certification in relevant areas such as CPR, First Aid, and healthcare management preferred.
